Recent cohort and case control studies of low-dose combined oral contraceptives (COCs) containing the new generation of progestogens have allowed classification of adverse effects into those which are rare but serious and should be considered risks and those which are more frequent but are less of a threat to health. Low-dose COCs continue to affect coagulation in a complex way, but the risk is less than with the older preparations, and it can be minimized by screening women for a personal or familial history of early or unusual thrombosis and for levels of protein C, S, and antithrombin III. migraine
with focal signs should also avoid using COCs. The relative risk of myocardial infarction (MI) may increase from 4:1 in women with one risk factor (age, smoking, hypertension, hyperlipidemia, and diabetes) to 20:1 with two risk factors and 128:1 with three or more risk factors. In the absence of all risk factors, a recent study indicated that the relative risk of MI with COC use was 1.9 for current and past use. COC use also causes a slight increase in hypertension in most women, especially those who are older or have a family history of hypertension. While the COC can affect carbohydrate and lipid metabolism, the new generation of progestogens has reduced these effects. The COC may accelerate presentation of gallbladder disease in predisposed women. The COC protects against benign breast disease but may increase the risk of breast cancer and cervical cancer slightly. There is a strong link between hepatocellular adenoma and COC use, but the incidence is low. Return to fertility after use has not been a problem. Both estrogenic adverse effects (nausea, dizziness, irritability, weight gain,
bloating
) and progestogenic adverse effects (vaginal dryness, acne, hirsutism, weight gain, depression, loss of libido) can occur in 50% of women, but these generally disappear after a few months of use. In conclusion, the low-dose, third generation COCs are associated with minimal risks in the absence of other risk factors and have many beneficial effects such as the prevention of ovarian and endometrial cancer; a decrease in pelvic inflammatory disease and ectopic pregnancies; and protection from anemia, primary dysmenorrhea, functional ovarian cysts, and benign breast disease as well as from the morbidity and mortality associated with pregnancy.

Beside well-established clinical benefits, the current doses of oestrogens may induce clinical side-effects leading to non-compliance and loss of efficacy. During a normal menstrual cycle the incidence of any cyclic discomfort is consistently reported to be lowest during the mild-follicular phase when plasma E2 remains between 60 and 150 pg/ml. The incidence of pregnancy-like symptoms such as
bloating
, breast tenderness and mood swings tends to increase in mid-luteal phase when E2 increases upto 150 pg/ml. On the other hand incidence of asthenia, sleep disturbances, depressive mood, headaches and
migraines
increase during perimenstrual days when E2 drops to 40 pg/ml or below. Accordingly experimental and human studies in castrated animals and postmenopausal women suggest that plasma E2 around 100 pg/ml is optimal for treatment of hot flushes, prevention of bone loss and cardiovascular protection. Due to large interindividual variation in estrogen clearance rate, it is unlikely that any standardized unique dose of oral or non-oral formulations will reproduce the optimal levels in all postmenopausal users. Efforts for individual titration are mandatory to improve compliance and actual efficacy on a long term. Because older postmenopausal women tend to have a better clinical tolerance to low E2 levels, objective markers of efficacy should also be identified when the aim of HRT is the prevention of osteoporosis or vascular diseases. In addition clinical and metabolic side-effects related to added progestins can be substantially reduced by the use of lower dose inducing amenorrhea and by progesterone instead of synthetic steroids.

The authors present the results obtained in Switzerland, as part of an international survey (DIGEST), on 3 months' prevalence of upper digestive symptoms (UDS) and their influence on quality of life and consumption of medical services. 514 randomized adults from the general population in 8 different cities were interviewed. In these interviews data were recorded concerning demographic and socio economic aspects, quality of life, severity and frequency of UDS, consultations and medication. The sub-population with relevant UDS (i.e. UDS at least once a week and/or of moderate to severe degree) was compared with the rest of the population interviewed. 19% of the interviewees reported relevant UDS; of these, two thirds were women. No differences were found between people with and without UDS as far as education, professional activities, consumption of alcohol or smoking are concerned. The most frequent symptoms reported were fullness,
bloating
and nausea. However, daily activities were most impaired by nausea, epigastric pain and heartburn. Interviewees with UDS more frequently reported "life events" in the preceding year (48% vs 33%). Interviewees with UDS also more frequently reported back pain (7% vs 2%) and
migraine
(10% vs 6%). Furthermore, more interviewees with UDS reported sick leave (11% vs 3%); they also had a poorer life quality score (74 vs 89, PGWBI), reported more medical visits (50% vs 19%) and consumed more medication, both prescribed (65% vs 25%) and non-prescribed (OTC: 70% vs 31%).

