Between January, 1982, and January, 1992, a total of 112 patients with adult T-cell leukemia (ATL) and 109 patients with
non-Hodgkin's lymphoma
(
NHL
) were admitted to our hospital. They were studied for their infectious complications. Infectious complications were seen in 90 patients (80.4%) with ATL, and 51 patients (46.8%) with
NHL
(p < 0.001). Documented infections were seen in 70 patients (62.5%) with ATL, and 30 patients (27.5%) with
NHL
(p < 0.001). Pneumonia (p < 0.005), skin infections (p < 0.05), Pneumocystis carinii pneumonia (p < 0.05), fungal infections (p < 0.05), cytomegalovirus infections (p < 0.05) and herpes simplex virus infections (p < 0.01) were identified infections at high risk for patients with ATL. Tuberculosis,
listeriosis
and salmonella infections were seen only in patients with ATL.

Listeriosis
is an emerging opportunistic infection in the immunocompromised host. A case of sepsis due to Listeria monocytogenes in a patient with advanced HIV infection and severe neutropenia, treated for an underlying
non-Hodgkin's lymphoma
, is described. Therapy with cotrimoxazole associated with rHuG-CSF (filgrastim) led to a rapidly favourable clinical and microbiological outcome, and to the correction of concurrent neutropenia. The case report is discussed according to a literature review of all cases of
listeriosis
reported until now in the setting of HIV infection and AIDS. In particular, the role of both cotrimoxazole and rHuG-CSF adjunct in the treatment of
listeriosis
in the immunocompromised patient is focused.

Adult patients with malignancies are considered to be at a high risk for Listeria monocytogenes meningitis. The Microbiology Laboratory's database of the University Hospital of Ioannina, Greece, was searched for cases of L. monocytogenes during the period from January 1990 to December 2002. Listerial meningitis occurred in three patients: one with brain tumour, one with chronic lymphocytic leukaemia, and one with
non-Hodgkin's lymphoma
. All the patients were older than 70 and they were actively receiving therapy for their malignancy. L. monocytogenes type 4b was isolated from blood and cerebrospinal fluid. All were treated with ampicillin and gentamicin, but they died shortly after the initiation of the treatment. Experience with the three present cases indicated the high mortality rate due to listerial meningitis in this immunosuppressed population. So,
listeriosis
should be suspected in patients with meningitis and underlying malignancy. Since meningitis due to L. monocytogenes is not distinguishable clinically from other types of bacterial meningitis, it is recommended to cover Listeria in the initial empirical therapy of bacterial meningitis in immunosuppressed patients.

Activation of naive T cells is tightly controlled and depends on cognate interactions with professional antigen-presenting cells. We analyzed dependency on secondary lymphoid tissues for the activation of naive and memory CD4(+) and CD8(+) T cells following primary and secondary
Listeria monocytogenes infection
, respectively. In splenectomized
lymphotoxin-beta
receptor-deficient mice, lacking all secondary lymphoid tissues, oral infection with L. monocytogenes failed to induce bacteria-specific CD4(+) and CD8(+) T cell responses. Treatment of splenectomized wild-type mice with FTY720, a drug that prevents egress of T cells from lymph nodes, also reduced T cell responses after oral L. monocytogenes infection and blocked T cell responses after intravenous infection. FTY720-treated wild-type and
lymphotoxin-beta
receptor-deficient mice show only slightly impaired recall responses. However, T cell responses were profoundly inhibited when mice were splenectomized subsequently to recovery from primary infection. T cell transfer experiments demonstrated that the impaired secondary T cell response was not simply due to removal of a large fraction of memory T cells by splenectomy. Overall, these results indicate that not only primary T cell responses, but also secondary T cell responses, highly depend on the lymphoid environment for effective activation.

Non-Hodgkin's lymphoma, a hematolymphoid malignancy, puts subjects at risk for complete infection. A 65-year-old man with
non-Hodgkin's lymphoma
Stage IV had undergone 5 R-CHOP courses in May 2008. Six days later, he was hospitalized for a high fever for which he was initially administered cefepime. When blood culture was positive for Listeria monocytogenes, he was administered ampicillin. His medical interview indicated that he had gone hunting and dressed wild animal meat at his mountain retreat, where he was exposed to wild animals and their excreta following R-CHOP course 5. CSF was not checked because his general condition was good. On hospital day 2, his fever dropped, and he was discharged following two weeks of ampicillin administration.
Listeriosis
cases reported in Japan number far fewer than in the United States, France or Germany. From January 1983 to February 2009, 153 cases were reported in Japan, 12 of whom were cancer patients. Despite the high incidence of meningitis with
listeriosis
, 7 of the 12 were not examined for CSF--an examination necessary in
listeriosis
, however well the subject appears.
