Regional cerebral blood flow (rCBF) was determined by single photon emission CT (SPECT) with N-isopropyl-p-[123I]iodoamphetamine in 22 patients with clinically diagnosed senile dementia of Alzheimer type (SDAT) and in 18 age matched controls. We calculated asymmetry indices (AIs) of rCBF for matched right-left regions of interest. rCBF of parietal lobe in SDAT patients was significantly most laterally asymmetrical, but the least in occipital lobe. Lateral asymmetry of rCBF in SDAT patients correlated with asymmetry of language and visuospatial functions; decreased rCBF in the left parietal lobe was associated with language dysfunction, and that in the right parietal lobe, with visuospatial dysfunction. Furthermore cerebellar AIs correlated negatively with those of the cerebral hemisphere and lower frontal region in SDAT patients. The results demonstrate that rCBF measurement by 123I-IMP SPECT is useful to detect lateral asymmetry in reduction of rCBF in SDAT.

Resting-state regional gray matter flow (Fg) values and cerebral vasoconstrictor responses induced by 100% oxygen inhalation were measured with the 133 Xe inhalation method in normal healthy volunteers aged between 15 and 86 years and in patients with senile dementia of Alzheimer's type (SDAT) or multi-infarct dementia (MID). Cross-sectional analysis revealed that there were linear decreases of oxygen responses with advancing age in 84 normal volunteers between the second to ninth decades. Eleven patients with SDAT showed bilateral and symmetrical reductions of resting-state Fg values compared with 22 age-matched normal healthy volunteers. Eight patients with MID showed no significant reduction of mean Fg values compared with normal controls and patients with SDAT. Oxygen vasoconstrictive responses in SDAT were symmetrical and similar to those seen in age-matched controls. Compared with patients with SDAT, patients with MID showed reduced oxygen vasoconstrictive responses that were asymmetrical between hemispheres as well as heterogeneous within hemispheres. Testing cerebral vasoconstrictor responses by 100% oxygen inhalation is helpful for differentiating SDAT from MID.

We report an 83-year-old woman with atypical senile dementia with Fahr-type calcification. Brain computed tomography demonstrated asymmetrical calcification predominant in the basal ganglia on the right side and pronounced diffuse cortical atrophy in the frontotemporal areas. The patient was clinically diagnosed with diffuse neurofibrillary tangles with calcification. Brain single photon emission computed tomography findings revealed that cerebral blood flow was reduced on the right side, as compared with the left side, in widespread areas. Hemispheric asymmetry in both calcification and cerebral blood flow suggests a relationship between calcification and vascular changes.
