Recent developments in genetic tissue typing have altered concepts of
ankylosing spondylitis
(AS) or variants of this spondylitic arthritis. It is now apparent that AS is separate and distinct from rheumatoid arthritis; and the term "rheumatoid" should be avoided except as it applies to the latter, usually characterized by rheumatoid factor or nodules and symmetrical arthritis of peripheral joints. Classical definitions of AS require X-ray evidence of sacroiliitis and/or restriction of chest expansion. Recent studies suggest that many patients, especially women, may have symptoms of AS without typical roentgenographic or clinical findings. Use of the HL-A B27 genetic test is useful for identifying AS patients. Radioisotope bone or joint scanning techniques further augment present diagnostic capabilities. Atypical AS may be a very common form of morbidity among patients with back pain in the United States. Since patients have a tendency to form osseous reankylosis after total hip replacement, it is especially important for orthopedic surgeons to identify incipient AS and related disorders. Inappropriate treatment or procedures may be avoided by use of these newer clinical tools in the evaluation of patients with chronic back or skeletal

Fourteen children with juvenile rheumatoid arthritis and two with
ankylosing spondylitis
received 29 total hip replacements (THR). The ages at THR were 12 to 18 years, the median duration of prior hip disease was 7.1 years, and the minimum followup was one year (range 1--4 years, median 2.2 years). The primary indication for THR was
pain
in 5 hips and severe malposition or flexion contractures in 24 hips. (Sixteen of these also had some degree of
pain
.) All 29 hips demonstrated improved postoperative range of motion, and all were free of
pain
. In children, active rheumatic disease in other joints, prosthesis longevity, and possible lack of adequate motivation all merit special consideration prior to THR, but the results are encouraging.

A study comparing 12 patients with
ankylosing spondylitis
(AS) to 25 with osteitis condensans ilii (OCI), referred to a rheumatic disease center, was carried out to determine whether OCI represents a varient of AS in women. In the group with OCI, chronic lumbodorsal
pain
was present in 9, 36%, a 'fibrositis' syndrome in 6, 24%, and 16, 64% had recurrent episodes of polyarthralgia. A definite arthritis with effusion developed in 10 patients, 40%. No patient with OCI had iritis or colitis, whereas 4 patients with AS had iritis and four had colitis. Radiographs of the spine showed no evidence of spondylitis in the OCI group. Of the 25 patients with OCI, only 2, 8% were HLA B27 positive compared with 11 of 12 patients with AS, 92%. These results suggest that OCI is not a variant of AS in women.

A multicentre double-blind clinical trial was conducted in general practice in which imipramine (Tofranil) was added to existing standard analgesic anti-rheumatic therapy in patients suffering from osteo-arthritis, rheumatoidtero-arthritis, rheumatoid arthritis or

. Twelve doctors admitted sixty-five patients to the trial. Fifty-five patients completed an eitht-week treatment period. According to clinical assessments imipramine brough about statistically significant improvements in

Pain
in osteoarthritis is always caused by secondary inflammation. Due to this data, the possibilities of treatment are discussed, especially those of nonsteroidal drugs.

We have experienced six patients of juvenile
ankylosing spondylitis
. Their chief complaints were polyarthralgia or coxalgia, however, any
pain
or stiffness in lumbar regions was absent. These clinical features were atypical for those of
ankylosing spondylitis
, and much more like those of a peripheral arthropathy such as juvenile rheumatoid arthritis. After a long-term follow-up, all the patients showed definite clinical features of
ankylosing spondylitis
. HLA-B27 was found in 4 out of 6 cases. This frequency is almost equal to that of adult
ankylosing spondylitis
patients. HLA typing was very important in the early diagnosis.

Hip involvement in
ankylosing spondylitis
(AS) is a common and disabling problem. The clinical and x-ray records of 87 patients with definite AS (Rome criteria) were examined to define and characterize their hip disease. Clinical hip disease was present in 33 cases (38%), was usually bilateral (91%), and tended to begin early in the disease course; it was the cause of 50% of the Class III and IV disability in the entire study group. Typical findings included regional
pain
, limitation of motion, muscle atrophy, and flexion contractures. Radiologic hip abnormalities occurred in 42 cases (48%). The radiographic pattern was distinctive when compared to that in two control groups and included axial migration of the femoral head (63%), concentric joint space narrowing (50%), rufflike femoral osteophytosis (36%), and protrusio acetabuli (30%). Eight patients required bilateral hip surgery. Para-articular ossification occurred in 8 of 16 replaced hips; in 5 of 8 hips it caused clinical immobility. This potentially serious complication may limit the usefulness of hip arthroplasty in some AS patients.

Thirty-nine Caucasian women with definite or probable
ankylosing spondylitis
were tissue typed and the radiographic and clinical features were correlated with the presence of B27 (formerly W27). This investigation showed that the same strong positive correlation observed in men obtains in women between B27 and unequivocal radiographic changes. As such changes develop slowly in women, clinical and laboratory features were identified which were sufficiently characteristic to enable the diagnosis to be reached in advance of decreased spinal mobility. Participation in open and double-blind crossover studies in rheumatoid arthritis confirmed that naproxen improved
pain
and stiffness. This observation suggested that naproxen might be effective in
ankylosing spondylitis
. The initial trial was open, but at six months, a double-blind crossover "placebo pulse" was superimposed on the open trial. Thirty-six patients entered the trial taking a daily dose of 500 mg naproxen. At the end of the first month, 35 assessed naproxen as being equally effective to, or better than, previous therapy. The first ten patients to complete six months on naproxen took part in a placebo pulse study comprising two consecutive four-week periods, in one of which each patient took 500 mg naproxen, in the other, identical placebo capsules, the order being randomized. Eight patients correctly identified the placebo capsules (P=0.02). During the 16 months of trial, six patients have withdrawn, two being in remission and four for lack of efficacy. The remaining 30 patients have completed six months and 22 have completed at least 12 months on naproxen. At the end of six months
pain
was less (P=0.02), morning stiffness had decreased (P less than 0.01), and immobility stiffness had improved (P less than 0.01). These patients are impressed by the improvement in
pain
and stiffness and have little disability. All continue full-time employment and have been able to increase their leisure activities without discomfort. No persistent side effects were observed, and naproxen appears to be a useful drug in the treatment of anklyosing spondylitis.
