A 43-year-old woman with isolated ACTH deficiency in association with transient thyrotoxicosis is reported. The initial evaluation revealed that plasma ACTH and cortisol did not respond to corticotropin-releasing hormone (CRH) in the presence of hyperthyroxinemia and hyperprolactinemia. During the replacement therapy with dexamethasone, she developed transient hypothyroxinemia with persistent hyperprolactinemia. Although thyroid open biopsy did not show any evidence of autoimmune thyroiditis or subacute thyroiditis, the data appear to provide other evidence of a possible relationship between acute adrenal insufficiency and transient thyroid dysfunction.

Complex interactions between the neuroendocrine and the immune systems are present in autoimmune diseases. The central opioid peptide beta-endorphin (BE) has been shown to modulate peripheral immune responses in normal animals. In the present study we analyze the hypothalamic concentrations of this peptide in two models of spontaneous autoimmune disease, the MRL [corrected] lpr/lpr mouse, that develops a lupus-like autoimmune disease, and the obese strain (OS) chickens afflicted with spontaneous autoimmune thyroiditis. In both instances, hypothalamic concentrations of BE are significantly lower than normal controls. In MRL [corrected] lpr/lpr mice, BE is already lower at 1 month of age, when no clinical sign of the disease is yet present. Similarly, low levels of BE are observed in OS chickens before the onset of thyroiditis, i.e., already at the embryonic stage. Moreover, a further decrease of BE is observed in OS chickens in correspondence with the first signs of thyroid mononuclear infiltration. Considering the immunosuppressive effects exerted by central BE, these results are suggestive of the fact that in autoimmune disease prone animals the low hypothalamic concentrations may be one of several factors predisposing for the development of autoimmune disease.

Thyroid state study was been carried out for Chernobyl NPP accident liquidators living in Ozyorsk, the city, located in the nuclear facility affected zone (Mayak PA). In total 531 individuals were examined. Thyroid assessment was made basing on physical examination, on ultrasound study and on hormones measurement of thyroid stimulating hormone, free thyroxine and thyroid peroxidase antibody. Screening revealed thyroid pathology for 27% of clean-up workers. As a result of the study carried the following conclusions were made: thyroid disease prevalence among the Ozyorsk residents participated in the breakdown elimination after Chernobyl accident was not differ from the individuals who didn't take part in this work; the prevalence of thyroid disease in liquidators was significantly connected with non radiation risk factors: with gender (for women compared to men), RR = 4.0, 95% CI 2.9-5.5; with age (over 60 years old vs under 60 years old), RR for women was 1.7; 95% CI 1.0-3.0 and for men -1.9; 95% CI 1.1-3.2; with hereditary predisposition, RR for autoimmune thyroiditis was 3.3, 95% CI 1.4-7.9.
