A 71 year old woman suffering from ankylosing spondylitis with aggressive peripheral joint disease for 46 years developed unusual radiographic features in the hips and knees. The pathogenic processes responsible are uncertain, and no histology is yet available. Although she had been treated with radiotherapy to the knees, there was no similar history to explain the hitherto unreported appearance of the hips.

A fracture of the cervical spine is a dangerous and life-threatening injury in patients suffering from ankylosing spondylitis. A 71-year-old motorist with a long history of ankylosing spondylitis was injured during a car accident. A rare transverse fracture through the body of C6 occurred and in addition a lesion of the trachea was found. This combination of injuries is described for the first time. Special problems relating to the initial rescue, diagnosis and treatment are discussed according to the literature. The necessity of handling the cervical spine with extreme caution during all medical procedures, taking into account the kyphotic deformity is stressed.

A 71-year-old man with ankylosing spondylitis and an unstable fracture of the 6th and 7th cervical vertebrae was managed with a halo vest. Eight weeks following application the halo had shifted because of a loose pin. The patient's only complaint at the time was a headache but this was followed two days later by a seizure. An MR scan of the brain showed a swollen cortex under the right dorsal pin as a result of a perforation of the internal lamina by the pin. The halo was removed and anti-epileptic medication commenced. The patient had no further seizures.