Clinically, it is known that: (1) magnesium (Mg) supplementation relieves premenstrual problems (e.g.,
migraine
,
bloating
and edema) occurring in the late luteal phase of the menstrual cycle; and (2)
migraine
syndromes, particularly in women, are associated with deficits in brain and serum ionized Mg levels. We investigated whether concentrations of sex steroid hormones, found in the serum during the menstrual cycle of women, are associated with changes in the levels of cytosolic free magnesium ions ([Mg2+]i in single cultured canine cerebral vascular smooth muscle cells. The resting level of [Mg2+]i in these cells was 645 +/- 89 microM before exposure to sex steroid hormones. Exposure of these vascular cells to a low concentration of estrogen (10 pg/ml) failed to interfere with the levels of [Mg2+]i. However, exposure to estrogen, at concentrations ranging from 40 to 200 pg/ml, induced significant loss of [Mg2+]i in a concentration-dependent manner. At a concentration of 200 pg/ml estrogen, the level of [Mg2+]i decreased approximately 30% in comparison with controls. Progesterone produced biphasic effects on the levels of [Mg2+]i, depending on its concentration. Exposure of the cultured cells to a low concentration of progesterone (0.5 ng/ml) resulted in an increased level of [Mg2+]i (from 690 +/- 50 microM to 753 +/- 56 microM, p < 0.05). However, when these cells were exposed to higher concentrations of progesterone (i.e., from 5.0 to 20 ng/ml), the cellular levels of [Mg2+]i were decreased significantly. The higher the estrogen or progesterone concentration, the lower the levels of [Mg2+]i. In contrast, testosterone, a male hormone, didn't produce any significant alteration in [Mg2+]i levels in these cerebral vascular smooth muscle cells. These data indicate that low, physiological concentrations of female sex hormones, estrogen and progesterone, help cerebral vascular smooth cells sustain normal concentrations of [Mg2+]i, which are beneficial to vascular function, whereas high levels of estrogen and progesterone deplete, significantly, [Mg2+]i in cerebral vascular smooth muscle cells, possibly resulting in cerebrovasospasms and reduced cerebral blood flows related to premenstrual syndromes,
migraine
and stroke risk. Our findings could provide new insight into the mechanism whereby
migraine
occurs frequently in the late luteal phase in the premenstrual syndrome. In addition, our results demonstrate that female sex steroids but not testosterone (in physiologic concentrations) can exert direct effects on [Mg2+]i in cerebral vascular cells.

Mental illness by which psychosis is meant here is known to be caused mainly by imbalances of certain neurotransmitters in the brain. But, what is causing these imbalances? There has been a recent flurry of interest focusing on the possibility of parasitical disease. The appropriateness of this is based on the fact that organisms of the animal kingdom produce the same neurotransmitters. In fact stinging insects release them in their venoms. The proposal here is that insect larval parasites acting on the human brain and body may release such neurotransmitters and cause imbalances and altered mental states and is supported by the occurrence of previously unexplained physical symptoms such as; diarrhoea, constipation, spasms, anaemia,

, weight loss, low blood pressure, low grade fever, amnesia and signs of allergy which may accompany mental illness. Some of these symptoms have been previously attributed to the medications prescribed to alleviate the psychotic symptoms but, many are also parasitical signs. It is proposed that the minute larvae may make sudden movements and may be highly motile and may move from pressure, hence evading the phlebotomist's needle. There is also the testimony of those with delusional parasitosis and related addictions, I propose the regularity with which humans are bitten, stung and have their foods infected with insects at all stages as a demonstration of how insectal disease may have the potential for common infection and disease; mental and physical.

This study explored the perspective of women using compounded progesterone preparations. Semi-structured interviews were conducted in Victoria, Australia with eight self-selected women who were dispensed a progesterone-only preparation. Participating women gained symptome relief for
migraine
, painful breasts, mood swings,
bloating
, hot flushes, and other conditions. The participants also experienced unexpected benefits such as improvement in irregular and painful periods, relief from cystitis, or increased libido, without any reported negative side effects. The participants appreciated the ability to adjust the dose and dosage form to their individual requirements and held a positive perception of progesterone as a "natural" product. Accessibility of progesterone treatment is limited due to the limited number of doctors that have knowledge about the treatment, little information available for lay people, and cost. Participants who had been treated with progesterone were willing to share their experience with others, and many heard about progesterone therapy from their social contacts. The cost of progesterone therapy, which was often more expensive than traditional hormone replacement therapy, was outweighed by the benefits received. These women found that progesterone treatment, although hard to access, was beneficial over conventional therapy.
