We investigated whether natural killer (NK) cells in the tumor microenvironment have a radiosensitization effect. The radiosensitization effect of combined CpG and Herceptin((R)) (Genentech, Inc., South San Francisco, CA) (CpG/Herceptin), given before or after radiation, was evaluated by using a murine colon cancer cell line overexpressing human HER2/neu, CT26HER2/neu. In vitro radiosensitization effects were investigated by coculture of CT26HER2/neu with splenocytes, CpG, and Herceptin before applying radiation. Tumor cells, cocultured with CpG-pretreated splenocytes and Herceptin, were more vulnerable to radiation damage. In BALB/c mice injected with CT26HER2/neu, CpG/Herceptin administered before radiotherapy was associated with a better retardation of tumor growth than when administered after radiotherapy. The radiosensitization effect was significantly abrogated by NK-cell depletion, indicating that NK cells play an essential role in it. Further, surviving mice treated with CpG or CpG/Herceptin and reverse transcriptase were resistant to renewed tumor challenge, suggesting the presence of an induced immune response to the tumor. Neoadjuvant immunotherapy with CpG/Herceptin may improve response to radiotherapy of HER2/neu-expressing tumors.

At the ASCO Breast Cancer Symposium in San Francisco last year, the keynote speaker, Dr George Sledge, chief of oncology at Stanford University School of Medicine and a preeminent breast cancer researcher, gave his 5 predictions for the state of breast cancer care in 10 years. One of his more startling predictions was that in 10 years, he believed that the era of HER2/neu would be over. He predicted that 3-year disease-free survival for patients with HER2/neu-positive disease would more than 92%, approximating outcomes in other good prognosis malignancies such as testicular cancer. In view of that expectation, he believed that the main research questions regarding HER2- positive disease would eventually involve decreasing toxicity and costs rather than the improving efficacy of HER2-targeted therapy. Essentially, HER2/neu-positive disease would cease to be a public health issue. In view of this remarkable but likely prescient statement, we note the recent publication of 2 trials (NeoSphere and TRPHAENA) that examined the efficacy and safety of the use of pertuzumab in combination with trastuzumab and chemotherapy in the neoadjuvant treatment of early-stage HER2-positive breast cancer.

Risk factors for the onset of cognitive impairment in Costa Rica are not well understood, despite a substantial elderly population stemming from a higher than average life expectancy for the western hemisphere. To investigate the risk factors that predict the onset of cognitive impairment in the rural elderly of Costa Rica, a modified version of the Mini Mental State Exam-designed for illiterate populations-was administered to 90 elderly inhabitants of San Carlos, Alajuela, Costa Rica between April and May of 2011. Subsequently, each participant took a structured interview assessing viability of risk factors and behaviors potentially contributing to a diagnosis of cognitive impairment. Results showed strong dependencies between age (p = 0.0001), education level (p = 0.0095), the ability to read (p = 0.0001) and write (p = 0.0153), frequency of reading (p = 0.0011), use of puzzles and mind games (p < 0.0001), vocation (p = 0.0225), area of residence (p < 0.0001), comorbid mental dis- eases (p = 0.0005), history of stroke or brain trauma (p = 0.0104), urinary or renal problems (p = 0.0443), consistent cooking practices (p = 0.0262) and number of living companions (p = 0.0299) in susceptibility for developing cognitive impairment. The study concluded that high intellectual use, or lack thereof, during the lifetime of a person was a predictor for cognitive status later in life. In addition, comorbid mental disorders, including neu- rological trauma due to stroke, impeded normal cognitive function. Future research should examine incidence and risk factors of cognitive impairment in urban, more educated populations.

Patients with positive Her-2/neu breast cancer and a high risk of recurrence are known to benefit from the addition of the dual blockade of Her-2/neu with Trastuzumab and Pertuzumab to the neoadjuvant chemotherapy, a combination which has been demonstrated to give a higher rate of a complete pathologic response in the breast and in the axilla. The purpose of this review is to outline the efficacy of the dual blockade with Trastuzumab and Pertuzumab in the neoadjuvant treatment of high-risk Her-2 positive breast cancer. Electronic databases (Pubmed, Medline, and Cochrane Database of Systematic Reviews) were searched for English- and German-language studies, which were published in the last ten years. The search has been focused on neoadjuvant clinical trials as well as on the data presented in the abstracts published at the San Antonio Breast Cancer Symposium as well as at the annual meeting of the American Society of Clinical Oncology. The results reported in the published clinical trials demonstrated a higher pathologic complete response rate in breast and lymph nodes after using targeted therapy with two anti-Her-2/neu agents - Trastuzumab and Pertuzumab in combination with neoadjuvant chemotherapy for early-stage Her-2/neu positive breast cancers. The pathologic complete response rate is the most important prognostic marker in Her-2/neu positive tumors, a higher pathologic complete response rate being demonstrated to be associated with a better survival outcome in terms of higher overall survival and disease-free survival rates.
