The authors have treated 55 deformities of the anterior chest wall in children. There were 37 cases of symmetrical pectus carinatum, 15 cases of
asymmetrical
pectus carinatum and 3 cases of pectus arcuatum. Associated lesions of the ribs, scapulae and spine are described. Treatment in every case was by plaster casts followed by a plaster jacket and exercises. The results were much more satisfactory in deformities associated with prominence than with
depression
of the sternum. In pectus carinatum, the results were better when the apex of the deformity was more distal. The best age for treatment was between 12 and 13 years. No patient was treated by operation.

It is classically considered that cochlear fatigue and acoustic trauma occur when intensity is such that the ear is saturated, i.e. when the microphonic potential fails to increase in a linear fashion in relation to intensity and when distorsion appears. The present report concerns a study in the guinea pig of the relationship between the non-linearity of microphonic responses, and their fatigability. Vincristine, other periwinkle alkaloids, and colchicine partially inhibit the energy dependent transport of alpha-aminoisobutyric acid in Ehrlich ascites tumor cells. The properties of this phenomenon were characterized in detail for vincristine. Maximum
depression
of the steady-state intracellular alpha-aminoisobutyric acid level was achieved with a vincristine concentration of less than 0.5 muM. The inhibitory effect of vincristine increases as the extracellular alpha-aminoisobutyric acid concentration is increased reaching a maximum, however, of only approximately to 25% at a level of 5 mM, leaving a large gradient for alpha-aminoisobutyric acid across the cell membrane. asymmetrical
uptake rate, while increasing the efflux of alpha-aminoisobutyric acid. Inhibition of net alpha-aminoisobutyric acid transport by vincristine was partially reversible (approximately to 40%). Colchicine (50 muM) reduced the steady-state alpha-aminoisobutyric acid level by 30%, an effect that was not reversible. Inhibition by vinleurosine and vinrosidine was comparable to that of vincristine. Addition of glucose to the medium resulted in a small, but significant, decrease in the inhibitory effects of both vincristine and colchicine. The data indicate that these agents inhibit a small component of the uphill transport of alpha-aminoisobutyric acid in Ehrlich ascites tumor cells. The inhibitory effect of vincristine cannot be attributed to an increase in the passive permeability of the cell membrane to this agent. Rather, the data along with other studies from this laboratory suggest that vincristine reduces the energy-dependent transport of alpha-aminoisobutyric acid by either inhibiting cellular energy metabolism or by inhibiting cellular energy metabolism or by inhibiting the coupling of energy-metabolism to the transport of this amino acid and raises the possibility that cellular microtubules play a role in these processes.

Serial electrocardiograms as well as echocardiographic studies of 51 pilgrims suffering from acute heat stroke (mean rectal temperature 41.6 degrees C) were performed. All patients were examined immediately after cooling and 24 h later whenever possible. Other cardiac abnormalities included right ventricular dilatation and increased in left ventricular internal dimensions in 4 cases (7.8%), respectively. Thirteen cases (25.5%) had normal echocardiographic findings. Forty (78%) patients had sinus tachycardia while 8 cases (15.7%) showed atrial fibrillation with uncontrolled ventricular rate, and 3 (5.8%) had sinus bradycardia. Between 1972 and 1987, 192 patients have been operated upon for pectus excavatum of which 152 patients were included in the study (79%). Mean age at operation was 15.3 +/- 5.5 years; 117 were male. Mean follow-up was 8.1 +/- 3.6 years. The deformity was noted before the age of 5 in 90%. Type I symmetrical and localized deformity was seen in 33.2%, type II symmetrical but diffuse

The operation consisted of subperichondral chondrectomy, transverse sternotomy and division of the intercostal bundles at the outer limit of the chondrectomy and suturing the edge of this broad sheet of muscle and perichondrium to the anterior surface of the chest wall more laterally and under tension, elevating and stabilizing the sternum. Results were satisfactory in 83.6% (excellent 44.1%, good 39.5%). Results were not significantly influenced by age, sex, severity, type, symmetry, the extent of cartilage resection or follow-up. Results were inversely influenced by the occurrence of wound problems. The optimal age for operation is considered to be between 5 and 10 years. Both physical as well as psychological cosmetic factors may serve as an indication for operation.

Brainstem acoustic evoked potentials (BAEPs) were recorded in 16 children with posterior fossa tumours. Based on these results BAEP measurement seems to be useful in the differential diagnosis of posterior fossa tumours of childhood, and it can be necessary even beside the CT scan. It can be useful in the early diagnosis, because the BAEP positivity can precede the CT scan positivity, in such a case repeated CT scan required. Finally BAEP measurement proved to be effective in the follow-up of posterior fossa tumours: it can document the tumour's chemotherapeutic regression, or the progression. It is a simple, non-invasive, and cheap method.

The authors present the case of an adult woman who was treated in the mental health system for many years for a syndrome of mixed anxiety and

unresponsive to psychopharmacologic or psychotherapeutic treatment. A computed tomography scan revealed

enlargement of the right temporal horn. This finding, as well as findings of severe difficulties in spatial relations together with mildly abnormal posturing of the left hand when walking (especially when the gait was stressed), suggested that the patient had a developmental disability of the right cerebral hemisphere. The authors conclude that because such patients might appear to have primarily psychiatric rather than neurologic problems, the diagnosis of right hemisphere dysfunction should be considered when the clinical presentation is indicative of atypical anxiety and affective disorders.
