One case of spontaneous Th 10 - Th 11 dislocation complicating
ankylosing spondylitis
is reported. The only functional consequences were cracks when the patient was moving. Surgical treatment was refused nevertheless after twelve year follow bone union had occurred without neurological impairment.

Sixteen cases of spontaneous dislocation of the atlanto-axial joint have been reviewed, fifteen in rheumatoid arthritis and one in
ankylosing spondylitis
. On four occasions there was some neurological impairment, but in eleven there was only pain and stiffness. The only case of
ankylosing spondylitis
was operated on after failure of conservative treatment. Three patients died between one and eight months after operation, two of them having had severe neurological impairment before operation. In the thirteen other cases, the average follow up was four years. All had satisfactory fusion after grafting from the skull to axis. Six had only a partial reduction of the displacement and united with some displacement of about 5 mm. The results were satisfactory with absence of pain and regression of neurological signs.

Self-efficacy is an important factor influencing exercise behavior in samples of students and the general population; however, few studies have examined the relevance of self-efficacy in understanding exercise in the context of chronic disease, wherein exercise is integral to treatment rather than a leisure pursuit. Further, the influence of disease factors, e.g., pain, on performance of therapeutic exercise is unknown. The present purpose was to examine the role played by self-efficacy in a sample of 169 people with one type of arthritis,
ankylosing spondylitis
. This painful, disabling and progressive condition has an early age of onset and thus necessitates adherence to therapeutic exercise over long periods of time. Analysis showed that concurrent severity of disease had a greater influence on exercise self-efficacy than past attainment. There was no evidence that self-efficacy mediated outcomes. Similar to findings for healthy people, respondents uniformly valued exercise but did not translate these beliefs into action.
