Aortic insufficiency and
aortitis
are frequent complications of
ankylosing spondylitis
but are considered rare in rheumatoid arthritis. A 49-year-old woman with severe rheumatoid arthritis had the cardiovascular changes common to both diseases. At autopsy, the heart and aorta showed granulomatous ane fibrinous pericarditis and epicarditis, "core" granulomas in the aortic valve cusps and mitral valve leaflets, and coronary arteritis. These are considered to be classical changes of rheumatoid carditis. In addition, there were mesoaortitis with obliterative endarteritis of the vasa vasorum and fibrosis of the aortic cusps with separation of the commissures. These are considered to be changes of
ankylosing spondylitis
. This case, therefore, represents a mixed form of cardiovascular involvement within the spectrum of the rheumatoid diseases.

Ten cases of post-inflammatory aneurysm of the thoracic aorta were studied morphologically using aortic wall and aortic valve tissue resected during the surgical repair of the lesions. Four cases of aortic aneurysm in Behcet's disease showed massive perivascular infiltration of the lymphocytes and plasma cells in adventitia and vasa vasorum. Fibrous obliteration of the vasa vasorum was seen, and neutrophilic infiltration was also associated in two cases with recurrent prosthetic valve failure. Three cases of aortic aneurysm associated with Takayasu's arteritis showed similar histologic features, but giant cells and histiocytic reactions were additional findings. Plasma cell infiltration was more pronounced compared to the Behcet's
aortitis
. One case of luetic
aortitis
showed a non-specific chronic inflammatory reaction with florid endothelial cell proliferation. One case of
ankylosing spondylitis
showed fibrous thickening at the periannular portion of the aortic valve cusps, but this finding was also seen in an aortic valve of a patient with Behcet's disease. One case without any disease association showed similar features of chronic inflammation with granuloma formation. The findings suggest that they have similar histologic features indistinguishable in terms of the clinical disease association, but a granulomatous reaction could be expected in Takayasu's arteritis.

Out of 887 consecutive patients who underwent aortic valve replacement between January 1976 and December 1981 at Hannover Medical School Hospital, 3 patients had severe aortic valve insufficiency associated with
ankylosing spondylitis
(Morbus Bechterew). One of them had huge aneurysmatic dilatation of the ascending aorta and successfully underwent replacement of the ascending aorta by a vascular prosthesis. Microscopical examination of the resected aortic wall showed characteristic findings of
aortitis
in
ankylosing spondylitis
. The 3 patients are in good clinical condition at 5 and 6 months, and 2 1/2 years, respectively, after uneventful surgery. It is concluded that aortic valve replacement in patients with
ankylosing spondylitis
can be performed feasibly and clinical results have been satisfactory. The risk of aneurysmatic dilatation of the ascending aorta resulting from

associated with

is emphasized.

has rarely been reported. This article discusses a case of juvenile onset spondylitis in which associated aortic, and possibly mitral, valve disease dominated the clinical course for many years before axial joint disease required total replacement of the right hip.

There is a definite association of heart disease with
ankylosing spondylitis
(AS). The magnitude of this relationship is less clear. Three types of inflammatory affections can be differentiated: 1.
aortitis
and aortic insufficiency with the possible necessity of cardiac surgery, 2. conduction disturbances of the atrioventricular node with a probable subsequent indication for a pacemaker and 3. myocardial involvement with a possible compromise of left ventricular function. There seems to be an HLA B27-associated cardiac syndrome consisting of aortic root disease and conduction abnormalities. Patients can be recognized at pre-clinical stages. Prospective studies have not been performed. HLA B27-related heart disease does not seem to be associated with increased mortality.

is described. An abdominal aortic aneurysm, 48-mm in diameter, in a 68-year-old woman with HLA-B27-associated
ankylosing spondylitis
was successfully replaced with a tube graft. The suture lines of the aortic wall were reinforced with Teflon felt strips. Pathologic examination of the aneurysmal wall revealed hyalinization of the connective tissue, with numerous lymphocytic infiltrates, remarkable calcification, and no elastic fibers. The original structure of the arterial wall was not recognized. These findings are compatible with
aortitis
reported in
ankylosing spondylitis

We describe a patient who had aortic regurgitation associated with Crohn's disease in the absence of
ankylosing spondylitis
.
Aortitis
and aortic insufficiency are fairly uncommon in Crohn's disease. The patient required aortic valve replacement because of severely uncoated cusps secondary to inflammation of the aortic wall and aortic valve. There was a saccular formation just above the right non-coronary commissure. This sac was closed with a pericardial patch. Pledgeted sutures were used for implantation of the prosthetic valve to avoid periprosthetic leakage. The right coronary ostium had narrowed due to aortic wall thickening. A right internal thoracic artery to right coronary artery bypass was done since there was no necessity for proximal anastomosis.

Patients with
ankylosing spondylitis
(AS) may develop cardiovascular manifestations ranging from asymptomatic forms to life threatening conditions. The most important cardiovascular manifestation of AS is
aortitis
, which frequently involves the aortic root and the ascending aorta leading to valvular insufficiency. The extension of the subaortic fibrotic process into the interventricular septum may cause conduction abnormalities that represent the second common cardiovascular manifestations occurring in AS patients. More rarely, an involvement of coronary arteries and of thoracic and abdominal aorta could be present. Rheumatologists managing AS patients should carefully consider, both in late and in early phases of the disease, the occurrence of an aortic involvement in order to promptly administer adequate treatment.
