A defined general population of 159,200 male and female native Swedes born in the period of 1911-1940, from an urban catchment area of the then only general hospital, was followed over a decade (1970-1979) with regard to inpatient hospitalization for all kinds of diagnoses. The erythrocyte sedimentation rate (ESR) was examined annually for 15 years in 1,000 asymptomatic men aged 18-33 years at entry into the study in 1968. The upper limit of the normal (mean + 2 SD) increased from 8 mm in the first hour in persons aged 18 years to 18 mm in those age 45. An increase in the ESR above the age adjusted upper limit of normal on at least three of four consecutive annual examinations was observed in 44 (4.4%) cases. Although occipitocervical fusion is frequently used for instability of the upper cervical spine and the occipitocervical articulation, most currently used techniques have one or more of the following disadvantages: the necessity for sublaminar wires, the use of occipital screws, a fixed angle of instrumentation, or the necessity for routine postoperative halo immobilization. Moreover, many reported techniques are associated with a high rate of nonunion or instrumentation failure. We present our experience with a technically simple method of obtaining rigid occipitocervical arthrodesis using a 5-mm malleable rod that is fixed to the skull by a pair of wires passed through four suboccipital burr holes. Segmental spinal fixation is achieved with Wisconsin interspinous wires and is occasionally supplemented with sublaminar wires. Supplemental autogenous bone graft is used in all cases. A cervical collar is routinely used for postoperative immobilization. The results of treatment were retrospectively reviewed in 16 patients with an average age of 49.4 years (range, 9-69). Mean follow-up was 24 months (range, 12-36 mo). This meta-analysis included (1) 7,462 patients exposed to celecoxib 200 to 800 mg/day for 1,268 patient-years compared with 4,057 patients treated with placebo for 585 patient-years, and (2) 19,773 patients treated with celecoxib 200 to 800 mg/day for 5,651 patient-years compared with 13,990 patients treated with nonselective NSAIDs (diclofenac, ibuprofen, naproxen, ketoprofen, and loxoprofen) for 4,386 patient-years. CV events were adjudicated by a 3-member expert end point committee (WBW, JSB, PBG) blinded to treatment group and study. The incidence rates of the combined CV events were not significantly different between patients treated with celecoxib and placebo or between those treated with celecoxib and nonselective NSAIDs. Event rates were similar for adjudicated and nonadjudicated data. Dose of celecoxib, the use of aspirin, or the presence of CV risk factors did not alter these results. In conclusion, these analyses failed to demonstrate an increased CV risk with celecoxib relative to placebo and demonstrated a comparable rate of CV events with celecoxib treatment compared with nonselective NSAIDs.

Between 1945 and 1955, several thousand patients were injected with Peteosthor, a preparation containing Radium-224 (Ra), as treatment for bone tuberculosis or
ankylosing spondylitis
. Ra, like Pu, is a bone seeking nuclide. During the course of early experimental work it became clear in 1948 that the short lived alpha-emitter Ra concentrates predominantly in the growing zones of the bones. Consequently, I released strong official warnings, at the 1950 German Congress of Orthopedics, against Peteosthor, and especially against its administration to juvenile patients-still in their period of growth. Epidemiological investigations were then initiated on a study population that comprises 899 persons (including 217 children or juveniles) who received injections of Ra. The study has now been conducted for a follow-up period of over 60 y. The most striking detrimental health effect following Ra injections are a large number of malignant bone tumors that occurred predominantly in childhood. This finding was the reason for my invitation to the first conference on "Delayed Effects of Bone seeking Radionuclides" in Sun Valley, ID, in September 1967, a meeting that was organized by Charles Mays. I reported on 50 Ra-induced bone tumors in children and adults, growth disturbances, osteochondroma, and cataracts, concluding that the younger the age at Ra injection, the more severe the late effects. Up to now 57 malignant bone tumors have been observed while less than one case would have been expected. The peak occurred 8 y after the first Ra injection and the last bone sarcoma arose 46 y after injection. A total of 270 non-skeletal malignant diseases were observed against a statistical expectation of 192 cases, the excess risk of mammary cancers in those treated in childhood being particularly striking. In the past two years increases of non-cancer diseases have become apparent in the exposed group compared to a control group of 166 living members with no exposure to Ra. Although 124 study group members are still alive, only the 81 members with ages at or below the maximum age in the control group were included in this comparison in order to attain approximate age matching. The breakdown of these diseases is kidney insufficiency, 12 (15%) study group members vs. 3 (2%) controls, where 5 (6%) study group members required dialysis vs. 2 (1%) controls; thyroid disease (struma nodosa), 28 (35%) study group members vs. 29 (17%) controls;

Drug rash, eosinophilia, and systemic symptoms (DRESS) syndrome represents one pattern of the cutaneous involvement in type IV hypersensitivity reaction to drugs.
