The immunocompetence of the circulating lymphocytes of a patient with non-Hodgkin's lymphoma was evaluated before, during and after treatment. The immunologic features investigated were (a) the degree of blastogenic transformation induced by phytomitogens (phytohemagglutinin-M, concanavalin A and pokeweed mitogen); (b) the formation of E, EA and EAC rosettes; and (c) antibody-dependent, cell-mediated cytotoxicity. Prior to treatment the subnormal in vitro responsiveness of the cells and the very low concentrations of circulating immunoglobulins suggested immunoincompetence. A gradual shift towards normal in vitro values was observed during treatment, which corresponded to the clinical improvement in the patient's condition.

The capability to form rosettes with sheep erythrocytes (E), antibody-complement-sensitized ox erythrocytes (EAC) and autologous erythrocytes (ARFC), and surface immunoglobulin determinations were studied using 21 lymph nodes and one tonsil with pathologically-proven non-Hodgkin's lymphoma and 10 lymph nodes with benign pathology. Fourteen of 22 non-Hodgkin's lymphoma patients (64%) had a high incidence of E-rosette formation and they were further differentiated into ARFC-positive and ARFC-negative lymphomas. The clinicopathological findings of the latter were compatible with those of adult T-cell leukemia. ARFC-positive lymphoma was regarded as non-Hodgkin's lymphoma of T-cell type and one patient showed lymphoblastic lymphoma with high ARFC counts. ARFC counts were very low in B-cell and non-T, non-B lymphomas. The results from benign lymph nodes were too variable to draw any conclusion, although ARFC counts were relatively high in lymphadenitis and hyperplasia.

The measurement of enzymatic activity in the blast cell is a recent investigational technique which seems to complement immunologic measurement of cell membrane receptors in childhood lymphoproliferative malignancies. We have identified a patient with non-Hodgkin's lymphoma, not readily classifiable on the basis of either morphology or cell surface markers, i.e., E rosette and SIg negative, EAC positive. Analysis of enzymatic function, however, revealed markedly elevated ADA activity and essentially normal TdT activity. ADA positive lymphoma can arise from a primitive T-cell line. It is, thus, apparent that the morphological description of malignant lymphoid lines must be accompanied by surface marker and enzymatic data in order to properly classify cell lineage and identify appropriate therapeutic modalities.
