Dye dilution curves (DDC) from 35 patients with arteriovenous fistulas and from 22 patients with hyperthyreoidism were performed and compared with results in 122 healthy subjects. In most cases Cardiogreen was injected into an antecubital vein during reactive hyperaemia. On the arterial side, dye concentrations were recorded by ear oximetry. The appearance time was shortened in most cases. During dilution time dye curves were interrupted by an early recirculation in hyperthyreoidism, whereas in 29 patients with haemodialysis fistulas this interruption was discrete and detectable only when compared with results after occlusion of the fistulas. Different shapes of dilution curves were observed: 1. in most cases DDC seemed to be normal or near normal, often with an accelerated indicator passage; 2. DDC with an interruption of the dilution limb by an early recirculation; 3. DDD, showing a hump of the dilution limb; 4. asymmetrical DDC or DDC with a flat shoulder of the dilution limb and without a recirculation wave. Patients with hyperthyreoidism and arteriovenous fistulas had no different shapes of dilution curves. The results are discussed with special reference to the DDC in patients with central left-to-right shunts.

Wenckebach behavior of DDD pacemakers occurring when the P-P interval varies between the programmed upper rate interval and the total atrial refractory period is symmetrical in a sense that the pacemaker response during atrial rate acceleration is similar to the pacemaker response during atrial rate deceleration. This phenomenon can be observed in all patients with persistent AV block in whom a DDD pacemaker is implanted, during exercise testing when the spontaneous atrial rate exceeds the selected upper rate, i.e., the programmed upper rate interval. However, this phenomenon will not be observed in all patients with intermittent intact AV conduction during exercise. In this case report we describe a patient who showed an asymmetrical response during a bicycle exercise test. There was 1:1 atrial sensing ventricular pacing until the atrial rate exceeded the upper rate of 140 ppm, while atrial sensing was restored during recovery when the conducted sinus rhythm had decreased to 105 beats/min.

A cohort of 300 ACI rats was kept under standard laboratory conditions. After 30 months or upon natural death, complete autopsy was performed. In the genitourinary tract four kidney and five bladder tumors were found. Two of these bladder tumors, RBT323 and RBT157, are serially transplantable. In the fifth transplant generation the RBT323 tumor becomes metastatic to the lungs in more than 90% of animals. The metastatic ability of the RBT157 tumor changes from low to intermediate (50% of the rats have lung metastases) in the fourth passage. Histologically, the initial passages of the RBT323 and 157 tumors are grade II transitional cell carcinoma (TCC). The histological pattern of the RBT157 tumor remains essentially unchanged, whereas the RBT323 tumor progresses to a grade III tumor in the third passage. Electron microscopical studies reveal oblong elliptical and round vesicles lined by an asymmetrical unit membrane in the tumor cells, which stresses the urothelial origin of the tumors. Immunohistochemically both tumors show expression of cytokeratin 5, 7, 8 and 18. The progression of the tumors to a metastatic phenotype, however, is not associated with a specific change in the morphological characteristics. Cytogenetic analysis shows that both tumors are peridiploid with few marker chromosomes. Interestingly, both of these independently arising tumors exhibit a loss of chromosome 5. Rat chromosome 5 is syntenic to the major portion of human chromosome 9 (p23-qter). Loss of chromosome 9 is a cytogenetic trait of human superficial TCC, hence the RBT model is also in cytogenetic respect similar to human TCC.(ABSTRACT TRUNCATED AT 250 WORDS)
