Pancreatic ascites is an entity defined as amylase levels up to 1.000 U/l in ascitic liquid. Frequently, it is secondary to a rupture of pancreatic ductus or pseudocyst and foreward communication to peritoneal space. We present a male diagnosed of calcified alcoholic chronic pancreatitis with pancreatic ascites secondary to a pseudocyst. Combination of parenteral nutrition and sintetic cyclic somatostatin was efficient. It would act by reducing pancreatic secretion in a long-term manner, which is the final purpose of the treatment. This association would be considered as a former tool in ascitic pancreatic patients, evacuatory punction or delayed surgery been relegated to a conservatory treatment failure or when primary pathology indicate it.

Pancreatic ascites is a rare complication of chronic pancreatitis, whose treatment continues to represent a difficult clinical problem. In this report we describe a case of a patient with chronic pancreatitis and pancreatic ascites who was successfully treated with somatostatin given by continuous intravenous infusion of 1.5 micrograms/kg/h for 2 weeks.

Pancreatic ascites, etiologically related to a leaking pseudocyst or ductal disruption, has been treated medically with hyperalimentation, somatostatin analog, and large-volume paracentesis. Surgery is ultimately required in more than 50% of such patients. Mortality figures in patients with pancreatic ascites approximate 15% to 25% with either treatment modality. We describe 4 patients who were found to have ductal disruptions in conjunction with pancreatic ascites who responded to transpapillary pancreatic duct endoprosthesis placement. There has been no recurrence of ascites in these patients at a mean follow-up of 12 months following stent-retrieval. Further evaluation of endoscopic therapy for pancreatic ascites appears warranted.
