A 57-yr-old man with a chronic lung cavity presumed to be related to ankylosing spondylitis and/or old cavitary tuberculosis presented with hemoptysis and rapidly developed lower extremity paresis and hypoesthesia. On chest radiograph he had a left upper lobe lesion suggestive of aspergilloma combined with a large left empyema with bronchopleural fistula. Serologic analysis demonstrated precipitins and very high titer IgG antibodies to Aspergillus fumigatus antigens. Decompressive laminectomy from T1 to T5 was performed, with drainage of A. fumigatus culture-positive material from an epidural abscess compressing the spinal cord. Chest drainage was required for control of the empyema. With a total course of 3 g of intravenously administered amphotericin B, rehabilitative therapy, and chronic empyema drainage, he is now at home and ambulatory with assistance. He is also being followed by regular serum assays of IgG antibodies to Aspergillus proteins. We report the case of an apparent long-term survivor of a formerly lethal and/or nonreversible paraplegic condition. The critical factors compared with previous cases with a poor outcome would appear to be prompt neurosurgical intervention, restoration of a normal number of T-cells, effective long-term chest drainage, and high dose amphotericin treatment.

A highly significant association was found between the bovine MHC class I antigen BoLA-A8 and a form of vertebral osteophytosis/ankylosing spondylitis known as chronic posterior spinal paresis (PSP) in Holstein bulls (P < 0.001). In a population study, restricted to unrelated bulls, BoLA-A8 was significantly associated with PSP (P = 0.0015) with a relative risk of 34.6. In a family study, one PSP bull, BoLA A8/A20, sired 13 offspring. BoLA-A8 was significantly associated with PSP (P = 0.0008). All five PSP sons inherited the A8 allele and the eight healthy sons each inherited the A20 allele. In three other families a complete association of BoLA-A8 and PSP was observed. Lod score analysis, using all available families, indicated a significant linkage between BoLA and PSP (lod score = 6.9). Based on clinical observation, pathology, age/sex predilection, and a strong association with a class I MHC molecule, this inflammatory disease appears analogous to the human condition known as ankylosing spondylitis.

Surgery in late stage ankylosing spondylitis (AS) most often tends to correct the sagittal balance with an extension osteotomy of the spine. In the literature, extension osteotomy was first described as an open wedge osteotomy but recently closed wedge osteotomy resecting the pedicles and posterior elements have become more popular. Only a limited number of cases have been reported in the literature and with limited focus on outcome of this major surgery. In this study, we reported the results of a large series of extension osteotomy in a population of patients with AS focusing on the technical aspects, complication rates, correction obtained and outcome evaluation using newer spine outcome measuring instruments. In the period from 1995 to 2005, 36 consecutive patients fulfilled the criteria where the files, radiographs and patients were available for further studies. The following data were recorded: Age, sex, comorbidity, indication, operation time and blood loss, level of osteotomy and estimated Correction. Furthermore, perioperative complications and all late complications were registered. The average follow-up was 50 months (3-128). Twenty-one patients also filled out questionnaires (SF36 and Oswestry Disability Index) preoperatively. At the end of the period all patients were contacted and filled out the same questionnaires. Fifteen of the patients had two pedicular resection osteotomies performed, 21 had one, and two had polysegmental osteotomies. Mean operation time was 180 min, bleeding was mean 2,450 ml, stay at the hospital was 13 days. One patient had partial paresis of the lower extremities all other complications were minor. The median correction was 45 degrees . The median Oswestry score improved significantly from 54 (range 20-94) preoperatively to 38 (range 2-94) postoperatively. The SF-36 score significantly increased, when evaluated on the major components Physical Component Summary (PCS) and Mental Component Summary (MCS). The thoracolumbar closed wedge pedicular resection osteotomy used in this series was a safe method for correction of incapacitating kyphosis in AS. There was an acceptable rate of perioperative complications and no mortality. The correction obtained was in average 45 degrees . All of the patients except one maintained their good correction and restored function. Outcome analysis showed a significant improvement in SF-36 and Oswestry Disability Index, and the mental component of the SF-36 showed improvement to values near the normative population. It is concluded that corrective osteotomy of the thoracolumbar spine in AS is an effective and safe treatment with improvements in quality of life.

A 21-year-old patient with ankylosing spondylitis under treatment with the TNF-alpha inhibitor infliximab developed a multifocal, demyelinating axonal neuropathy affecting several peripheral nerves simultaneously (mononeuritis multiplex). This represents an additional rare peripheral nervous system side effect of infliximab therapy. The underlying cause is unknown. Intravenous immunoglobulin therapy (0.4 g/kg per day for five days) led to a complete regression of muscle paresis and sensory defects in this case.

In this study, we aimed to evaluate the audiovestibular functions in the patients with ankylosing spondylitis (AS). This prospective study was performed in collaboration by the Otolaryngology and Rheumatology Departments of Bozok University School of Medicine between May 1, 2012, and January 1, 2013. We studied 80 subjects consisting of 40 AS patients (37 men and 3 women) in whom the diagnosis confirmed by the criteria of New York and 40 healthy controls (35 men and 5 women). All participants were evaluated by routine audiologic (including tympanometric evaluation, pure-tone audiograms, speech tests) and vestibular studies (including spontaneous nystagmus, gaze, optokinetic, saccadic movements, smooth pursuit, caloric test and Dix-Hallpike tests). The tympanometric values did not show a statistically significant difference between the AS group and the healthy subjects (p > 0.05). At low frequencies (250, 500, 1,000, and 2,000 Hz) pure-tone audiologic evaluations also proved statistically non-significant results at mean air conduction thresholds (ACT) and bone conduction thresholds (BCT) between the AS and control groups (p > 0.05). At high frequencies (4,000, 6,000, and 8,000 Hz), the ACTs and BCTs in AS group were lower than control group which was statistically significant (p < 0.05). The results of spontaneous nystagmus, gaze, optokinetic, canal paresis and saccadic movement tests between the two groups were statistically insignificant (p > 0.05). The comparison of smooth pursuit and Dix-Hallpike tests reached statistical significance (p < 0.05). Videonystagmographic test (VNG) revealed central abnormalities in 7 patients (17.5 %), peripheral abnormalities in 16 patients (40 %), and mixed abnormalities in 3 patients (7.5 %). Our findings suggest a possible association between AS and audiovestibular system dysfunction. We assume that the hearing and vestibular disturbances in AS are more prevalent than previously recognized.
