Balloon tamponade and vasoactive drugs such as vasopressin, glypressin, vasopressin and nitroglycerin combined and somatostatin are the mainstay of noninvasive emergency treatment of bleeding gastroesophageal varices. However, their hemostatic efficacy is limited and recurrent bleeding occurs in at least one half of the patients. Survival is not improved. Unwarranted side effects and complications may be severe. Therefore, vasoactive drugs and balloon tamponade can only serve as temporizing measures until some means of definite control of bleeding such as sclerotherapy is available.

The concentration of immunoreactive (IR) vasoactive intestinal polypeptide (VIP) in extracts from bovine adrenal medulla was 29.9 +/- 7.2 pmol/g wet wt., which was about 100 times that of IR neurotensin and 30 times that of IR somatostatin. Chromatographic analysis showed that most of the IR-VIP was the same molecular size as synthetic VIP(1-28). On retrograde perfusion of isolated bovine adrenal gland, release of VIP with catecholamine (CA) was marked on stimulation with high K+, but slight on stimulation with acetylcholine, which induced marked release of CA. These results suggest that most of the VIP is localized not in CA storing granules in chromaffin cells, but in other intraadrenal neuronal components. In immunohistochemical studies, IR VIP fibers with large varicosities were observed around the vessels in the adrenal medulla.

The paraventricular nucleus (PVH) of the hypothalamus is a key region for the integration of the autonomic and neuroendocrine mechanisms. This integration becomes less reliable with age. Some critical functions, such as eating and drinking, body-temperature regulation, autonomic and endocrine responses which regulate the cardiovascular system seem to be particularly affected by the aging-related processes. In this paper, we analysed by means of immunocytochemistry the neurochemical organization of the magnocellular and parvocellular component of the PVH in old male rats. The main results concerning the neurohormones and the carrier proteins are the following: a significant decrease in the number of the oxytocin- (OXY) like immunoreactive neurons of the medial and lateral parvocellular nuclei; a decrease in the vasopressin- (VAS) like immunoreactive neurons of the medial and lateral parvocellular nuclei and also of the medial magnocellular nucleus; a decrease in the neurophysin- (NRP) like immunoreactive neurons of the lateral parvocellular nucleus. We also found a decrease in the mean area of magnocellular OXY- and VAS-like immunoreactive neurons, a decrease in the extension of the dendritic tree sampled in the medial part of the nucleus; a decrease in the number of varicosities along the neurons coming from the PVH, and in the density of axons in the median eminence and in the vagal complex. The NRP-like immunoreactive structures in the substantia gelatinosa of the spinal cord of old rats were also decreased in respect to younger adult animals. Among the neuropeptides investigated (corticotropin-releasing factor, leu-enkephalin, somatostatin, cholecystokinin and neurotensin) we found a decrease in the leu-enkephalin-like immunoreactive neurons of the dorsal and medial parvocellular nuclei. Our data support--from a morphological point of view--the existence of involution processes in the neurochemical organization of the PVH during aging.

Bleeding from esophageal varices is related to the size and pressure of varices, endoscopic danger signs, and severity of liver failure. Prevention of bleeding with propranolol has given conflicting results in controlled trials, but is a safe treatment. Prophylactic sclerotherapy has been shown to reduce bleeding in European studies, but this has not been confirmed by studies in the United States. Acute variceal bleeding can usually be controlled by sclerotherapy, which may be supplemented by pharmacotherapy with vasopressin, nitroglycerin, or somatostatin. Recurrent bleeding is prevented initially by sclerotherapy, with surgery reserved for patients who have not responded to this treatment. Once bleeding has been controlled, the suitability and timing of hepatic transplantation must be considered.

An analysis of Nissl stained sections of the spinal cord taken from four species of elasmobranch showed that seven distinct cytoarchitectonic laminae are present. These laminae are compared with laminae described previously in the spinal cord of other vertebrates. The distribution of immunoreactivity to serotonin, substance P, somatostatin, calcitonin gene-related peptide, neuropeptide Y, and bombesin was determined in the brown stringray (Dasyatis fluviorum), the eagle ray (Aetobatis narinari), the shovelnose ray (Rhinobatis battilum), and the black-tip shark (Carcharhinus melanopterus). In all species, dense immunoreactivity to most substances tested was found in the outer part of the substantia gelatinosa. Many fibres and varicosities immunoreactive to substance P, calcitonin gene-related peptide, and bombesin were found in this region and smaller numbers of fibres were found in the nucleus proprius. Immunoreactivity to somatostatin consisted of coarse fibre bundles that entered the dorsal horn at the nucleus proprius and radiated dorsally to the substantia gelatinosa. Axons and varicosities immunoreactive to serotonin and neuropeptide Y were found in all regions of the dorsal horn but were concentrated in the outer part of the substantia gelatinosa. The distribution of immunoreactivity to met-enkephalin in the shovelnose ray was concentrated in the lateral third of the substantia gelatinosa and to a lesser extent in the nucleus proprius. The distribution of these substances is compared with that described in other vertebrates. Although the sensory information reaching the elasmobranch spinal cord is limited, compared with that of mammalian species, the distribution of these neuroactive factors in the dorsal horn of the two groups is strikingly similar.

