Eight patients treated for histologically confirmed primary spinal epidural
non-Hodgkin's lymphoma
diagnosed between January 1979 and August 1989 (6.6% of all cases of intraspinal lymphoma) were studied. There were six men and two women. The median age was 70 years (range, 43-80 yr). Patients sought treatment for a prodrome of back pain (median duration, 3 mo) followed by an acute neurological deterioration (median duration, 6 d). The most common findings were a discrete sensory level in 5 patients, hyperreflexia in 5 patients, and
paraparesis
or paraplegia in 5 patients. Radiographically, there was an absence of bony destruction by these tumors. All patients underwent a decompressive laminectomy, subtotal tumor resection, and spinal irradiation (median dose, 3800 cGy). Two patients had low-grade lymphomas (one B cell and one T cell), and 6 patients had intermediate-grade lesions (six B cell). Two patients with B-cell lymphomas (one low-grade and one intermediate-grade) developed metastatic disease 15 and 17 months after the initial diagnosis; no evidence of lymphoma developed in the other 6 patients. The median survival was 22 months (range, 2-71 mo). Lymphoma was the cause of death in only 1 of the 4 patients who died, and the 4 younger patients are alive and well. Primary spinal epidural

should be a diagnostic consideration in the older patient who seeks treatment for spinal cord compression manifested by a prodrome of back pain, followed by a rapid neurological deterioration, normal plain spine radiographs, and neuroimaging consistent with an extradural compressive lesion. Surgery for this diagnosis followed by spinal irradiation should result in significant neurological improvement.

Anaplastic large cell Ki-1 lymphoma is an uncommon type of
non-Hodgkin's lymphoma
that rarely presents primarily in the bone. Three such cases are reported. All patients were young and had bone pain; one had
paraparesis
as a complication of collapse of the thoracic vertebral body. The involvement was either monostotic or polyostotic. Radiologically, the lesions were lytic and had ill-defined borders. Histologically, the large neoplastic cells had pleomorphic bizarre nuclei, prominent nucleoli, abundant deeply amphophilic cytoplasm, and paranuclear pale hof. They were admixed with variable numbers of inflammatory cells. One case each was of T-cell, B-cell, and non-T non-B lineage. All three cases showed excellent responses to chemotherapy with or without radiation therapy. Recognizing the lymphomatous nature of this highly pleomorphic tumor is important because of its potential curability with appropriate chemotherapy.

Primary
non-Hodgkin's lymphoma
of bone (PNHLB) accounts for less than 5% of all primary bone tumours and less than 1% of all
non-Hodgkin's lymphoma
. Due to its rarity, only a few retrospective studies have been published describing the prognosis and its treatment. We report our experience of 20 cases of PNHLB with their clinicopathologic correlation that were treated at our centre over a period of ten years. There were 16 cases of the mono-ostotic subtype and four cases of poly-ostotic subtype. All of these had a histological diagnosis of diffuse large B-cell lymphoma. The age of presentation was fifth to sixth decade. The mono-ostotic subtype commonly presented with the involvement of femur or humerus, while the poly-ostotic subtype commonly had
paraparesis
due to vertebral involvement. Cyclophosphamide, doxorubicin, vincristine, prednisone (CHOP)-based chemotherapy was given to all patients, but definitive radiotherapy was used only in the mono-ostotic subtype. At median follow-up of 38 months (range 5-96 months), event-free survival was 81% and 25% with mono-ostotic and poly-ostotic subtypes, respectively. Thus poly-ostotic PNHLB is a distinctive entity with a poor prognosis, and larger studies are needed for better management of this subtype.
