A series of cases of non-Hodgkin's lymphoma (NHL) were studied by morphology and flow cytometry. The DNA analysis was performed by applying a trypsin digestion method to archival paraffin-embedded samples. DNA aneuploid tumors constituted 47.4% of the total cases, and the frequency in the intermediate and the high malignant grade groups (51.9%, 76.8%) were significantly higher than that of the low grade group (17.6%; P less than 0.05). The S phase fraction (SPF) increased significantly with a high histological malignant grade (P less than 0.005). The mean SPF values of the low, intermediate and high grade lymphomas were 6.5 (0.6-18.8)%, 13.5 (3.2-37.3)% and 23.4 (4.4-41.4)%, respectively. Differences in ploidy status were not associated with survival rate. The survival rate of the cases with high SPF value in all cases of our series and low grade group of NHLs tended to become lower (P = 0.1 and 0.08, respectively). These results suggest that the flow cytometric analysis of DNA content should be a useful means for prediction of prognosis in non-Hodgkin's lymphoma.

Primary pancreatic lymphoma is extremely rare. A case of the primary pancreatic lymphoma was described. A 74-year-old man was admitted to our hospital on May 26, 1990, because of epigastralgia and 5-kg weight loss. On admission, physical examination revealed mild epigastric tenderness without hepatosplenomegaly and superficial lymphadenopathy. The serum amylase level and trypsin level were normal but serum CA19-9 was slightly elevated. The diagnosis of the pancreatic cancer (body and tail) was strongly suspected on subsequent examination by US, CT, ERP and angiography. Partial pancreatectomy and splenectomy were performed. Pathological examination of the tumor in the pancreas showed diffuse non-Hodgkin's lymphoma and B-cell origin. Peripancreatic fat was involved but peripancreatic lymph nodes and spleen were intact, which strongly suggested that the isolated lymphoma was pancreatic origin.

Histological material was studied in five unselected cases of intestinal large-cell non-Hodgkin's lymphoma, occurring in patients either with previously diagnosed coeliac disease, or with atrophic mucosa at the time of diagnosis. The morphological diagnosis in each case was centroblastic lymphoma: these tumours were composed of large cells with pale nuclei and prominent nucleoli. No phagocytosis was evident, but some cells showed considerable pleomorphism. Polykaryotic giant cells were infrequent. Immunohistochemical staining for lysozyme, alpha-1-anti-trypsin and alpha-1-anti-chymotrypsin failed to demonstrate any of these proteins in the tumour cells, although they were identified in accompanying reactive macrophages. There is thus no evidence for a histiocytic nature in these five cases. The tumours were immunoglobulin-negative. Again, polyclonal immunoglobulin could be demonstrated in reactive (plasma) cells in and near the tumour. The relevance of these immunological markers is discussed. We suggest that these tumours, and possibly some of those reported in a similar situation by other investigators, are in fact lymphocytic in origin. They are probably examples of centroblastic lymphoma, although T-cell lymphoma, rare in the gastrointestinal tract, cannot be ruled out by our immunohistological studies.

Mast cells (MC) are critical for a number of pathological conditions, including acute and chronic inflammation and tumor angiogenesis. We have previously demonstrated that in B-cell non-Hodgkin's lymphoma (B-NHL) angiogenesis is correlated with total methachromatic and tryptase-positive MC and that both counts increase in step with the increase in malignancy, whereas the role of MC in malignant lymph nodes is not fully clear. An extensive ultrastructural study has been made of representative samples of 30 B-NHL and 10 benign lymphadenopathies. A heterogeneous population of MC characterized by the presence of granules with a semilunar aspect and containing scrolls was observed. The former are the expression of a slow but progressive release of angiogenic factors due to chronic, progressive stimulation of MC degranulation, while the latter contain tryptase, an angiogenic factor. These two ultrastructural data confirm the important role played by MC in the angiogenesis associated with progression in B-NHL.
