We evaluated the clinical significance of serum CA 19-9 in various cancers, benign diseases and healthy controls. The percentage of positive cases for CA 19-9 (higher than 37 U/ml) was 83% in pancreas cancer, 78% in biliary tract cancer and 75% in urinary tract cancer. Benign diseases showed a low frequency of positive cases for CA 19-9, and their serum levels of CA 19-9 were low. Benign diseases with high serum levels of CA 19-9 were rarely seen, but they were easy to differentiate from malignant tumors by simultaneous examination of other tumor markers. In cancer patients with high serum levels of CA 19-9, CA 19-9 showed a weak positive correlation to CEA and ferritin.

Over 100 patients have received cyclic treatment with polyclonal 131I labeled anti-ferritin and anti-carcinoembryonic antigen (CEA) antibodies from different animal species (rabbit, pig, cynomolgous monkey, bovine, and baboon). Because survival was prolonged from original cyclic treatment, retreatment with original antibodies (recycling) became a necessary consideration. An assay using autoradiography of Ouchterlony gels, with diffusion of patients' sera against the varied radiolabeled antibodies, was developed to detect anti-antibody precipitin bands. Anti-antibody could be detected with a sensitivity to the 60 ng level. Sera from 35 patients given from 1 to 7 separate cycles (2 injections/week, total antibody 6 mg/cycle) of radiolabeled foreign antibody were studied for the production of anti-antibodies. Anti-antibodies were detected in 11 of 22 primary hepatoma patients studied, 3 of 4 intrahepatic biliary cancer patients, and 0 of 9 Hodgkin's disease patients. In all but two of the patients, the anti-antibodies produced were specific for the species used in the treatment of the patient. Eight patients were reinjected (recycled) with previously used antibodies and the presence or absence of precipitin bands correlated with the ability of these antibodies to deposit in the tumor or to be rapidly degraded. The importance of this assay is its simplicity, sensitivity, and the rapid detection of anti-antibody activity for patients requiring treatment with radiolabeled antibodies.

We have measured serum ferritin level using double antibody radioimmunoassay kit (Eiken ICL) and evaluated the characteristics of the kit and clinical usefulness. Satisfactory results were observed in standard curve, reproducibility, dilution and recovery test. In clinical evaluation, we have measured in normal subjects and patients with various diseases. The range in normal males and females were 13.0-158.7 ng/ml and 7.3-73.0 ng/ml, respectively. Serum ferritin level was elevated in patients with hepatoma, biliary cancer, lung cancer and other malignant diseases. Measurement of serum ferritin value would be useful in the monitoring of cancer patients.

To assess the diagnostic significance of CA19-9, the serum levels in 225 healthy subjects, 201 patients with cancers, 423 patients with benign diseases and 21 pregnant women, were determined by RIA. The mean CA19-9 level of the healthy subjects was 11.2 +/- 0.4 U/ml (range, 6-100 U/ml). Only 3.1% of them were above 37 U/ml. The CA19-9 levels were elevated above 37 U/ml in 7.9% of 293 patients with non-carcinomatous diseases of the digestive system. Among digestive system cancers, elevated levels were found in 18.2% of 11 patients with esophageal cancer, 42.7% of 68 patients with gastric cancer, 39.1% of 23 patients with colorectal cancer, 27.8% of 18 patients with primary hepatic cancer, 71.4% of 35 patients with biliary cancer, and 75% of 20 patients with pancreatic cancer. Most of the patients with levels above 100 U/ml had carcinomatous diseases. The CA19-9 positive rates for patients with gastric cancer and colorectal cancer were extremely low at stages I, II and III, while in patients at stage IV and in patients with recurrent cancer, a tendency for rapid increase in the positive rates and concentrations of CA19-9 was noted. Based on combination assay of CA19-9, CEA and ferritin, in comparison with the positive rates for CA19-9 alone, it was found that the rates were raised to 42.7% in gastric cancer, to 39.1% in colorectal cancer, and to 71.4% in biliary cancer, suggesting the simultaneous determination with these tumor markers may serve to elevate their usefulness.

Studies on the purification, characterization and clinical application of pancreatic oncofetal antigen were reported. This antigen was purified from fetal pancreas, and migrated in the beta-region on electrophoresis. Its molecular weight was about 80 x 10(4) daltons on gel filtration with a Sephacryl S-300. This antigen is clearly different from other oncofetal antigens such as alfafetoprotein, carcinoembryonic antigen or ferritin. Clinically, this pancreatic oncofetal antigen was positive in sera of 68.4% of the patients with pancreatic cancer. However, elevated level of this antigen was also observed in the sera of some patients with biliary tract cancer, colon cancer or gastric cancer. The antigen was also found in pancreatic juice obtained from patients with pancreatic cancer in almost the same incidence as in their sera. It is suggested that a pancreatic oncofetal antigen assay of sera and pancreatic juice in combination with other oncofetal antigens is valuable for the diagnosis and monitoring the clinical course of pancreatic cancer.