Hippocampal CA3 neurons from fetal rats were grafted to excitotoxic lesions in the CA3 subfield of the adult rat hippocampus and the formation of graft-host brain nerve connections examined. The excitotoxic lesions were induced by localized, stereotaxic injection of ibotenic acid (IA), a glutamic acid agonist, into CA3 of the dorsal hippocampus. The result was a so-called axon-sparing lesion with localized degeneration of nerve cells, but preservation of the extrinsic afferent fibers, now deprived of their targets. One week after the lesion a suspension of embryonic (E18-20) CA3 cells was grafted to the lesion site. Six weeks or more later the recipient brains were processed and analyzed by ordinary cell stains, histochemistry for acetylcholinesterase (AChE) and heavy metals (Timm staining), immunohistochemistry for the neuropeptides cholecystokinin and somatostatin and glial fibrillary acidic protein (GFAP) for astroglia, electron microscopy, and axonal tracing with retrogradely axonal transported fluorescent dyes or lesion-induced, anterograde degeneration combined with silver staining or electron microscopy. More than 90% of the grafts survived. They contained the normal types of CA3 neurons, which are mainly pyramidal cells, in addition to some normal, peptidergic, cholecystokinin- and somatostatin-reactive neurons. The grafts were innervated by AChE-positive, host cholinergic fibers, Timm-positive mossy fiber terminals from the host fascia dentata, and host commissural fibers traced by axonal degeneration. Efferent transplant projections were traced to the ipsilateral host CA1 (Schaffer collaterals) and the contralateral host hippocampus by retrograde axonal transport of fluorochromes injected into these host brain areas. All grafts analyzed by electron microscopy contained axonal varicosities resembling axonal growth cones even after long survival times. The results demonstrate that fetal rat hippocampal neurons, grafted to excitotoxic, axon-sparing lesions in the adult brain, can become both structurally and connectively well incorporated in the mature host central nervous system.

The localization of Substance P(SP)-, Methionine-Enkephalin(met-Enk)-, Somatostatin(SOM)- Serotonin(SER)-, Cholecystokinin(CCK)-, and Vasoactive intestinal polypeptide (VIP)-like immunoreactivity (-LIR) has been determined immunocytochemically in the thoracic spinal cord intermediate zone of male and female guinea pigs. All neuroactive substances studied are exclusively localized in nerve fibre varicosities and terminals building up the vegetative network of the thoracic spinal cord intermediate zone. This network is situated dorsally to the central canal as a longitudinal plate of approximate thickness of 90-100 microns. Immunoreactive fibres are observed in the two Fasciculi longitudinales laterales and the two Fasciculi longitudinales mediales which are interconnected by transverse and oblique peptide-containing bundles (the terminology used by Petras and Cummings 1972; Galabov and Davidoff 1976). All these bundles interconnect the nuclei intermediolaterales principales and funiculares, the nuclei intercalates spinales and the nuclei intercalates paraependimales in ipsi- and contralateral as well as in rostral and caudal direction. The neurones of these nuclei are surrounded by immunoreactive varicosities and terminals. The quantity of the immunoreactive structures and intensity of the staining varied for the different neuroactive substances. As to the origin of the vegetative network immunoreactive fibres three main possibilities exists: a). From primary afferent neurones situated in the dorsal root ganglia, which send their axons via the dorsal roots (mainly for SP and perhaps for CCK); b). From supraspinal neurones which send their axons descending in the white matter funiculi and in the fasciculi longitudinales laterales and mediales and c). From intrinsic spinal cord neurones, which send their neurites in ascending and descending directions, ipsi- and contralaterally and interconnect the spinal cord segments. The different origin of the vegetative network immunoreactive fibres as well as the complex innervation of the preganglionic sympathetic nerve cells in the intermediate zone of the spinal cord suggests that this network may play an important role in the integration of the central and peripheral vegetative nervous system as well as probably in the integration of the somatic and the vegetative nervous system.

