Right heart failure
in patients with carcinoid heart disease is a serious prognostic sign. Consideration and adequate timing of valvular operations seem essential for the postoperative outcome. Without any relation to duration or progression of the metastasizing tumor disease,
right heart failure
developed and increased rapidly for a period of 12 to 17 months in four patients with classic carcinoid syndrome. Invasive hemodynamic and cardiac ultrasound investigations revealed severe carcinoid heart disease, and medical decompensation treatment gradually failed. Tricuspid and pulmonic valve replacement operations resulted in dramatic improvement in three of the patients, and these patients were still free of cardiac symptoms 10, 12, and 38 months postoperatively. One patient died 5 days postoperatively probably of septicemia. The preoperative and postoperative development of the cardiac disease is evaluated clinically, by cardiac ultrasound and plasma atrial natriuretic peptide concentrations, and related to the tumor disease. Surgical anatomy and operative technique are reported, and the beneficial value of prophylactic treatment of the effects of tumor-released vasoactive substances by a
somatostatin
analog is emphasized.

Neuroendocrine gut and pancreatic tumors have over the years presented a therapeutic challenge. The patients present with a wide range of clinical symptoms related to hormone production that can sometimes be easily managed but are sometimes life threatening. The most frequent clinical symptom related to endocrine gut tumors is the carcinoid syndrome, with flushing, diarrhea, bronchoconstriction, and

. Until the middle of the 1980s, when a patient was not cured by surgery, very little could be offered except chemotherapy. Biotherapy has revolutionized the treatment of malignant neuroendocrine gut and pancreatic tumors, in which both interferon-alpha and
somatostatin
analogues improved the quality of life for these patients and possibly also increased survival. Chemotherapy, with response rates of 40% to 60% in endocrine pancreatic tumors, is still first-line treatment in this group of patients, whereas in patients with carcinoids of the gut, no beneficial value of chemotherapy so far has been noticed. Both interferon-alpha and
somatostatin
analogues provide biochemical responses in 40% to 70% of patients with carcinoid tumors, whereas significant tumor reduction is only noticed in a few cases. Development of biotherapy is just at its beginning, and in the future, when we have learned more about tumor biology and mechanisms of action of these treatments, even better therapeutic results might be encountered. Combinations of biotherapy with chemotherapy as well as combinations of different biotherapies are now under clinical investigation.

In this case report the typical echocardiographic features of carcinoid heart disease are presented. Newer treatments such as the use of a
somatostatin
analogue, interferon and hepatic de-arterialisation have improved the prognosis in patients with carcinoid syndrome. Nevertheless this syndrome portends a poor prognosis in patients with cardiac involvement. Cardiac lesions are mainly located in the right side of the heart. , are the most common cardiac manifestations of the disease. Elevated levels of serotonin are probably responsible for the development of these cardiac lesions. Despite treatment resulting in significant reductions of urinary levels of 5-HIAA, regression of the cardiac manifestations in carcinoid syndrome has not been observed. Two-dimensional and Doppler echocardiography are the main tools to establish the diagnosis and severity of carcinoid heart disease. Cardiac surgery for carcinoid heart disease might improve symptoms and longevity, but the scarce data report on early mortality of over 35%.
