Using rats, we studied how best to assess hepatic damage after administering therapeutic doses of each of five anti-cancer drugs or of the hepatotoxin, carbon tetrachloride. As indexes, we compared measurement of the concentration of administered antipyrine in plasma with measurement in serum of alpha-fetoprotein or of the activities of five enzymes that reportedly best reflect hepatic damage. The biological half-life of antipyrine in the plasma was increased more than threefold on pretreating the rats with any of the five cytotoxic drugs or with carbon tetrachloride. In contrast, the concentrations of alpha-fetoprotein, alkaline phosphatase,
gamma-glutamyltransferase
, or glutamate dehydrogenase were not consistently increased. Of the enzymes tested in serum,
aspartate aminotransferase
and ornithine carbamoyltransferase best indicated hepatic impairment resulting from the treatment with anti-cancer drugs. Our results imply that determination of the pharmacokinetics of marker drugs such as antipyrine better indicates hepatic dysfunction induced by cytotoxic agents than does measurement of the enzymes liberated into serum as a result of damage to liver mitochondria.

Serum gamma
glutamyl transpeptidase
(GGTP), isocitrate dehydrogenase (ICD), ornithine carbamoyl transferase (OCT), alanine aminotransferase (AlT),
aspartate aminotransferase
(AsT), and alkaline phosphatase (ALP) activities were assayed in 67 alcoholics and 40 drug dependent patients. Bilirubin, total protein, albumin, and globulin were also measured. GGTP elevation was observed in 48% of alcoholics and in 50% of drug dependents. The incidences of elevated levels of other enzymes were: ICD 39 and 38-7%; OCT 23-7 and 36-1%; AlT 30 and 33%; AsT 24-2 and 21-7%; ALP 10-4 and 5% respectively. Measurement of GGTP is thus more useful as a screening test for involvement of the liver in alcoholics and drug dependent patients than that of the other enzymes.

Chem. 19, 1122 (1973)] computer-controlled instrument system for sequential chemical testing to select and perform tests of hepatic status, to aid the clinician in the diagnosis of liver disease. , alanine aminotransferase, creatine kinase, and total and direct bilirubin. Kit methods for the latter four tests were evaluated on the system; results were similar to manual procedures. A software, enzymatic ratemeter was found to be better than the previously described hardware ratemeter. The follow-up tests of serum prescribed by the system are compared to clinician-prescribed follow-up tests and discharge diagnoses. In 10 of 19 cases, the system and clinician ordered similar follow-up tests; in three cases follow-up differed, and in six cases, the system ordered follow-up tests and the clinician ordered none.

and S-GT were still frequently seen at the end of the observation period. For S-pancreatic isoamylase, both decreases and increases from initial activities were frequently found. In patients with low activities initially, the S-pancreatic isoamylase activity increased during the observation period and thus, pathologically increased activities were recorded more often 9 days after alcohol withdrawal than on arrival at the hospital.

(GT) in 40 male chronic alcoholics were determined on admission to hospital. The serum activities of the enzymes were highest in patients with established alcoholism for less than 5 years. The serum concentration of ethanol, however, was lowest among these patients and gradually increased with the duration of alcoholism. No correlation was found between the serum ethanol level and the activity of any of the enzymes. The duration of the current debauch, which was shortest in cases of long-standing alcoholism, showed a positive correlation with the S-GT activity.

The methodology of a large prospective study on the influence of repeated anaesthetics on liver function is reported and the problems involved are discussed. The most suitable patients were those presenting for endoscopic examination of the bladder and urethra, for urethral dilatation and for cervical implantation of radium. Blood samples were taken immediately before induction of anaesthesia and on days 3-4 and 13-15 after operation, when a clinical assessment of the patient was also carried out. The effect of danazol in a dose of 600 mg a day was studied in 20 women with moderate or severe endometriosis. The clinical effect was found to be excellent and repeat laparoscopy after about 6 months treatment revealed a marked regression in all patients with only small residual foci of endometriosis in two of them. The side effects were few. The metabolic studies revealed a significant increase in serum
aspartate aminotransferase
(
AST
), serum alanine aminotransferase (ALT), serum potassium, serum albumin and serum creatinine, but a significant decrease in serum gamma
glutamyl transpeptidase
(GT). Serum sodium showed no alteration. A longitudinal study of basal plasma luteinizing hormone (LH) and follicle-stimulating hormone (FSH) and their responses to 25 microgram gonadotrophic releasing hormone (GnRH) i.v. as well as basal plasma levels of oestradiol, oestrone, progesterone and prolactin was performed. During treatment with danazol (600 mg a day) basal levels of LH, FSH, oestradiol, oestrone and progesterone were low but did not differ from the levels found in the early follicular phase of the menstrual cycle. On the other hand the pituitary response to GnRH was significantly greater for both LH and FSH than observed during the early follicular phase. These conflicting results are discussed. It seems that danazol inhibits the pituitary secretion of biologically active LH and FSH and this action is responsible for the decreased ovarian steroid secretion. Whether the atrophy of the uterine and ectopic endometrium is an effect of the reduced oestradiol levels or is a direct effect of danazol on endometrial oestrogen receptors, or a combination of both modes of action, is not clear.

Chromosomal morphology from cultured peripheral lymphocytes was studied in 81 men; 57 of the men were employed on plants manufacturing vinyl chloride or polyvinylchloride, 19 were on-site controls and 5 were off-site controls. There was a significant increase in chromosomal abnormalities in the exposed workers when compared with the controls. The greatest statistically significant increase in total B and total C cells occurred in autoclave operators, with smaller increases in other job categories. The increase in chromosomal aberrations was correlated with the length of exposure and with a history during the year prior to sampling (1973--1974) of exposure to excursion levels of vinyl chloride. Information on smoking habits was obtained 18 months after blood sampling and a positive correlation between these and total C cell abnormalities was found. There was no positive correlation with various other parameters (bilirubin, platelets,

The urinary excretion of D-glucaric acid, a catabolite of glucuronic acid, is considered to be a reliable index of the state of hepatic microsomal enzyme activity. Because enzyme activity may be altered in liver disease, we examined the effect of liver disease on the excretion of this metabolite and its correlation with liver function tests. We studied 89 patients with nonhemolytic jaundice, 39 with viral hepatitis, 33 with obstructive jaundice, six with cirrhosis, and 11 patients with jaundice of mixed etiology. Glucaric acid excretion was significantly increased in all these patients as compared to controls, most pronounced in the obstructive jaundice group. No correlation was found between glucaric acid excretion and concentrations of bilirubin, albumin, globulin,
aspartate aminotransferase
, alkaline phosphatase, cholesterol, or
gamma-glutamyltransferase
in serum, even though the concentrations of these analytes did vary with the type of liver disease. We suggest that this increase in glucaric acid excretion is an indication of normal or even increased glucuronidation (UDP-glucuronosyltransferase activity), which occurs in liver disease.
