Six cases of elastofibroma located in the subscapular region are reported. The age of the patients ranged from 45 to 71 years (mean 59.5 years). In three cases the symptoms were posterior shoulder pain with arm motion, and one of these also had a snapping scapula. Two cases had tumor prominence as the only symptom, and in one case the tumor was found accidentally while thoracotomy was being performed. On magnetic resonance imaging a nonencapsulated soft-tissue mass closely related to the thoracic wall and elevating the scapula was identified. At surgery the tumor was densely adherent to the periosteum of the ribs and the external fascia of the rib cage, and peripherally it proceeded into the loose connective tissue of the subscapular space. Local excision was performed. At follow-up 1 to 8 years after surgery no recurrence was observed, and all patients with painful lesions had complete relief of pain.

BAK-C is a new autostabilizing interbody cage which is implanted during an anterior cervical procedure to provide stability to the motion segment and allow fusion to occur. Special instrumentation is provided with a bone collecting reamer. The system utilizes surgical site bone graft as the osteo-inductive material within the implant. Biomechanical testing indicates improved stability and animal studies show good fusion. The basic principle is distraction-compression using the tension forces of the annulus fibrosus. Operative material concerns a two years experience with 80 patients (101 levels), 72 with cervical radiculopathy, 8 with myelopathy. Clinical evaluation is assessed on a ten point analogue pain scale for neck and arm/shoulder pain, with neurological examination. Radiological evaluation includes dynamic X-rays, myelo-CT and MRI. Patients are re-evaluated at 1, 6, 12 months postoperatively. Results for neck and radicular pain is excellent, but neurological recovery for radiculopathy and myelopathy is quite different. Radiological results are also good with (except one case) no instability, no cage migration, no kyphosis, no pseudarthrosis. Bone fusion is assessed at 6 and 12 months. Complications are few with proper technique, mainly correct distraction, symmetrical endplate drilling and lateral X-ray control. Only one patient needed an early re-operation with additional miniplate fixation. Immediate stability with good clinical response and no graft morbidity are the advantages of this implant compared to conventional cervical interbody grafting techniques.

Snapping scapula is a rare and poorly known cause of shoulder pain and is related to mechanical impingement between the scapula and the rib cage. We present a case in which CT scan was valuable, as a complement to clinical evaluation, for demonstrating the bony component of this entity. 3D reconstructions greatly facilitated diagnosis.

We report the case of a 25-year-old African-American man presenting to the Henry Ford Hospital emergency department with acute dyspnea secondary to a pneumothorax resulting from a migratory acupuncture needle. The patient received acupuncture treatment approximately 5 years prior to this presentation for treatment of posttraumatic chronic right shoulder pain. Chest radiography revealed retained needles in his right shoulder girdle and a needle overlying the thoracic cage with an attendant pneumothorax. Catheter aspiration for simple pneumothorax provided immediate symptomatic relief. Video-assisted thoracoscopy was then used to remove the migratory acupuncture needle from the chest wall. The patient recovered without complication and was discharged to home.

To clarify anatomy-related risk factors in the cervical spine with subsidence of titanium mesh cage (TMC) after one-level cervical corpectomy and fusion, we have assessed the radiological examinations and clinical outcomes for 236 patients. All the patients were underwent one-level corpectomy and TMC fusion between August 2003 and March 2006. The effects of the cervical posture, segmental curvature and endplate gradient on the postoperative phenomenon for these patients were evaluated. Our results suggested that in the patients who were followed up for 12 months, TMC subsidence occurred in 54 (28.6%) cases. C6 corpectomy had a significant higher risk (26/60, 43.3%) for TMC subsidence, which was correlated with the variation of the gradient of the vertebral endplates against cervical levels. Although the clinical outcome was comparable with those in the literature, the patients may have subsidence-related problems such as neck-shoulder pain, neurological deterioration and instrumental failure. In conclusion, to reduce the incidence of subsidence, TMC design should be optimized to be in line with anatomic characteristics of the cervical spine.
