The application of immunofluorescence technique with anti-insulin, anti-glucagon, anti-
somatostatin
, and anti-pancreatic polypeptide (PP) antisera to sections of precisely sampled regions of the human pancreas allowed the quantitative evaluation of the total content of these four endocrine cell populations in 13 nondiabetics, in 2 insulin-dependent diabetics (IDDM), and in 2 non-insulin-dependent diabetic subjects (
NIDDM
) of various age and sex. In nondiabetic subjects, PP-cells appear sex-related. Male individuals have a significantly greater volume of PP-cells than female. In diabetic subjects, the only marked difference as compared with nondiabetics is the reduction of insulin cell volume in IDDM. Other small differences between individual endocrine cell volumes are detectable in both IDDM and
NIDDM
as compared with nondiabetics, but their significance is at present unclear. The qualitative changes of islet structure accompanying insulin cell reduction in IDDM were not considered in the present study.

plays a role in reducing postprandial glucose. In 6 of 7 normal controls plasma SRIF-LI increased and reached a maximum at 30 min after guar intake. In 5 of 7
NIDDM
patients SRIF-LI reached a maximum at 90 min after guar intake, while in five IDDM patients SRIF-LI significantly decreased through 120 min after guar intake. These results suggest the possibility that guar induced
somatostatin
secretion contributes to reduction of postprandial glucose levels at least in
NIDDM
patients as well as in normal subjects, for
somatostatin
is reported to inhibit glucose uptake from gastrointestinal tract.

To assess the relationship between beta-cell function and the level and duration of hyperglycemia during generalized beta-cell impairment, we studied the effects of acute and prolonged infusion of
somatostatin
in seven normal men. Twenty minutes after beginning an acute infusion of
somatostatin
(200 microgram/h) plus glucagon replacement (0.75 ng/kg/min), plasma glucose (PG) remained unchanged, but plasma insulin (IRI) and acute insulin response to isoproterenol had fallen markedly. Seventy minutes after beginning
somatostatin
-plus-glucagon, a rise in PG was associated with an increase in the acute insulin response to isoproterenol, though not to the control level. In a separate study, after 46 h of the
somatostatin
-plus-glucagon infusion, at a glucose level similar to the 70-min level, plasma insulin had returned nearly to the control level and the acute insulin response to isoproterenol had returned completely to the control level. Such increases inb basal and stimulated insulin secretion most likely represent a time-dependent adaptation by the beta-cells to the persistent hyperglycemia. First- and second-phase insulin responses to intravenous glucose were markedly inhibited after 46 h of
somatostatin
-plus-glucagon. In summary, a 46-h infusion of
somatostatin
with glucagon replacement in humans leads to hyperglycemia, a slightly diminished basal insulin level, markedly decreased insulin responses to glucose, and an insulin response to isoproterenol maintained at a normal level by acute and probably chronic adaptation to the hyperglycemia. We speculate that beta-cell adaptation to hyperglycemia may explain the similar abnormalities of islet function observed in patients with
NIDDM
.

To study the role of
somatostatin
in the pathophysiology of glucose intolerance in man, plasma
somatostatin
-like immunoreactivity (SLI) was measured in 8 normal subjects, 6 patients with insulin dependent diabetes mellitus (IDDM), 13 with
non-insulin dependent diabetes mellitus
(
NIDDM
), and 9 with hyperthyroidism, by extraction of plasma SLI and radioimmunoassay. The extraction method gave a recovery rate for synthetic somatostatin-14 and somatostatin-28 of 72 +/- 6 and 55 +/- 7%, respectively. No SLI corresponding to somatostatin-28 in human peripheral blood was observed. Incubation of somatostatin-28 in plasma gave a rapid decrease of immunoreactivity, and no conversion to somatostatin-14 was observed. It is speculated that SLI extracted with acid-acetone mainly represents a molecular weight similar to somatostatin-14. After oral administration of glucose (75 g), a clear and sustained rise in plasma SLI was seen in normal subjects from an initial value (+/- SEM) of 29.9 +/- 5.4 pg/ml to a peak value, at 60 min of 93.4 +/- 15.5 pg/ml. The increase of plasma SLI after 75 g glucose was also observed in IDDM and
NIDDM
. The peak level of SLI was significantly less than that for normal subjects. The extraction of plasma SLI with acetic acid and acetone gave reproducible results and showed a fluctuation of SLI with glucose concentration.

