, but the presence of this sign may be indicative of the earliest onset of optic nerve damage in patients with ocular hypertension. Although the presence of an afferent pupillary defect should suggest the consideration of a neurologic lesion, glaucomatous damage may be the sole explanation for this phenomenon.

, we attempted to determine whether the transition from ocular hypertension to

might be typified by lateral differences in either intraocular pressure (IOP), papillary excavation, or visual field. Such lateral differences were sought in a retrospective study of the medical records of 47 patients exhibiting ocular hypertension, of whom 25 remained hypertensives and 22 developed
glaucoma
during the period considered. Although all visual field indices in the latter group had remained within the normal range, the same side consistently yielded significantly worse results in mean sensitivity, and the absolute value for the lateral difference in these individuals was significantly higher than that found for the patients who did not develop
glaucoma

is manifested when the mean sensitivity remains worse in one eye.

Outcome, prognosis and course of pigmentary glaucoma were studied in 38 patients (75 eyes). The mean age of the subjects at the time of diagnosis was 34.0 years and at the time of the study 44.1 years. The follow-up time ranged from 2 to 30 years with a mean follow-up time of 10.1 years. At the time of the study eighty-nine percent of the eyes had normal vision (0.7 or better), 3% slight impairment of vision (0.6-0.3), 5% low vision (0.2-0.05) and 3% were blind. Visual field defects were mild in 78%, moderate in 13% and severe in 8% of the eyes. One eye (1%) had normal fields. In 8 patients the stage of the field defects was
asymmetrical
between the two eyes. All were able to read at least monocularly with glasses. No one was blind, retired early or professionally limited because of pigmentary glaucoma. Thirty-nine eyes were on topical treatment. Fifteen eyes had additionally been treated with laser trabeculoplasty when the

could not be stabilized with topical treatment combined with systemic acetazolamide. Further twenty eyes had undergone trabeculectomy, 18 of them when medical treatment had failed and 2 following LTP. The non-involved eye of a patient suffering from a monocular disease was without treatment. The mean intraocular pressure at the time of the diagnosis was 33.5 mmHg (SD 10.4 mmHg) and at the final visit 17.5 mmHg (SD 3.4 mmHg). The local mean and the average difference of four pairs of test locations within the 26 degrees visual field, situated above and below the horizontal nasal meridian, were used to predict the global field indices MD and CLV of the Gl
glaucoma
program. Out of 539 examinations (194 eyes suspected of having
glaucoma
), the local indices NDIFF (describing
asymmetrical
behavior around the nasal horizontal meridian), ND0 (the mean defect in the nasal region), and the global indices MD and CLV were calculated. Seven hundred fifty-five examinations (446 normal eyes) served as a control group. First and second examinations of 146
glaucoma
suspect eyes were used to calculate the retest reliability scores for the indices in question. When analyzing the
glaucoma
suspects, the local index NDIFF, together with the local mean defect, ND0, yielded highly reliable estimates of the global indices MD and CLV, with a retest correlation r = 0.86 for NDIFF, and r = 0.96 for ND0. The covariance of NDIFF with CLV was r = 0.67, while the co-variance of MD with ND0 was r = 0.95. The ranges of the local indices ND0 and NDIFF were each classified into 'normal range' and 'range of suspected pathology', in analogy to the normal and pathological ranges of the global field indices. Equivalence of the local indices with the corresponding ranges of MD and CLV was investigated and the results are shown. The establishment of local indices may prove to be a powerful tool in early detection of glaucomatous damage.

The clinical significance of our estimating PTM pigmentation at the 12 o'clock position is discussed. In view of the accelerated optic nerve damage associated with the development of

Australian legal blindness is defined as bilateral corrected visual acuity less than 6/60; if visual acuity is better than this, collateral visual impairments may be included. Persons thus affected qualify for the Invalid Pension (Blindness). From October 1975 to June 1982, I studied clinical data of 311 persons assessed as legally blind, and recorded diagnoses. I personally examined 259 of these persons and a medical colleague examined 52. Examinees were resident in Brisbane (or environs), Australia. Eleven persons had

conditions (analysed separately), and 300 had symmetrical conditions. Causes of the latter were genetically transmissible (20.3%), adult maculopathy (18%), congenital (12.7%), vascular (non-diabetic) (8.7%), adult

(8.3%), diabetes (7.3%), trauma (6%), and others (18.7%). Of 61 symmetrical genetic cases causes were retinitis pigmentosa (34.4%), congenital cataracts (16.4%), retinal dystrophy and maculopathy (13.1%), Leber's optic atrophy (9.8%), and others (26.3%). These results suggest that more attention should be directed to genetic counselling.

Between 1975 and 1983 the authors saw 5 patients (average age 52) with massive unilateral episcleral vascular congestion and secondary open-angle
glaucoma
. All of the patients were found to have a quite large,
asymmetrical
cupping of the optic disk and 3 of them had glaucomatous scotomata. There was no exophthalmos, no motility disturbances and no evidence of intracranial circulation problems. Sinosotomy was performed initially in all the patients, resulting in normalization of pressure in 3 cases; normalization was also achieved in the fourth patient with additional drug therapy. Compensation was achieved in the fifth patient following goniotrepanation. The cause of the episcleral vascular disturbance is still unknown.

We describe two patients who developed an UGH syndrome respectively 5 and 6 years after an ECCE with implantation of a flexible closed-loop one-plane PC IOL of the Anis type. In 35 patients with bilateral primary open-angle
glaucoma
(POAG), with
asymmetrical
evolution of the neuropticopathy between the two eyes, we compared within each patient, the response to ibopamine and the perimetric defect. In 88% of cases (31/35 patients) the eye with the most severe perimetric defect had high intraocular pressure or a larger IOP increase after ibopamine. This result was highly significant at the sign test (P < 0.001). IOP and its increase after ibopamine significantly differed in the most and least affected eye, being higher in the eye with the most impaired visual field. Since ibopamine can be used to quantify the hydrodynamic impairment, which is thus presumably correlated to the perimetric defect. This study further confirms its importance in the development of glaucomatous damage.
