One hundred and five infants of birth weight 2000 g or less who received peripherally administered parenteral nutrition for periods of three or more weeks, were randomly assigned to groups receiving different amounts of zinc and copper supplement. The blood concentrations of zinc, copper, retinol-binding protein, prealbumin, alkaline phosphatase and aspartate transaminase were followed weekly. Mean serum zinc, retinol-binding protein and prealbumin declined significantly over time while alkaline phosphatase rose. Only the group receiving the highest zinc supplement maintained a mean serum zinc concentration within the normal range at seven weeks. No difference in the protein or enzyme concentrations was found between the different zinc supplement groups. No difference was seen in serum copper or ceruloplasmin between copper dose groups although one intravenous supplement was double that of the other.

Liver damage in a woman who had taken an overdose of paracetamol and dextropropoxyphene was assessed by monitoring serum prealbumin concentrations and by routine plasma enzyme determinations. The plasma aspartate aminotransferase returned to normal levels after 3 days, alkaline phosphatase was slow to show increases in activity, and serum albumin concentration was in the normal range throughout. Prothrombin-time, although initially very high, returned almost to normal as a result of the administration of plasma. In contrast, serum prealbumin concentration decreased significantly after 36 h and continued to decrease, showing the course of failing liver function, until the patient's death 15 days after presentation. Prealbumin, a functional plasma protein synthesised in the liver, has a short half-life, is a true index of liver function, and seems to be a more reliable indicator of liver function in drug overdose than plasma enzymes, prothrombin-time, or plasma drug concentration.

Of 33 components analyzed in overnight fasting serum from 30 patients with alcoholic liver cirrhosis, portal hypertension, and bleeding esophageal varices, total serum bile acids, gamma-glutamyltransferase, prealbumin, and tyrosine were the most frequently abnormal 'liver tests'. Total serum bile acids correlated significantly with bilirubin, immunoglobulin M, threonine, glycine, methionine, and tyrosine. Gamma-glutamyltransferase correlated with aspartate aminotransferase, glutamine, and alanine. Prealbumin correlated with albumin and immunoglobulins G and A. Tyrosine correlated with total bile acids, orosomucoid, and 10 amino acids. The amino acid ratio of valine + isoleucine + leucine to tyrosine + phenylalanine was lowered in all patients. It is concluded that the clinical picture and pattern of serum components in patients with alcoholic liver disease are influenced by many complex pathophysiological mechanisms.

A training program was started in nine patients with chronic active hepatitis in clinical remission while receiving immunosuppressive therapy. The patients were examined before and after a training period of 4-5 weeks and 10-12 weeks, respectively. The calculated oxygen consumption increased by 19% and 29%, and the estimated work load capacity improved. No change occurred in alanine aminotransferase, aspartate aminotransferase, alkaline phosphatases, gamma-glutamyl-transpeptidase, serum bilirubin, or prealbumin, whereas creatine kinase and lactate dehydrogenase increased significantly. The clinical condition did not worsen in any patient, and most of the patients felt that their physical performance capacity had improved. We conclude that long-term regular physical training is well tolerated by patients with chronic active hepatitis in clinical remission and that training leads to improvement in the oxygen consumption and the estimated work load capacity in such patients.

A group of 291 children aged 3 weeks to 6 1/2 years was examined at a public maternal and child health center and 260 of them - who were considered to be healthy - were included in the present study. By venipuncture, serum was obtained for the analysis of 6 enzymes, and plasma for the estimation of 9 proteins and for lipid analyses. In different age groups, high levels were found for alkaline phosphatase, lactate dehydrogenase, aspartate aminotransferase, alanine aminotransferase, creatine kinase and gamma-glutamyl transferase. Haptoglobin, alpha 1-antitrypsin, prealbumin and transferrin were present at low concentrations during the first months of life. Transferrin rose later in childhood to above adult levels. Only immunoglobulin M showed a sex difference, with higher values for girls. Breast-fed infants had higher (non-fasting) concentrations of cholesterol and triglycerides than formula-fed babies, and they also had higher levels of aspartate aminotransferase and alanine aminotransferase.

