Beside demonstrating morphological changes, CT enables one to make quantitative measurements of the spinal canal (sagittal diameter, transverse diameter, area of the spinal canal, width of the lateral recess and symmetry of the articular joints). A reduction of the transverse diameter of the spinal canal or asymmetry of the articular joints is associated with an increased and earlier incidence of back pain. In addition, patients with
asymmetrical
vertebral joints showed more frequent and earlier development of
disc prolapse
.

The present case report describes the topographical anatomy and radiological study of an
asymmetrical
inferior articular process of a lumbar vertebra, which was detected during routine osteology teaching of undergraduate medical students. The inferior articular process of the lumbar vertebra on the left side was rudimentary, while that on the right was normal in size. On the left side an additional bony projection was noted anterior to the rudimentary inferior articular process. The difference in height between the inferior articular processes of the two sides may play an important role in the kinematics of the particular joint. The orientation of the facets of the articular processes of the vertebrae are important for axial weight transmission and anomalies involving these can possibly alter the orientation of movements in that particular segment. An
asymmetrical
inferior articular process may be related to
disc prolapse
and may be a cause of back pain. In view of the paucity of research reports of anatomicoradiological study of the inferior articular process of a lumbar vertebra in relation to other parts of the vertebra, we, as anatomists, believe that knowledge of anomalies of the inferior articular process may be relevant for academic, anthropological and clinical purposes.

Lumbar disc disease occurs mainly in the adult population. A
disc prolapse
in the paediatric population is very rare. Cauda equine syndrome resulting from compression of the cauda equina is a rare syndrome and is one of the few spinal surgical emergencies. Here we present a 13-year-old boy with pain in the lumbar region radiating bilaterally to the lower limbs, with
asymmetrical
weakness of lower the limbs, perianal hypoaesthesia and urinary retention. MRI of lumbar spine confirmed disc protrusion at the L3-L4 level with severe spinal canal stenosis. Patient was treated with microdiscectomy at the L3-L4 level. Postoperatively, his neurological deficit disappeared gradually. Although very rare, lumbar
disc prolapse
in the paediatric age group can lead to cauda equina syndrome. Early diagnosis and treatment can prevent life-long disability.
