Cefpodoxime proxetil is a new third generation oral cephalosporin, which shows potent antibacterial activity against both Gram-positive and Gram-negative bacteria, and high stability in the presence of beta-lactamases. Low concentrations of cefpodoxime inhibit most respiratory pathogens, including Haemophilus influenzae, Streptococcus pneumoniae, and Moraxella (Branhamella) catarrhalis.
Cefpodoxime
reaches concentrations of 0.24 +/- 0.06 mg/kg in tonsils, 0.89 +/- 0.80 mg/kg in lung parenchyma, and 0.91 +/- 0.01 mg/kg in bronchial mucosa; these values exceed by far the minimum inhibitory concentrations (MICs) of cefpodoxime for respiratory pathogens. Preliminary clinical studies were carried out in 181 patients with upper respiratory tract infections: the results indicated an overall clinical response in 88.4% of patients; in 30% the clinical efficacy was excellent and in 58.5% it was good. Further studies showed clinical cure in 90.3% of patients with pharyngotonsillitis, and clinical efficacy (cure plus improvement) in 95% of those with acute sinusitis. , streptococci, or M. catarrhalis.
Cefpodoxime
appears to be an effective new antibacterial that can be recommended as a drug of first choice in the treatment of most upper respiratory tract infections.

Cefpodoxime
, cefixime, and ceftriaxone inhibited Branhamella catarrhalis at less than or equal to 1 microgram/ml, beta-hemolytic streptococci at less than or equal to 0.25 microgram/ml, Neisseria meningitidis at less than or equal to 0.06 microgram/ml, and Haemophilus influenzae (other than beta-lactamase-negative, ampicillin-resistant isolates) at less than or equal to 0.12 microgram/ml. The MICs for 50% of isolates of the family Enterobacteriaceae other than Citrobacter freundii, Enterobacter aerogenes, and Enterobacter cloacae were less than or equal to 1 microgram/ml for all three cephalosporins. The MICs of each cephalosporin for 90% of staphylococci, enterococci, and Pseudomonas aeruginosa isolates were greater than 16 micrograms/ml. H. influenzae
, Moraxella catarrhalis, Streptococcus pyogenes and Streptococcus pneumoniae was investigated and compared with that of another 8 commonly used oral antibiotics. beta-Lactamase negative, beta-lactamase positive and multiresistant strains of
H. influenzae
were inhibited by cefpodoxime at a concentration of 0.13 mg/l. 10% of Moraxella catarrhalis isolates were moderately susceptible to cefpodoxime, with minimum inhibitory concentration (MIC) of cefpodoxime ranging between 0.13 and 2.0 mg/l. All isolates of Streptococcus pyogenes and Streptococcus pneumoniae were susceptible to < or = 0.25 mg/l cefpodoxime.
Cefpodoxime
was clearly more active than the older oral cephalosporins against all species tested. The activity was comparable to that of cefixime against all species except Streptococcus pneumoniae, against which cefpodoxime was more active than cefixime.

This study explores the influence on the intrinsic activity of different oral beta-lactams of beta-lactamase production in Haemophilus influenzae and penicillin resistance in Streptococcus pneumoniae. Three substudies were performed: a) a general susceptibility study, analyzing 550 strains received by the Spanish Laboratorio de Referencia de Neumococos throughout February and March 2005; b) a study on the influence of penicillin resistance on the activity of beta-lactams, analyzing 251 penicillin-susceptible strains (MIC<or=0.06 mg/l), 165 penicillin intermediate-resistant strains (MIC 0.12-1 mg/l) and 139 penicillin-resistant strains (MIC>or=2 mg/l) randomly chosen among those received by the Spanish Laboratorio de Referencia de Neumococos throughout 2005; and c) an
H. influenzae
susceptibility study analyzing 150 strains received by Instituto Valenciano de Microbiologia throughout 2005. A total of 71% of S. pneumoniae strains were susceptible to penicillin, 21% exhibited intermediate resistance and 8% strains presented full resistance.
H. influenzae
beta-lactamase production rate was 18.6%. Of the non-beta-lactamase-producing strains, 3% were not susceptible to ampicillin.
Cefpodoxime
and cefixime exhibited the highest intrinsic activity against
H. influenzae
, while amoxicillin and cefpodoxime were the most active compounds against S. pneumoniae. All
H. influenzae
strains were susceptible to oral cephalosporins and amoxicillin/clavulanic acid. The increase in penicillin resistance in S. pneumoniae influenced cefixime, cefaclor and cefuroxime to a higher degree than amoxicillin and cefpodoxime.
