A 30-yr-old woman is described with an intermediate-grade
non-Hodgkin's lymphoma
, localized in the mediastinum, diagnosed in the 22nd week of gestation. Combination chemotherapy with cyclophosphamide, doxorubicin, vincristine, prednisone, teniposide and bleomycin was instituted in the second and third trimesters, resulting in regression of the tumour. Foetal distress due to pre-eclampsia forced us to perform a Caesarean section at 31 weeks of pregnancy. A healthy boy was born without congenital abnormalities. Further chemotherapeutic treatment, combined with radiotherapy, led to a complete remission of the
non-Hodgkin's lymphoma
; the remission lasted for 6 months. A review of the literature on chemotherapeutic treatment in
non-Hodgkin's lymphoma
during gestation is provided. Despite the limited number of cases reported, it can be concluded that chemotherapy in
non-Hodgkin's lymphoma during pregnancy
is less harmful than might be thought.

Six patients with
non-Hodgkin's lymphoma
diagnosed during late pregnancy or shortly thereafter are reported. Three patients had high-grade lymphoma and three, intermediate-grade histologic features. Five of the six patients were in Stage 4 and one was Stage 1. Only three had the diagnosis of
non-Hodgkin's lymphoma during pregnancy
, and in these three cases the disease appeared to progress rapidly after delivery. The other three women probably had lymphoma during pregnancy; lymphoma was widespread at the time of their diagnosis, but the diagnosis was only established on biopsy specimens after delivery in all three instances. Five patients had full-term natural deliveries with normal infants. A single patient with Burkitt's lymphoma had a premature delivery with a cesarean section in the 29th week because of abruptio placentae probably due to lymphoma of the uterine wall. In this case both mother and child died within 48 hours of delivery. The other five patients were treated soon after delivery. Published literature dealing with this topic is scanty and inconclusive; accordingly this report attempts to discuss management of lymphoma in pregnancy, based on the authors' limited experience with these six patients.

Non-Hodgkin's lymphoma is infrequently diagnosed during pregnancy and puerperium. A 21-year-old woman, para II, gravida II, on the seventh postpartum day arrived at the Emergency Gynaecologic Clinic complaining of colicky abdominal pain associated with bilious vomiting. A large tender mass of firm consistency was palpated in the midline towards the left hypochondrium. A CT scan of the upper abdomen showed thickened small bowel with halo, specifically at the junction of the jejunum with the ileum, resembling imaging of intussusception. Also, blocks of mesenteric and paraortic lymph nodes and multiple lymph nodes inducing enlargement of the anterior mediastinum were found. These findings rendered a picture of lymphoma in Stage at least III. Subsequent exploratory laparotomy confirmed our preoperative diagnosis. Histopathological examination of the lymph node biopsies established the diagnosis of a primary
non-Hodgkin's lymphoma
. Judging from the extent of the
non-Hodgkin's lymphoma
in our patient seven days after delivery the disease probably already existed during the last trimester of pregnancy. This study demonstrates a remarkable delay in diagnosis of
non-Hodgkin's lymphoma
in pregnancy. The diagnostic dilemmas of
non-Hodgkin's lymphoma during pregnancy
are emphasized and the literature is reviewed.
