During the period 1960 to 1978, 98 patients underwent intracardiac repair of Fallot's tetralogy after palliative operations. Preoperative symptoms were cyanosis, dyspnea, increased
fatigue
with squatting and hypoxic spells. The
hemoglobin
concentration varied from 19 to 22 g/100 ml. At correction only 65 of 95 shunts were patent and needed surgical closure. Seventeen early deaths occurred (19%), the main causes being cardiac failure and arrhythmia. One patient died 3 years after correction from pneumonia. The subjective clinical result was excellent or good in all surviving patients. At repeat heart catheterization in 26 patients a high percentage of residual ventricular septal defects and pulmonary stenosis/insufficiency was found. However, the majority of defects were of minimal haemodynamic significance, and so far did not seem to do harm to the patients' subjective function.

The efficacy of a new estrogen-progestin contraceptive agent Ovidon was evaluated in 65 women. The women were taking the contraceptive for 47 cycles (each Ovidon pill contained 0.25 mg of D-norgestrel and 0.05 mg of ethinyl estradiol). Prior to the Ovidon treatment, 15 women did not use any contraceptive agents or devices, 5 used hormonal contraceptives, 2 used IUDs, 4 used mechanical devices, 20 practiced coitus interraptus, and 9 used a biological method. Ovidon administration provided a 100% contraceptive effect. Side effects of Ovidon included
fatigue
(15 women), headache (4), irritability (3), fullness of the breast (27), hemorrhage (12), vaginal discharge (6), and changes in libido (22). Ovidon administration did not induce amenorrhea. A 1-3 kg weight gain was observed in 5 women. Laboratory studies showed no changes in
hemoglobin
levels, leukocyte count, cholesterol, and alkaline phosphatase levels. These findings indicated the safety and contraceptive efficacy of Ovidon.

A good tumoricidal activity of vindesine (VDS) has been reported in a variety of animal tumors and in human leukemias and lymphomas. We treated 22 patients who had received no prior chemotherapy and were suffering from a variety of malignant neoplasms with 0.5 mg/m2 to 3.0 mg/m2 VDS i. v. once or three times at weekly intervals and recorded the clinical, hematologic, and especially, neurological side effects. Clinically we observed
fatigue
in nine patients, paresthesias in seven, myalgias in three, vertigo and diarrhea in two, and skin pains, tinnitus, gastric pains, alopecia, and tremor in one patient each. There was no obvious dose-action relationship. Paravenous injection caused cellulitis similar to that seen with vincristine. No side effects were apparent in liver (SGPT) and renal (creatinine) function tests. Hematologically there was a clear trend toward leukopenia with higher doses of DVA and a mean increase in the thrombocyte count by 51 X 10(3)/mm3 was found (sign test: P greater than 0.05). The

level did not change. Clinical neurological examination and monitoring by electroneurography revealed no changes in tensiometer performance, motor and sensory nerve conduction velocity, motor or sensory nerve action potential amplitudes, or H-reflex responses. There was dose-related diminution of the proprioceptive reflexes, especially in the lower extremities. Even with as little as 2.0 mg/m2 VDS i. v. at weekly intervals for 3 weeks Achilles and patellar tendon reflexes were diminished or absent in all patients.

Total of seven teenagers and two adults were given sweet potato diets that supplied slightly below requirement levels of protein for 32 (experiment 1) or 53 days (experiment 2) in two separate experiments. In experiment 1 and during the first 36 days of experiment 2, nitrogen (N) balance studies with the teenagers were conducted with 0.67 and 0.71 g of protein per kilogram of body weight, respectively. Results of the N balances including skin N loss were -0.5 mg N/kg per day in experiment 1 and -3.2 mg N/kg per day in experiment 2. Two adults were given 0.63 g/kg of protein and gave average N balance of +6.0 mg N/kg per day (experiment 2), however, one of them had a slightly negative cumulative N balance if miscellaneous N losses were included in calculation. Plasma urea N of both teenagers and adults decreased significantly from 8 to 11 to 2 to 3 mg/100 ml in experiment 2. After 32 or 53 days on the sweet potato diets, the plasma free amino acid pattern of the teenager subjects showed abnormality. Furthermore, the subjects were easier to get

by physical exercise, and took longer nap due to sleepiness during the latter days of the experiments. Thus, although the mean

, hematocrit, plasma total protein, and plasma albumin levels were within the normal ranges, the protein nutrition status of the teenagers and one of the two adults was considered to be not normal. The results suggested that intestinal N-fixation that was proposed by several investigators to occur in sweet potato eaters probably did not occur. The subjects' feces were examined in vitro for the ability of N-fixation with negative results. During the last 17 days of experiment 2 the effect of excess calorie on N balance was examined.