The present study was designed to determine the effect of naloxone, a specific opiate receptor antagonist, on postprandial levels of insulin, glucagon, pancreatic polypeptide (PP),
somatostatin
-like immunoreactivity (SLI) and gastrin in response to carbohydrate and fat-rich test meals in a group of 6 healthy volunteers. The addition of naloxone to a meal consisting of 50 g sucrose dissolved in 200 ml water augmented the rise of plasma insulin levels significantly during the first 30 min after its ingestion and reduced the rise in plasma insulin and pancreatic polypeptide and elevated glucagon levels during the last 30 min of the experimental period. When sucrose was dissolved in 200 ml cream the addition of naloxone augmented the postprandial rise of insulin levels between 15 and 60 min after ingestion of the meal and elicited an increase of plasma SLI and PP levels throughout the entire experimental period which indicates that post-prandial levels of insulin, glucagon, PP and SLI are modulated via endogenous opiate receptors during the ingestion of carbohydrate and fat test meals and that this effect depends on the composition of the ingested nutrients. These data raise the possibility that endogenous opiates participate in the regulation of postprandial insulin, glucagon,
somatostatin
and pancreatic polypeptide release not only in certain disease states as demonstrated recently for insulin secretion in
type II diabetes mellitus
but endogenous opiates may also be of importance under physiological conditions.

The utilisation of blood glucose may be used for definition of insulin resistance in
type II diabetes mellitus
. To evaluate this possibility we adapted an infusion test of
somatostatin
in 10 normal persons (age 26 +/- 3 years, relative body weight 26 +/- 10% according to Broca) in a randomized cross-over therapy with bezafibrate (3 X 200 mg/die). As the coefficient of variation (VC) of measured blood glucose continuously increased the best time of a steady state was between 90 and 130 min after beginning the infusion (mean VC 8.9%). While insulin remained nearly constant (41 (45; 38) microU/ml) blood glucose dropped by about 14% and reached a steady state of 89 (134; 45) mg/dl. During the therapy of bezafibrate blood glucose was significantly decreased by 36% 130 min after beginning the infusion. Although the effect was not significant during the whole time of the steady state it became evident by a negative correlation with lactate (r = -0.687) and pyruvate (r = -0.843). A correlation with a concomitant decrease of triglyceride and cholesterol also induced by bezafibrate could not be proven. The results show that the infusion test of
somatostatin
is fitted to measure a steady state of blood glucose and insulin and that it is possible by this technique to quantify a changed utilisation of blood glucose induced by specific therapy.

We have investigated in 400 patients, 159 male and 241 female, mean age 58 years, with
type 2 diabetes
, the incidence of peptic ulcer and on 127 patients with peptic ulcer, the incidence of diabetes mellitus. In conclusion the incidence of peptic ulcer is no more than 3,9% in diabetic patients. In peptic ulcer the female to male ratio is 1:0,9. The seat of the ulcer is resulted to be in ratio of 1,4:1 more in the duodenal tract than in the gastric tract. The result are attributed to the increased secretion of the gastrointestinal hormones (Bombesin and
Somatostatin
) in the diabetic patients. However the neuromodulator of gastrointestinal hormones release is mediated of the Dorsal Motor nucleus of the Vagus nerve connected to the endocrine pancreas exclusively via vagal fibers and has a role in neurally mediated insulin release.

