Levels of two fusosyltransferases were measured in plasmas of patients with
non-Hodgkin's lymphoma
at different phases of the disease. The level of a
GDP
-fucose: galactoside fucosyltransferase (EC 2.4.1.69) was found elevated in nonresponding patients and was correlated with estimated tumor burden. Enzyme levels in the normal range were found in patients in remission, maintained on chemotherapy, or unmaintained. The plasma level of a
GDP
-fucose; N-acetylglucosaminide fucosyltransferase (EC 2.4.1.68) was elevated in all individuals receiving drug therapy regardless of diesease status, but returned to normal levels during unmaintained remissions.

There has been a large, unexplained rise in the incidence of
non-Hodgkin's lymphoma
(
NHL
) in many countries. It has been hypothesised that increased exposure to solar ultraviolet (UV) radiation may have been a factor in this increase. The hypothesis that exposure to ultraviolet radiation is a factor in
NHL
can be tested by examining whether geographical variations in UV and in the incidence of the disease are positively correlated. Previous studies have given mixed results but some of these have failed to take into account confounding by socio-economic factors and the multilevel structure of data derived from several different countries. It was therefore decided to carry out a study using data on
NHL
mortality for the period 1971-80 for level II administrative units in 9 countries in the European Community. Estimated levels of solar UV and per capita
GDP
were also derived. Poisson regression models of the relationship between
NHL
mortality, UV and per capita
GDP
, taking into account the multilevel structure of the data, were fitted using the MLn package. Simple models that did not adjust for the effects of variations in per capita
GDP
or account adequately for the structure of the data produced apparent negative or quadratic associations between
NHL
and UV. However, further models show that there is a highly significant positive association between
NHL
and per capita
GDP
. Once this is included in the fixed and random parts of the multilevel model the association between
NHL
and UV becomes positive although non-significant (p = 0.081) at the conventional 0.05 level. These results underline the need to control for socio-economic factors and to take into account the multilevel structure of the data. Studies using international data that do not do this run the risk of producing misleading results.

The prognosis of relapsed or refractory aggressive
non-Hodgkin's lymphoma
(
NHL
) after front-line therapy remains poor. The development of more effective and less toxic salvage regimens remains a major challenge. Survivin is a member of the family of inhibitors of apoptosis, and survivin was associated with short survival and bad prognosis. This study was to evaluate the efficacy of
GDP
regimen (gemcitabine, dexamethasone and cisplatin) on relapsed or refractory aggressive
NHL
and various prognostic factors with special emphasis on survivin and observe the . Forty-six patients with relapsed and refractory
NHL
, intermediate or high-grade
NHL
(Revised European American Lymphoma Classification), who at least one regimen were enrolled into this study, which was carried out at Department, , Tanta University from July 2012 to July 2014. The patients were treated with
GDP
regimen (gemcitabine 1,000 mg/m(2) on days one and eight, dexamethasone 40 mg on days 1-3, and cisplatin 25 mg/m(2) on days 1-3) every 3 weeks. The efficacy and adverse events were evaluated according to the WHO criteria. All patients were assessed for efficacy and toxicity. The overall response rate was 58.7 %. Fourteen patients showed a complete response, thirteen partial responses, twelve stable diseases, and seven progressive disease. The 24-month overall survival was 50.8 %. Survivin is associated with low overall response and shorter overall survival. Grade 3 anemia was observed in four patients, grade 3 leucopenia in six patients, grade 3 neutropenia in six patients, and grade 3 thrombocytopenia in four patients. Non-hematologic toxicity included grade 3 infection in four patients. The present schedule of
GDP
showed modest efficacy and mild toxicity in patients with relapsed or refractory aggressive
NHL
.