Fatigue
and headache were common, and syncopal attacks occurred in four patients. Symptoms disappeared and the elevated red-cell volume decreased markedly in all five patients who were able to reduce severely their smoking habit; low plasma volume increased in three of four patients studied. We conclude that carbon monoxide exposure from cigar and cigarette smoke is a frequent cause of an elevated red-cell volume or a reduced plasma volume (or both). Measurement of carboxyhemoglobin should be a routine part of the evaluation of all polycythemic subjects.

A subgroup of workers from a secondary lead smelter was defined to include those with blood lead levels not exceeding 80 microgram/100 ml and with no past history of elevated blood lead. Central nervous system symptoms (
tiredness
, sleeplessness, irritability, headache) were reported by 55% of the group and muscle and joint pain by 39%. Zinc protoporphyrin (ZPP) levels were elevated in 71% of cases. Low
hemoglobin
levels (less than 14 gm/100 ml) were found in more than a third of the workers. While BUN and creatinine were mostly in the normal range, there was nevertheless a correlation between ZPP and both BUN and creatinine. Reduced nerve-conduction velocities were present in 25% of the group; this was not significantly different from findings in a control group. The data indicate that a blood level of 80 microgram/100 ml is an inappropriate biological guide in the prevention of lead disease.

A pilot study was conducted to determine the differences in frequency of physical complaints between 179 Black patients (112 oral contraceptive (OC) users and 67 IUD users) with sickle cell trait (AS) and those without AS. 6 physical complaint types were analyzed: dysmenorrhea; mental depression; vaginal discharge;
fatigue
; headaches; and nausea-vomiting. Out of 5690 Black patients with
hemoglobin
AA, 112 OC users and 67 IUD patients with similar age and parity were randomly selected to serve as controls. Cumulative complaint rates (CCR) were measured in comparing the experiences of AS and control patients in the 6 complaint categories. No significant differences between AS and control CCRs at level .05 for any of the 6 complaints were observed for both the OC and IUD groups. For OC users, AS patients reported higher CCR's than control patients for cramps and mental depression while for IUD users, AS patients had higher CCR's for mental depression. The relationship between OCs and mental depression is not clear yet, and further research would benefit AS patients. Another major concern for OC users is the possible development of hypercoagulable state with resultant thromboembolism. Also, evidence suggests that crisis, splenic infarction, and hematuria can develop in AS patients under special conditions; the role of OCs in enhancing the infarctive process in AS patients is not known and needs to be studied further. Further clinical research should also be done to determine the effect of contraception on dysmenorrhea and mental depression in AS patients. AS patients should be observed regularly.

Eleven patients with hairy cell leukemia (HCL) were treated with YK-176 (2'-deoxycoformycin) at a dose of 5 mg/m2 by intravenous injection every week or every other week. Patients received a median of eight (range 4-19) injections of YK-176. Five patients had previously been untreated, four of whom had massive splenomegaly. Six patients had previously been treated, four with interferon-alpha (IFN-alpha) or IFN-alpha and chemotherapy and two with prednisolone. Two patients had had splenectomies. Five patients achieved complete remission (CR) and six, partial remission (PR) according to WHO criteria (remission rate 100%, 95% confidence interval (CI) 74-100%). All six neutropenic patients recovered > 1,500/microliters neutrophils, six of seven anemic patients recovered > 12.0 g/dl
hemoglobin
and five of nine thrombocytopenic patients recovered > 100,000/microliters platelets following the treatment. According to the response criteria for HLC, five patients achieved CR, two PR and four minor response. The overall remission (CR + PR) rate was 64% (95% CI 35-85%). The CR and PR have lasted from > 30 to > 718 days (median, > 281 days) so far with no relapses. Of four patients previously treated with IFN-alpha, two achieved CR and one, PR. All patients were alive with a median survival time of > 290 days from treatment (range > 50- > 763 days). The treatment was generally well tolerated.