Basal and postprandial levels of gastrin,

, vasoactive intestinal polypeptide (VIP) and pancreatic polypeptide (PP) were followed up in 105 patients with
non insulin dependent diabetes
mellitus (20 with autonomic neuropathy only, 35 with peripheric neuropathy only, 30 with autonomic and peripheric neuropathy simultaneously and 20 without any sign of neuropathy) and in the control group of 40 individuals. Serum levels of gastrin,
somatostatin
, VIP and PP are determined by a RIA (used kits of Prof. SR Bloom, Hammersmith Hospital, London). The results of investigation showed significantly higher basal and postprandial levels of gastrin and VIP in patients with autonomic neuropathy in comparison with the group without neuropathy and with the control group (p < 0.001). The serum levels of
somatostatin
did not differ significantly between the groups of diabetics with and without neuropathy. Basal level of PP was significantly lower and postprandial PP levels remained low in patients with autonomic neuropathy in comparison with the group without neuropathy (p < 0.001). We postulate that basal and postprandial gastrin and VIP levels raised secondary to partial vagotomy in diabetics with autonomic neuropathy. Measuring PP serum levels in diabetics after a protein rich meal can be useful to check vagus nerve function in the gastrointestinal tract in order to detect autonomic neuropathy.

To examine whether sulphonylureas influence hyperglycaemia-induced glucose disposal and suppression of hepatic glucose production (HGP) in
type 2 diabetes
mellitus, a 150-min hyperglycaemic (plasma glucose 14 mmol/l) clamp with concomitant
somatostatin
infusion was used in eight type 2 diabetic patients before and after 6 weeks of glipizide (GZ) therapy. During the clamp a small replacement dose of insulin was given (0.15 mU/kg per min). Isotopically determined glucose-induced glucose uptake was similar before and after GZ administration which led to improved glycaemic control (basal plasma glucose 12.2 +/- 1.3 vs 8.9 +/- 0.7 mmol/l; P < 0.01). Glucose-induced suppression of HGP was, however, more pronounced during GZ treatment (0.96 +/- 0.14 vs 1.44 +/- 0.20 mg/kg per min; P < 0.02). Following GZ treatment hyperglycaemia failed to stimulate glycogen synthase activity. Moreover, GZ resulted in a significant increase in the immunoreactive abundance of the insulin-regulatable glucose transport protein (GLUT 4) (P < 0.02). In conclusion, these results suggest that GZ therapy in type 2 diabetic patients enhances hepatic sensitivity to hyperglycaemia, while glucose-induced glucose uptake remains unaffected. In addition, GZ tends to normalize the activity of glycogen synthase and increases the content of GLUT 4 protein in skeletal muscle.

Islet amyloid polypeptide (IAPP) is a 37-amino acid residue polypeptide, originally isolated from the pancreatic amyloid deposits of patients with
type II diabetes mellitus
. Subsequently, IAPP was found to be colocalised with insulin in beta-cell secretory granules of the normal mammalian pancreas. Recently, IAPP has been reported to inhibit glucose-stimulated insulin release from isolated rat islets and to be released in response to glucose and arginine. To investigate further the regulation of IAPP release from the islet, we used a previously developed specific radioimmunoassay for IAPP and measured IAPP secretion from isolated rat islets of Langerhans. Release of IAPP-like immunoreactivity (-LI) was stimulated by glucose: 3.3 +/- 0.3, 3.9 +/- 0.3, and 11.1 +/- 1.5 (n = 5, mean +/- SEM) fmol/islet/60 min at 2, 7, and 20 mM, respectively. Carbachol (0.1 mM) increased the release of IAPP-LI at the lower glucose concentrations: 8.1 +/- 0.9, 8.7 +/- 0.6, and 11.7 +/- 1.8 fmol/islet/60 m in at 2, 7, and 20 mM glucose.
Somatostatin
(1 microM) suppressed glucose-stimulated IAPP-LI release (17.5 +/- 1.5 vs. 5.1 +/- 0.5 fmol/islet/60 min). Chromatographic characterisation of the IAPP-LI released into the incubation medium revealed two immunoreactive forms: The major peak (74% of the total IAPP-LI) corresponded to synthetic IAPP-37, while a smaller form, comprising 26% IAPP-LI, eluted later. In acid extracts of islets, all (> 95%) immunoreactivity co-eluted with the synthetic IAPP.
