Squamous cell carcinomas (SCCs) of the mouse skin, as well as several types of preinvasive carcinoma precursor lesions, were produced by complete carcinogenesis protocols with benzo[a]pyrene (B[a]P). Groups of mice were studied histologically at several time points. Tumors and precursor lesions were systematically counted on microscope slides. The main feature of tumor development using this ubiquitous human carcinogen was the sequential appearance of in situ flat lesions with progressive degrees of
dysplasia
. These changes, preceding the development of SCCs, were observed 20 weeks after beginning the carcinogen treatments. At this time point, in situ lesions outnumbered SCC approximately 10:1 at the higher total carcinogen dose examined. Ten weeks later, this ratio was approximately 1:1. With the lower total carcinogen dose protocol, progression was delayed since at 27 weeks preinvasive lesions outnumbered SCCs approximately 8:1. In addition to the in situ lesions, papillomas and keratoacanthomas were noted with the high B[a]P dose protocol, but tended to disappear at the end of the experiment, also indicating their probable role as SCC precursors. A study of histochemical markers showed that gamma-glutamyltranspeptidase (GGT) and keratin 13, although good markers of malignant changes in early papillomas produced by two-stage carcinogenesis protocols, were mainly negative in dysplastic lesions produced by complete carcinogenesis with B[a]P. Immunohistochemical detection of
p53
showed that 50% of SCCs were positively stained, whereas only 3% of in situ lesions were
p53
immunoreactive. Similarly, 62% of SCCs were immunohistochemically positive for cyclin D, but no precursor lesions were positive. Molecular analysis of the tumors showed the absence of H-ras mutations. No amplification of the cyclin-D-1 gene was detected in eight SCCs examined. Collectively, these findings indicate that preinvasive in situ lesions are frequent during early stages of carcinogenesis when B[a]P is used in a complete carcinogenesis protocol. Although the absence of
p53
immunoreactivity in this mouse model differs from the observed changes in human premalignant squamous lesions, the sequence of morphological changes and the final incidence of
p53
and cyclin D staining abnormalities are very similar to the well-known alterations that take place during human squamous carcinogenesis.

Leukoplakia has evolved as a clinico-pathologic concept over many years, with the current clinical designation being accepted worldwide. Reflective of the biology of leukoplakia is the concept of cellular atypia and epithelial
dysplasia
. Adding to a better understanding of leukoplakia in general has been the definition of relevant clinical subsets which, in some cases, includes etiology (snuff), while in other cases a verrucous clinical appearance will suggest a more aggressive anticipated behavior pattern. Tobacco usage, in many of its forms, remains the prime etiologic factor; however, other considerations also apply. More recently, the potential etiologic role of Candida albicans has been stressed, as well as its possible role in carcinogenesis. So-called oral hairy leukoplakia has been defined in relation to a possible Epstein-Barr viral infection, usually in the immunosuppressed patient. Other viruses, human papilloma virus in particular, have been implicated in leukoplakia, while genetic alterations involving tumor suppressor elements (
p53
) have also been investigated. Finally, the management of this common condition remains a variable and includes local, topical, and systemic therapies such as anti-oxidants, carotenoids, and retinoids.

The expression of the
p53 protein
was investigated in flat serrated neoplasias as well as in other histological phenotypes of flat or exophytic hyperplasias or neoplasias of the colorectal mucosa. A total of 104 such lesions were analyzed: 24 were flat serrated neoplasias (22 flat serrated adenomas and 2 flat serrated adenocarcinomas), 26 flat tubular adenomas, 17 flat hyperplastic polyps, 29 exophytic tubular and/or villous neoplasias (23 adenomas and 6 exophytic adenocarcinomas) and the remaining 8, exophytic hyperplastic polyps. Deparaffinized, rehydrated sections were treated immunohistochemically to detect those overexpressing the
p53 protein
. Lesions having slight (+), moderate (++) or intense ( ) staining were considered immunoreactive. The results showed that 50% of the flat serrated adenomas with low-grade
dysplasia
(LGD) and 66.7% of those with high-grade
dysplasia
(HGD) had
p53
immunoreactivity. None of the flat tubular or of the exophytic adenomas with LGD expressed
p53
, but immunoreactivity was present in 61.5% of the flat tubular adenomas with HGD and in 52.3% of the exophytic adenomas with HGD. All adenocarcinomas had an intense
p53
reaction. Weak
p53
expression was demonstrated by 11.7% of the flat hyperplastic polyps but none of the exophytic polyps reacted. The occurrence of
p53
expression in flat serrated adenomas with LGD suggested that, despite its low histological profile, one-half of those lesions could be biologically already committed to independent growth. The occurrence of
p53
expression in nearly 12% of the flat hyperplastic polyps was totally unexpected and deserves further investigation. Flat serrated adenoma emerges as a novel, independent histological entity among the various phenotypes of flat neoplasias of the colorectal mucosa.

