An analysis was made of 17 cases of microscopically proven rib neoplasms, with the objective of evaluating the variability of clinical and radiographic findings, the neoplastic cell types, the occurrence of metastases, and the survival time following the diagnosis. In 15 dogs with primary rib neoplasms and in 2 dogs with secondary (presumably metastatic) rib neoplasms, the clinical signs commonly included dyspnea and a localized swelling on the thoracic wall. For the dogs with primary rib neoplasms, the mean age was 4.5 years, with a range of 1.3 to 16 years. On radiography, rib osteolysis, extra and/or intrathoracic soft tissue masses often containing areas of mineralization, and pleural effusion were commonly identified. Ribs 5 through 9 were the most commonly affected and the right side was involved twice as often as the left side. Osteosarcoma was the most frequently diagnosed tumor, but chondrosarcomas and hemangiosarcoma were also found. At necropsy, over 50% of the dogs with primary rib neoplasms had metastases in the lungs. Less than 10% of the dogs with primary rib osteosarcomas survived more than 4 months after the diagnosis.

A case is reported of osteosarcoma of the 11th rib in a 38-year-old woman who presented with dry cough and dyspnea due to bloody pleural effusion and the literature on osteosarcoma of the rib in Japan is reviewed. Osteosarcoma was suggested by roentgenologic findings and examination of a percutaneous needle biopsy specimen. Therefore, we administered chemotherapy, but the tumor showed progressive expansion and invasion, causing paraplegia. The patient died on the 282nd day of admission. We performed an autopsy and diagnosed osteosarcoma histologically. The tumor was covered by a hypervascular capsule, suggesting its association with the bloody pleural effusion. Osteosarcoma of the rib has not been previously reported in association with bloody pleural effusion. Considering the prognosis and characteristics of such tumors, we recommend prompt diagnosis in cases presenting with bloody pleural effusion.

Osteosarcoma is a malignancy whose various sites of metastasis greatly modify its ultimate prognosis. We report a case of simultaneous pulmonary and cardiac metastases in a 41-year-old male patient with osteosarcoma of the tibia, presenting after more then one year of completion of adjuvant therapy with progressive dyspnea and cyanosis. Diagnosis was made on computerized tomogram and echocardiogram. The metastatic mass entirely occupying the right ventricle and the pulmonary artery proved fatal.

A 49-year-old man was admitted to our hospital for an expanding tumor in the pulmonary artery. He had visited a previous hospital complaining of dyspnea on effort and had syncope 4 months before admission, and pulmonary embolism was diagnosed because enhanced chest CT showed filling defects, with calcification in the pulmonary trunk and left pulmonary artery. Despite thrombolytic and anticoagulant therapy, the filling defects grew and expanded into the extravascular portion. Additionally, CT showed multiple pulmonary nodules and a small calcified nodule in a right-sided back muscle also appeared. At our hospital, FDG-PET showed abnormal uptake in each lesion shown on CT. We then performed a CT-guided needle biopsy of the nodule of the back muscle, which was pathologically diagnosed as osteosarcoma. He was finally given a diagnosis of osteosarcoma of the pulmonary artery. We administered cisplatin and doxorubicin with partial inhibitory effect on tumor growth. Osteosarcoma of the pulmonary artery is extremely rare, and its diagnosis is difficult before surgery or autopsy. To the best of our knowledge there have been no reports of chemotherapy for osteosarcoma of the pulmonary artery.

In children and adolescents with chest pain and dyspnea, pneumonia, pleural effusion, and empyema are the frequent causes in the differential diagnosis. Malignant tumors of the chest wall are rare and most originate from the ribs. In children, the most frequent malignant tumor of the rib is Ewing's sarcoma. Osteosarcomas of the rib are very rare. Osteosarcoma has a predilection for rapidly growing long bones including the femur, tibia and humerus in adolescents. In this paper, we present an adolescent girl who presented with chest pain and dyspnea with osteosarcoma that originated from the rib and extended to the right hemithorax.