Study of newborn infants established that the total content of soluble proteins in the abomasum and intestine mucosa does not vary significantly. In the composition of the small intestine mucosa proteins the content of prealbumin and albumin increases. The aspartate aminotransferase activity of the studied tissues becomes more intensive with the calves age.

We clustered a selected population of patients at Bridgeport (Conn) Hospital into distinct nutritional classes using critical decisions points for the serum concentrations of aspartate aminotransferase activity, cholesterol, total protein, albumin, and prealbumin. The decision points that divided the populations with the highest efficiency were delineated by the information in the data set. At these decision values the hospital population adjusted malnutrition rate was just more than 30%. This was lower than the reported prevalence of malnutrition in the hospital population and was better in agreement with global estimates of malnutrition. The study identified a problem in the assignment of decision values that are used for nutritional support which has implications for nutritional interventions.

Tamoxifen is widely used as an adjuvant treatment for breast cancer. To correctly interpret laboratory test results during tamoxifen treatment, clinicians should be aware of the possible effects of the drug on laboratory tests. This study investigated the effects on serum hormones, proteins, lipids and common biochemistry in seven postmenopausal women with breast cancer during 3 months after initiating the therapy. Statistically significant decreases occurred in serum gonadotropins, alkaline phosphatase, calcium, total protein, prealbumin, orosomucoid, haptoglobin, immunoglobin M and total cholesterol whilst significant increases occurred in serum sex hormone-binding globulin (SHBG), cortisol, parathyroid hormone, aspartate aminotransferase, urate, alpha-1-antitrypsin and ceruloplasmin. The alterations could result from tamoxifen therapy, radiation or changes in lifestyle. All the changes, apart from serum urate, remained within the reference limits. In addition, only serum gonadotropins, SHBG, urate and cholesterol showed clinically significant changes. Alterations in the other laboratory tests are unlikely to disturb diagnoses based on laboratory test results during tamoxifen therapy.

Effects of 6-aminonicotinamide (6-AN) on the levels of proteins, metabolites and enzyme activities in the plasma of Japanese quail were investigated. The concentrations of soluble proteins in the pectoral and hindlimb muscle of the 6-AN treated and the pair-fed groups were significantly reduced compared to the control group. In the plasma, the levels of total proteins and albumin were not affected, but the levels of globulin were significantly lower than those of the control and pair-fed groups. In contrast, the levels of glucose and creatine were significantly elevated. Cellulose acetate gel electrophoresis showed that 6-AN induced a new synthesis of prealbumin and also increased the levels of beta-globulin relative to the control and pair-fed groups. In contrast, the levels of gamma-globulin were markedly lower than those of the control group, whereas the levels of alpha-globulin were not affected. The specific activity of alkaline phosphatase of the 6-AN group was significantly lower than that of the control and pair-fed groups and that of aspartate aminotransferase only lower than that of the control group but not the pair-fed group. The specific activities of creatine phosphokinase and lactate dehydrogenase of the 6-AN group were the greatest among the three groups, whereas those of the pair-fed group were greater than those of the control group. The results suggest that 6-AN may interfere with the proper maintenance of energy charges and the immune system function.

Interpretation of the results of standard biochemical analyses, including BUN, alanine aminotransferase, aspartate aminotransferase, creatine kinase, gamma glutamyltransferase, bilirubin, ammonia, alkaline phosphatase, cholesterol, bile acids, glucose, albumin, globulins, calcium, phosphorus, prealbumin (transthyretin), fibrinogen, iron, and ferritin, is reviewed and discussed in relation to these physiological differences. The use and interpretation of alternative analytes appropriate for avian species, such as uric acid, biliverdin, glutamate dehydrogenase, and galactose clearance, also are reviewed. Normal avian urine and appropriate use of urinalysis, an integral part of laboratory diagnosis in mammalian species that frequently is omitted from avian diagnostic protocols, is discussed.