Numerous molecular genetic events occurring in the development of sporadic colorectal neoplasia have been previously defined. The most frequent genetic alterations are mutations of the APC, KRAS, and
TP53
genes, as well as loss of the DCC gene and of the second
TP53
allele. The data from several groups indicate that these genes play an important role in ulcerative colitis-associated dysplasias and cancer, as they do in sporadic colorectal adenomas and carcinomas. KRAS and
TP53
mutations were detected in
dysplasia
, but also in villous regeneration and active colitis, and affect a subpopulation of the cells composing these lesions. We conclude that in histologically defined
dysplasia
, clones can be found that genetically represent precancerous lesions in ulcerative colitis. Seen in this way, part of the active colitis and villous regeneration lesions might be considered as preneoplastic. When present, KRAS mutation is an excellent genetic marker to map populations of preneoplastic cells.

In the Barrett's oesophagus (BE) progression from metaplasia, via
dysplasia
, into invasive cancer, an aberrant cell proliferation governed by genetic change plays a central role. Alterations of the
p53
tumour-suppressor gene appear especially critical and, like the proliferation marker Ki67, can be detected by immunohistochemistry. The purpose of this study therefore was to investigate the clinical value of
p53
and Ki67 as markers for tumour progression in BE, and at the same time test the validity of the concept of a metaplasia-
dysplasia
-carcinoma sequence in BE by correlating the expression of these markers with various grades of
dysplasia
. Thirty-two lesions (seven negative for
dysplasia
, five indefinite for
dysplasia
, 11 low-grade
dysplasia
and nine high-grade
dysplasia
) from 25 archival resection specimens were selected for study. Increasing grades of
dysplasia
showed increasingly
p53
accumulation;
p53
accumulation was never observed in mucosa without
dysplasia
. The increasing
p53
expression was accompanied by an increased Ki67-labelling index and an upward shift of the proliferative compartment. The results lend support to the multistep progression model of a metaplasia-
dysplasia
-carcinoma sequence in BE. Expression of
p53
and Ki67, markers which can be easily applied on archival material, can be valuable adjuncts for the histopathological diagnosis of
dysplasia
and may have predictive value for cancer risk.

Although expression of the bcl-2 protein has been investigated in a number of non-haematological malignancies, little is known of its distribution in premalignant lesions. Expression of bcl-2 was investigated immunohistochemically in archival biopsies of normal (n = 8) and dysplastic bronchial epithelium (n = 56) and in 31 bronchial resection margins and their corresponding carcinomas. All dysplasias had lost the prominent basal staining pattern seen in histologically normal epithelium. Two were negative and six had occasional basal positive cells. In 37 cases up to 66% of the epithelial cells throughout the full epithelial thickness were bcl-2 positive with weak to moderate staining intensity. In 11 cases, all severe dysplasias, strong expression was observed in > 90% of the epithelial cells. Four patterns of bcl-2 expression in dysplasias were identified and an increasingly aberrant pattern of bcl-2 expression correlated with an increasing grade of
dysplasia
(Spearman's rank correlation, P < or = 0.0001). Sixty-five per cent of the carcinomas contained bcl-2-positive cells. Patients with non-small-cell lung carcinomas (n = 27) in which > 50% of the tumour cells were bcl-2 positive showed a survival advantage compared with those with 0-25% bcl-2-positive cells (P = 0.02). No correlation was found between
p53
expression (Walker et al., 1994) and bcl-2 expression in dysplasias or carcinomas.

