A 48-year-old woman was admitted with neck tumors and cutaneous nodules. On the histological basis of the skin nodule biopsy, a metastatic anaplastic carcinoma was suspected. Immunohistochemical studies showed the presence of Ki-1 antigen, IL-2 receptor antigen, leukocyte common antigen (LCA), CD3 and CD4 on the tumor cells compatible with Ki-1 positive anaplastic large-cell lymphoma. This case was, however, finally diagnosed as adult T cell lymphoma (ATL) of a helper/inducer phenotype. She was born in Kagoshima. The serum anti-ATL associated antigen (ATLA) was positive. Southern blot analysis on the DNA extracted from the skin tumor cells showed a monoclonal integration of HTLV-1 proviral DNA. The results suggested that Ki-1 positive lymphomas may include a subset of ATL with a large-cell histology.

Differential expression of various isoforms of leukocyte common antigen (CD45), which arises from alternate mRNA splicing, identifies naive and memory populations of human T cells. Some memory (CD45RO+ CD45RA-) populations of CD4+ T cells from adult individuals express IL-2 receptor (IL-2R) alpha-chain (CD25), but naive (CD45RO- CD45RA+) CD4+ T cells only do so to a small degree. We found that a small but significant fraction of CD4+ T cells in neonatal blood expressed CD25, although most generally exhibited the phenotype of naive cells. It was demonstrated that purified neonatal CD25+ CD4+ T cells expressed mRNA for the IL-2R alpha-chain. Two-color immunofluorescence analysis disclosed that a CD25+ population of neonatal CD4+ T cells had the naive (CD45RA+ CD45RO-) phenotypes. These CD25+ CD4+ T cells from newborns could express mRNA for some specified lymphokines such as IL-4, IL-5, and interferon-gamma on activation in a similar manner to CD45RO+ (memory) CD4+ T cells from adults. Notably, polymerase chain reaction analysis demonstrated that neonatal CD45RA+ CD4+ T cells expressing CD25 contained spliced mRNA transcripts possibly encoding CD45RO in addition to CD45RA-associated transcripts, seemingly indicating that this population might be in the recently antigen-primed states. Such a small population of CD45RA+ CD4+ T cells expressing CD25 appeared to be present in the blood throughout human life. The results suggest that CD4+ T cells with the naive (CD45RA+) phenotype expressing IL-2R alpha-chain (CD25) represent the novel transitional population in the maturation process of naive into memory CD4+ T cells.

T cells can be divided into unprimed virgin (T0) and primed memory (T') subpopulations by their expression of different isoforms of the leukocyte common antigen. We have separated the CD4+ T cells into T0 and T' subpopulations and examined their capacity to respond to activation signals via the CD2 receptor molecule. On stimulation with a mitogenic combination of anti-CD2 antibodies, the T' population was induced to express IL-2 receptor, increased levels of the 4F2 antigen and to proliferate, whereas the response of the T0 populations was reflected solely by a minimal increase in the 4F2 antigen. The addition of IL-2 or monocytes to T0 cells stimulated with anti-CD2 antibodies did not enhance their expression of the IL-2 receptor or proliferation. However, T0 cells stimulated with the triad of anti-CD2 antibodies, monocytes, and IL-2 responded with high levels of IL-2 receptor expression and proliferation. The T0 subpopulation could also be induced to respond when cultured with anti-CD2 antibodies and phorbol myristate acetate. The results suggest that in order to respond to stimulation via the CD2 molecule, virgin T helper cells require additional signals that can be jointly provided by monocytes and IL-2. In contrast, memory T helper cells can be activated via CD2 signal transduction alone.

Sex steroid hormone action on target tissues is mediated through binding of estrogen and progesterone to specific intranuclear proteins, the estrogen and progesterone receptors (ER and PR). Therefore, in the present report the authors investigated for the presence of ER and PR in lymphoid cells of endometrial stroma that may serve as potential targets for estrogen- and progesterone-mediated effects in endometrium. The presence of ER was shown in nine proliferative and ten secretory endometria and the presence of PR in three secretory and one proliferative endometria. ER and PR were localized by monoclonal antibodies, to the nuclei of cells, with the use of, respectively, peroxidase-antiperoxidase (PAP) and avidin-biotin-peroxidase complex (ABC) methods. Lymphoid cells were then delineated by decoration of their plasma membranes with the use of monoclonal antibodies to HLA-DR, leukocyte common antigen (LCA), Leu-4 (CD3), and IL-2 receptor molecules with the use of an ABC staining procedure. A group of cells in the lymphoid aggregates in endometrial stroma showing membranous staining for HLA-DR, LCA, and Leu-4 molecules had nuclear ER. IL-2 receptor-positive cells were rare in endometrium, and no PR-positive cells were found in lymphoid aggregates. Furthermore, HLA-DR and ER were expressed in the glandular and surface epithelium in the proliferative phase and in occasional glands in the basalis in the late secretory phase. The presence of an ER-positive lymphoid cell population in endometrial lymphoid aggregates suggests that these cells may serve as target cells for estrogen.

A 59-year-old woman with a large nodular ulcerative lesion on her neck was presented. She had a 3 year history of recurrent cutaneous nodules which spontaneously regressed before regional lymphadenopathies appeared. She has followed an indolent clinical course for seven years after the first overt lymphadenopathies appeared. Histological findings were compatible with anaplastic large cell lymphoma (ALCL). The tumor cells strongly expressed Ki-1 (CD30), HLA-DR, IL-2 receptor (CD25) and leukocyte common antigen. These findings led to the diagnosis of primary cutaneous Ki-1+ ALCL. Although the majority of the tumor cells did not express T-cell related antigens, the detection of monoclonal TCR gene rearrangement clearly established the T-cell lineage nature.
