A defined general population of 159,200 male and female native Swedes born in the period of 1911-1940, from an urban catchment area of the then only general hospital, was followed over a decade (1970-1979) with regard to inpatient hospitalization for all kinds of diagnoses. Psoriasis cases (n = 372) are significantly (p less than 0.001) associated with a spectrum of diseases: male as well as female psoriatics seem to show excess rates of viral infections, alcoholism, hypertension, pneumonia, liver cirrhosis, urticaria, and rheumatoid arthritis. Psoriasis in males only seem to be associated with iritis and ankylosing spondylitis, whereas psoriasis in females only is associated with lung cancer, diabetes, obesity, myocardial infarction and asthma.

A patient who developed an epidural haematoma with multifactorial aetiology (bleeding diathesis, ankylosing spondylitis, chronic alcoholism and acute pancreatitis) after epidural analgesia for pain relief is described. Our conclusion is that adequate laboratory screening of blood coagulation, including platelet count, should be carried out in this category of patient before attempted epidural blockade, the risks of which must be weighed against the benefits. The block should be allowed to wear off intermittently and repeated neurological assessment performed if an epidural catheter is used for repeated injections or for a continuous infusion of local anaesthetic. Neuroradiological examination should be carried out promptly if an epidural haematoma is suspected and surgical decompression performed without delay if the diagnosis is confirmed.

Herman Melville developed debilitating physical and psychiatric disorders in middle age after writing, perhaps, the greatest of American novels, Moby Dick. This article critically examines claims that Melville had bipolar affective disorder and alcoholism, and suggests he may also have suffered from post-traumatic stress disorder. Melville was active and vigorous in youth but in middle age he developed recurrent attacks of eye pain, photophobia and disabling low back pain. Melville's contemporaries usually attributed his physical problems to 'neurasthenia' and his biographers have often dismissed them as psychosomatic. However, Melville's clinical course, abnormally rigid posture, loss of 1(3/8) inches in height between the ages of 30 and 37, and a family history of rheumatological disease, suggest a diagnosis of ankylosing spondylitis.