We screened for
p53
alterations in 71 early gastric cancers of differing histological types and growth patterns, 18 advanced cancers of diffuse type, 19 dysplastic lesions, and 12 extensive intestinal metaplasia cases. Tumors were investigated for gene mutations (exons 5-8) with PCR-based denaturing gradient gel electrophoresis and sequencing techniques, and for protein accumulation with immunohistochemical methods. Nontumor samples were studied with immunohistochemistry alone. Of the early cancers, intestinal tumors showed a much higher
p53
mutation frequency (41%) than did diffuse cancers (4%). When comparing early and advanced tumors of the same type, we observed a similarity in mutation frequency (41 versus about 50%) for intestinal tumors, and a significant increase for diffuse tumors (from 4 to 33%). Immunopositive case distribution between tumor types and stages paralleled that of mutated cases. Immunohistochemical and genetic analysis gave concordant results for all samples with gene mutations. Eighteen of the 65 (28%) nonmutated tumors displayed significant immunoreactivity. Early tumors that massively penetrated the submucosa, i.e., the early tumors for which prognosis is worst, showed the highest frequency both of
p53
gene mutation and of nonmutated protein accumulation. Twelve of 19 dysplastic lesions showed significant immunoreactivity, whereas intestinal metaplasias proved unreactive in all but a few cells. Our results yield two implications: that
p53
alterations have a crucial and early role in gastric carcinogenesis of intestinal type, likely acting at the transition step between metaplasia and
dysplasia
; and that the alterations are mainly associated with tumor progression in cancer of diffuse type.

We studied immunohistochemically the distribution of

overexpression and Ki-67 as a marker of cell proliferation, in 2 cases of advanced colorectal carcinomas occurring in UC, in comparison with histopathological features such as carcinoma and
dysplasia
. One is a case of 4 independent colorectal carcinomas surrounded by widespread

, and the other is a case of solitary rectal carcinoma. In these 2 cases, formalin-fixed paraffin-embedded tissues covering all of the resected specimens were examined for

. Some areas of the dysplastic mucosa histologically represented "hyperplastic pattern of villous feature." In these areas

and Ki-67 are useful diagnostic markers for grading the degree of
dysplasia

seems to be consistent with that in cancers occurring in the surrounding dysplastic area in most of our cases.

This study examined the association between 17p allelic loss and
p53
gene mutation in a series of 16 esophageal adenocarcinomas arising on a background of Barrett's esophagus. Two highly polymorphic dinucleotide repeat polymorphisms mapping to 17p13 were analyzed to assess the frequency of 17p allelic loss in these tumors. Mutations in the
p53
gene were detected by direct DNA sequencing. Ninety-four % (15 of 16) of samples were informative at one or both polymorphic loci. Allelic loss at one or both loci was detected in 80% (12 of 15) of samples. Mutations were detected in 69% (11 of 16) esophageal adenocarcinomas, and there was a close association between 17p allelic loss and
p53
gene mutation (P = 0.00879; Fisher's Exact Test). The tumors that were analyzed demonstrated a specific
p53
mutation spectrum, with G:C to A:T base transitions at CpG dinucleotides accounting for 80% (8 of 10) of single-base substitutions. In three cases, the same
p53
mutation was detected in both high-grade
dysplasia
and adjacent tumor. These results indicate that
p53
gene alterations contribute to the development of esophageal adenocarcinoma and precede the development of invasive carcinoma in patients with Barrett's esophagus.

Protein accumulation in early gastric carcinoma was studied in relation to the histological type (Lauren classification) and the type of growth pattern, including the chronology of
p53 protein
accumulation during carcinogenesis. Forty five, paraffin embedded gastrectomy specimens from early carcinomas were examined for the presence of chronic atrophic gastritis, subtypes of intestinal metaplasia, and
dysplasia
. The Lauren type and the type of growth pattern were reassessed for all early carcinomas.
p53
Protein accumulation was examined using the monoclonal antibody DO-7. Complete absence of
p53 protein
accumulation was observed in normal gastric mucosa, chronic atrophic gastritis, and intestinal metaplasia, irrespective of subtype. In gastric
dysplasia
(one mild, two moderate, and one severe), only severe
dysplasia
was
p53
positive. Intestinal type (n = 20) and diffuse type early gastric carcinomas (n = 25) were
p53
positive in 70% and 52% of cases, respectively. Both tumour types differed significantly in the percentage of

positive tumour cells per tumour (p < 0.01) and in staining intensity (p < 0.05). No significant difference in
p53 protein
accumulation was found between early carcinomas with different types of growth pattern. It is concluded that

accumulation--usually reflecting missense
p53
gene mutation--seems to be a late event in gastric carcinogenesis. Moreover, it is suggested that missense
p53
gene mutation occurs in a final pathway common to both intestinal and diffuse type of early gastric carcinoma. Finally, the types of growth pattern do not seem to differ in
p53 protein
accumulation.
