An earlier localization ROC (LROC) study that found attenuation correction (AC) degraded the detection of solitary pulmonary nodules (SPN) in hybrid SPECT lung images had several potential shortcomings related to the simulation methods. We sought to address these issues with a revised LROC study. Clinical Tc-99m NeoTect scans acquired with a simultaneous transmission-emission protocol defined the normal cases in a single-slice LROC study. Abnormal cases contained a simulated 1-cm lung lesion. Four rescaled-block-iterative EM (RBI) reconstruction strategies applied: 1) AC, scatter correction (SC), and resolution compensation (RC); 2) AC only; 3) RC only; and 4) no corrections (NC). Images from these strategies underwent 3D Gaussian post-smoothing. Performances were defined by the average area under the LROC curve obtained from three human observers. The strategy ranking in order of decreasing performance was: 1) RBI with RC; 2) RBI with all corrections; 3) RBI with AC; and 4) RBI with no corrections. A multireader-multicase (MRMC) analysis only found significant patient and patient-strategy effects. The conflicting results concerning AC from this study and the previous one may revolve around lesion masking effects, which, by design, were not a factor in the current study.

Fundus auto-fluorescence (FAF) imaging is a non-invasive technique for in vivo ophthalmoscopic inspection of age-related macular degeneration (AMD), the most common cause of blindness in developed countries. Geographic atrophy (GA) is an advanced form of AMD and accounts for 12-21% of severe visual loss in this disorder [3]. Automatic quantification of GA is important for determining disease progression and facilitating clinical diagnosis of AMD. The problem of automatic segmentation of pathological images still remains an unsolved problem. In this paper we leverage the watershed transform and generalized non-linear gradient operators for interactive segmentation and present an intuitive and simple approach for geographic atrophy segmentation. We compare our approach with the state of the art random walker [5] algorithm for interactive segmentation using ROC statistics. Quantitative evaluation experiments on 100 FAF images show a mean sensitivity/specificity of 98.3/97.7% for our approach and a mean sensitivity/specificity of 88.2/96.6% for the random walker algorithm.

It is recommended that trilostane therapy of canine hyperadrenocorticism is monitored using an ACTH stimulation test, however this has never been validated. Three cortisol concentrations (pre-trilostane, 3-hour posttrilostane and 1-hour post-ACTH stimulation) were compared to a clinical score obtained from an owner questionnaire. There were 110 sets of 3 cortisol measurements and questionnaires obtained from 67 trilostane treated dogs. Questionnaire results were used to classify each dog as well or unwell. Well dogs were then categorised as having excellent, moderate or poor hyperadrenocorticism control, using thresholds produced by 14 independent veterinarians. Correlation co-efficients were used to compare the three cortisol concentrations to the owner score and the Kruskal Wallis and Mann-Whitney U tests were used to compare the three cortisol concentrations between categories of control. Cortisol cut-off values between significantly different categories were determined using ROC curves. Pre-trilostane and 3-hour post-trilostane cortisol were better correlated to the owner score and had cut-offs to differentiate between categories of control that had superior sensitivity and specificity results, than the post-ACTH cortisol. Iatrogenic hypoadrenocorticism was not detected in any unwell dog. This study shows that the pre-trilostane and 3-hour post-trilostane cortisol are potentially better monitoring methods than the ACTH stimulation test.