We have developed a dissociated primary cell culture of noradrenergic neurons from the locus ceruleus of postnatal (1- to 5-d-old) mice or rats. Slices of the brain stem were made on a Vibratome. Then the region of locus ceruleus, which was identified by observing the slices under a dissecting microscope, was dissected out from the slices. The removed fragments of brain slices were dissociated and cultured up to 3 weeks on a non-neuronal feeder layer, which consisted predominantly of astroglial cells, or on a fibronectin-treated collagen substratum. After 2 weeks of culture, about 70% of total neuronlike cells revealed positive catecholamine histofluorescence, indicating that they were probably noradrenergic neurons. About 98% of large- and medium-sized cultured neurons (soma diameter greater than or equal to 20 microns) was histofluorescence positive. The fluorescence-positive cells had long processes rich in varicosities, and the shape of their soma was either multipolar or fusiform. Electron microscopy using permanganate fixation revealed that the varicosities along their processes had small granular vesicles, which may contain norepinephrine. Physiological properties of these noradrenergic neurons were investigated with intracellular microelectrodes or with the whole-cell version of the patch clamp. We observed that many cells were producing spontaneous firing. Many of these spontaneously firing cells had no obvious contact with neighboring cells. The neurons were depolarized when glutamate was applied by pressure ejection. They also responded to GABA and glycine with either hyperpolarization or depolarization, and these responses were antagonized by picrotoxin and strychnine. Application of substance P generally produced depolarization with an increase in input resistance. The neurons responded with hyperpolarization to somatostatin, beta-endorphin, and enkephalin. This culture system will become a useful tool for elucidating the cellular and molecular properties of the central noradrenergic neurons.

We have used light and electron microscopic immunocytochemical techniques to study the distribution and morphology of neurons that contain vasoactive intestinal polypeptide-like immunoreactivity (VIP-Ir) in the adult rat striatum. VIP-Ir cells were sparsely distributed throughout all rostrocaudal levels of the striatum. Cell bodies were of medium size (12-17 microns) and gave rise to three to five primary dendrites, which branched close to the soma and became varicose. These dendrites appeared aspiny at the light microscope level and could be traced up to 250 microns in length. Dendrites frequently traversed axonal bundles in the striatum, a pattern not exhibited by neurons containing somatostatin-like or substance P-like immunoreactivity. In several instances, very fine varicose processes arborized extensively within 40 microns of the VIP-Ir soma; these may represent axons. In thin-sectioned preparations, examined under the electron microscope, the nucleus of VIP-Ir neurons was eccentrically located and showed several deep invaginations. Immunoreactive dendrites of VIP-Ir cells appeared virtually spine-free. Synapses with asymmetric or symmetric junctional specializations were present on the dendritic surface. Several VIP-Ir varicosities were found to terminate on the VIP-Ir cell body, forming synaptic junctions with symmetric specializations; these synapses may derive from recurrent collaterals. VIP-Ir cells thus resemble other aspiny striatal neurons considered likely to be local circuit neurons.

With the use of several region-specific antisera and the peroxidase-antiperoxidase (PAP) technique, several regulatory polypeptides were localized in nerves of the kidney. Neuropeptide Y (NPY)- immunoreactivity (IR), neurotensin (NT)-IR and vasoactive intestinal polypeptide (VIP)-IR occurred at high densities in all segments of the renal arterial system forming a perivascular plexus. Furthermore, NT-IR nerves were particularly frequent at the juxtaglomerular apparatus (JGA). Calcitonin gene-related peptide (CGRP)-IR was mainly concentrated in nerves supplying the hilus arteries and the JGA. Substance P (SP)-IR was predominantly found in large varicosities close to large renal arterial vessels and in the vicinity of the JGA. Somatostatin (SOM)-IR was only observed in single varicosities located at the media-adventitia border of large renal hilus arteries. The peptidergic nerves are correlated to their ultrastructural counterpart. In addition, the distribution patterns and the frequency of the different types of renal peptidergic nerve fibres are evaluated and compared. The functional role of these neuropeptides and their origin within the efferent branch of this part of the peripheral autonomic nervous system is discussed. Furthermore, the implication of some of the neuropeptides studied in afferent renal innervation is also substantiated.
