In a group of 34 parous women, mean age 36.2 years, fitted with a copper-releasing IUD (18 with Multiload 250 or 375, and 16 with Nova T) the following parameters were assessed before and 4 months after the device insertion: hemoglobin, hematocrit, iron and
ferritin
. The only complaint of all patients was the shortening of the menstrual cycle, from 28 +/- 1.2 to 26.8 +/- 1.9 days, and the longer period of days with vaginal spotting (from 3.9 +/- 1.1 to 7.1 +2- 1.0 days). While Hb, HTC and iron remained within normal limits, there was a drastic fall in
ferritin
, from 24.2 +/- 16.4 to 9.3 +/- 8.7 ng/ml (p less than 0.01), a sign of iron deficiency anemia. Periodic
ferritin
assessment and iron treatment, if needed, should be the first choice. Removal of the IUD is recommended only when
ferritin
remains low after antifibrinolytic,
prostaglandin synthetase
inhibitors, and iron therapy.

Platelets incubated with
prostaglandin synthetase
inhibitors, i.e. aspirin (ASA) and diclofenac-sodium (Voltaren) show a decreased capability to aggregate. While ASA induces the complete loss of platelet pseudopodia, Voltaren causes the appearance of long, needle-shaped pseudopodia, a finding which may suggest a rather increased tendency to aggregate. Platelets labeled with
ferritin
hydrazide and examined with a transmission electron microscope and by X-ray analysis show a greater deposition of
ferritin
hydrazide after incubation with Voltaren in comparison to those incubated with ASA and to the controls. This finding suggests an increased negative charge of the stretched platelet membrane after incubation with Voltaren with a subsequent increase in the repulsive forces between platelets.

A prospective study involving 50 women recruited from a district general hospital in South Wales indicated that the levonorgestrel-releasing intrauterine system (LNG-IUS) represents an effective nonsurgical treatment for menorrhagia and dysmenorrhea. Treatment for menorrhagia with a combination of
prostaglandin synthetase
inhibitors and antifibrinolytic drugs had failed in these women, and they were on a waiting list for hysterectomy or transcervical resection of the endometrium. Menstrual loss was estimated using a modified pictorial chart and the full blood count and
ferritin
were measured preinsertion and at 3 and 6-9 months postinsertion. The device was spontaneously expelled in 6 women and almost all subjects experienced some unscheduled bleeding during the first 6-8 weeks postinsertion. Of the 42 women who attended the 3-month visit, 37 were satisfied with the results of the LNG-IUS device and wished to continue; only 5 had no significant reduction in their menstrual scores. Also observed was a reduction in clots and "flooding" and marked improvements in associated dysmenorrhea. 4 women became amenorrheic and 28 were cured of their premenstrual syndrome symptoms. There was no significant change in the full blood count or
ferritin
. At the 6-9 month visit, 4 of the 5 women who previously reported no change in menstrual scores now reported their menstrual loss was acceptable.

The mRNA levels of 39 target genes were monitored in unfertilized eggs of 14 rainbow trout sampled the day of ovulation and again 5, 14, and 21 days later. For all 56 collected egg batches, an egg sample was fertilized to estimate egg quality by monitoring embryonic development. Remaining eggs were used for RNA extraction and subsequent real-time PCR analysis. A significant drop of egg quality was observed when eggs were held in the body cavity for 14 or 21 days post-ovulation (dpo). During the same period, eight transcripts (nucleoplasmin or Npm2,
ferritin
H, tubulin beta, JNK1, cyclin A1, cyclin A2, cathepsin Z, and IGF2) exhibited a differential abundance at one or several collection time(s). Interestingly, we observed higher levels of cyclins A1 and A2 mRNAs in eggs taken 5 days post-ovulation than in eggs taken, from the same females, at the time of ovulation. In addition, seven transcripts exhibited a differential abundance between low quality and high quality eggs. Low quality eggs were characterized by lower levels of Npm2, tubulin beta, and IGF1 transcripts. In contrast, keratins 8 and 18, cathepsin Z, and
prostaglandin synthase
2 were more abundant in low quality eggs than in high quality eggs. In this study, we have demonstrated differences in mRNA levels in the rainbow trout egg that are reflective of developmental competence differences induced by post-ovulatory ageing. The putative role of these transcripts in post-ovulatory ageing-induced egg quality defects is discussed with special attention for corresponding cellular functions.
