The aim of the work was to investigate functional changes of tubular cells after i.v. urography. As evidence the authors used assessment of urinary levels of membrane-bound enzymes--alkaline phosphatase (AP), gamma-glutamyl-transpeptidase, lysosomally bound enzymes
N-acetyl-beta-D-glucosaminidase
and its isoenzyme B and the low molecular protein, beta-2-microglobulin. The above substance were assessed in 15 patients with different nephropathies where i.v. urography was indicated. The examinations were made in 24-hour urine samples before i.v. urography and in two 24-hour samples after administration of the contrast substance. In all patients a significant rise of tubular enzyme excretion was observated as well as of beta-2-microglobulin. The greatest rise was recorded in alkaline phosphatase in the second sample after administration of the contrast substance (432% of the initial value).
Beta-2-microglobulin
and N-acetyl-D-glucoseaminidase rose already during the first collection period (B2M to 357% and NAG to 181% of the initial values). The authors conclude that i.v. urography made by hyperosmolar iodinated preparations (Verografin, Iodamide) significantly affects the function and integrity of the proximal tubule. The applied spectrum of examinations is suitable also for further investigations of the effect of contrast substances on cells of the proximal renal tubule.

Cadmium and lead have been identified as very toxic metals, which are widely present in the environment due to natural and anthropogenic emissions. Many studies have shown that the food chain is the main pathway of cadmium and lead transfer from the environment to humans. It is well documented that many factors will affect their transfer through food chains. Previous investigations on heavy metals were mostly concentrated on one contaminant in isolation. However, in real environments, exposure to mixtures of metals is ubiquitous such that cadmium pollution is invariably being associated with lead and zinc, etc. This study focuses on the contamination and health effects of the metal mixtures. For this purpose, a dietary survey was taken for 3 groups in Nanning in October 2002. Samples of soils, plants (vegetables), urine and blood of humans were measured for Cd, Fe, Cu, Zn, Ca and Pb, in addition, the urinary indicators of renal dysfunction Albumin (ALB),

Beta-2-microglobulin
(beta2-MG) and Retinol-binding protein (RBP) in urine were also measured. Results showed that soil contamination with metal mixtures had caused significant renal dysfunction of the local residents living in the contaminated area, and the dose-response curve was somewhat altered by the mixed contamination of Cd and Pb as well as the intake of other minerals. The importance of mixtures of metal contamination and human health are also discussed in this paper.

Balkan nephropathy (BN) has not been described in children; however, some previous studies in children from families with BN have revealed abnormalities of the urinary tract. In this study, urinary excretion of beta2-microglobulin,
N-acetyl-beta-D-glucosaminidase
(
NAG
) and gamma-glutamyl transpeptidase (GGT) was studied three times a year: spring, autumn, and winter, during a 3-year period, in 703 healthy children, initial age 9-13, from endemic and nonendemic settlements around the South Morava River.
Beta-2-microglobulin
excretion in urine, in all three seasons, was highest in children from families with BN compared with the excretion in children from the city, nonendemic villages, and those from nonendemic families. Increased urinary GGT excretion in children from endemic villages in October was higher than in children from the city and control villages, being the same in both endemic and nonendemic families. However, in February, it was similar in children from the city, endemic, and control villages. In conclusion, children from families with BN excreted significantly more beta2-microglobulin in all three seasons (spring, autumn, winter) of the study, in multivariate analysis significant for family status, gender, and the season (p < 0.001).
NAG
emerged as a potentially useful marker for seasonal exposure to an environmental nephrotoxin.
