Calcemic alterations in prostate cancer are extremely rare. Hypercalcemia may be seen in cases of multiple osseous dissemination, and even in the absence of this in response to humoral mechanisms. The existence of hypercalcemia may indicate tumoural recurrence, and may at times be a datum prior to the diagnosis of the tumour. In cases of disseminated prostate adenocarcinoma hormones treatment may secure the normalization of blood calcium. We present a case of prostate carcinoma diagnosed in a 76-year-old patient, as a result of the presentation of a hypercalcemic metabolic syndrome, which was corrected after treatment by means of complete androgenic blocking.

The case of a 4 years old boy, hospitalized for an unexplained coma, is reported. He is the first child of a non-consanguin couple. The psychomotor development of this child was considered as normal up to the age of 18 months; then, a delay in language development, behaviour disorders with an important instability interrupted by episodes of somnolence, were observed. This child was treated for psychotic disorders. At the age of 3 and half, he had two episodes of seizures associated with fever. He was hospitalized for a 24 hours coma (4 years old). An hepatomegaly and a dry, brittle hair were then observed. Hyperammonemia was made obvious by a protein tolerance test. The diagnosis of argininosuccinate lyase (ASAL) deficiency was based on the increased levels of ASA in plasma and urine. The deficiency was proved by a fibroblast culture. With protein restriction, hepatomegaly disappeared, hair became normal, the behaviour disorders and the delay in language development was improved. However, some school difficulties persist. This case shows that an hereditary metabolic syndrome can be revealed by psychotic like symptoms in childhood.

Using the baseline data of the diabetes intervention study (DIS) from 1126 newly manifested type II-diabetics our analysis demonstrates higher mean-values of some components of the so-called metabolic syndrome in patients with ECG-abnormalities indicating coronary heart disease (CHD) in diagnosis of diabetes compared with subjects without ECG-findings. The impact of general risk factors for the prevalence of CHD in diagnosis and after a 5-year follow-up is obviously different in both sexes. In multivariate analysis only systolic blood pressure was persistently a significant predictor in both sex groups. With increasing age life-duration gets as time-related factor importance for the development of CHD. The mathematically demonstrated association of triglyceride levels to the presence of ECG-abnormalities agrees with the results of WHO multinational study of vascular disease in diabetes mellitus. In the interventions as well as in the control-groups diabetic subjects with CHD after 5 year follow-up showed in comparison to diabetics without CHD higher levels of investigated risk factors which develop their pathogenetic effect probably by their clustering impact, because the differences of their mean-values are only in some cases significant. The common lower level of the most risk factors at the intervention group compared with the conventionally treated group is the result of the intervention measures.

Yucheng (oil disease) is a clinical and metabolic syndrome reported in Taiwanese who consumed rice oil contaminated with large amounts of various polychlorinated biphenyls (PCBs) and polychlorinated dibenzofurans (PCDFs), including the 2,3,4,7,8- and 1,2,3,4,7,8-PCDF congeners which are similar in structure and toxicity to 2,3,7,8-tetrachlorodibenzo-p-dioxin. A well known characteristic of Yucheng is the marked decrease in birth weights, although the underlying mechanism of this effect is unclear. Placental epidermal growth factor (EGF) receptor binding and autophosphorylation studies were done using tissue samples taken from Yucheng and unexposed control patients. EGF-stimulated receptor autophosphorylation of the human placental EGF receptor in the Yucheng subjects was decreased more than 60% of control levels, 4-5 years after the exposure had occurred. The decrease in EGF receptor phosphorylation was significantly correlated with decrease in birth weights. Nonlinear regression analysis of the 125I-EGF receptor binding data revealed that there were two distinct EGF receptor binding isotherms representing the high affinity-low capacity (HALC) and the low affinity-high capacity (LAHC) binding sites. In contrast to the placental EGF-stimulated phosphorylation data described above, the binding kinetics of the EGF receptor were not significantly altered in the control [HALC site Kd = 0.10 +/- 0.02 (SE) nM, Bmax = 788 +/- 255 fmol/mg of protein; LAHC site Kd = 17.4 +/- 8.2 nM, Bmax = 62 +/- 32 pmol/mg) compared to the Yucheng subjects (HALC site Kd = 0.11 +/- 0.02 nM, Bmax = 784 +/- 305 fmol/mg; LAHC site Kd = 49.5 +/- 24.7 nM, Bmax = 147 +/- 80 pmol/mg). GC-MS analysis of placental specimens showed elevated levels of selected PCB and PCDF congeners in the Yucheng compared to control individuals. Total PCB levels were 0.5 +/- 0.2 ppb and 20.0 +/- 4.8 ppb for the control and Yucheng subjects, respectively. A significant dose-response relationship was observed between the placental EGF receptor phosphorylation levels and the PCB concentrations (total or concentrations of 2,2',4,4',5,5'-hexa- and 2,2'3,3'4,4',5-heptachlorobiphenyls). In contrast, no significant relationship was found between the EGF receptor phosphorylation activity and the 2,3,4,7,8- or 1,2,3,4,7,8-PCDF congeners, which were at nondetectable levels in the control and between 104 and 374 parts per trillion in the Yucheng subjects. In summary, our data reveal that decreased placental EGF receptor phosphorylation capacity is associated with decreased birth weight. Furthermore, PCB tissue concentrations might be a better predictor of effects than are PCDF concentrations.

Data from a health screening survey with over 18,000 adult participants were used to determine the relations between serum calcium concentration and the cardiovascular risk factors hypertension, hyperglycaemia, and hyperlipidaemia. Blood pressure and serum glucose and cholesterol concentrations were all positively related to each other independent of age, sex, kidney function, and obesity. Similar relations between the risk factors were found in subjects with hypertension or hyperglycaemia independent of the degree of overweight. These results suggested that there might be a metabolic syndrome of cardiovascular risk factors. Serum calcium concentration was positively related to systolic and diastolic blood pressures and serum glucose and cholesterol concentrations. Thus a common feature in the syndrome is an increased serum calcium concentration. The relations between serum calcium concentrations and the cardiovascular risk factors were not limited to the upper parts of the distribution, being seen over a wide range. Changes in calcium metabolism seem to be related to a metabolic syndrome of hypertension, impaired glucose tolerance, and hyperlipidaemia.

In order to prevent myonephropathic metabolic syndrome (MNMS) following acute arterial occlusion of the extremities, the efficacy of plasma filtration to eliminate myoglobin which causes acute renal failure and other metabolites was evaluated by experimental model. Twenty five mongrel dogs weighing 10 to 20 kg were used. Infra-renal aorta and unilateral femoral arteries were ligated and 2/3 of the femoral muscle was carved. After 24 hrs, these ligations were released and thrombectomy was carried out. The dogs were divided into two groups: Group 1 was consisted of the dogs without any treatment and group 2 dogs were treated by plasma filtration. The changes in serum myoglobin, potassium, lactic acid and etc. were compared between the two groups. The plasma filter was hollow-fiber type that was made of polypropylene fiber. The serum level of myoglobin, potassium, CPK, GOT and BUN tended to decrease remarkably after revascularization in group 2 in contrast with those in group 1. There were statistically significant differences between the corresponding data of the two groups. Although the lactic acid levels were lower in group 2 than group 1, those were not significant statistically. In conclusion, the application of plasma filter was effective for the treatment of hyperkalemia and removal of myoglobin and unknown moderate molecular weight pathogenic substances to prevent MNMS.

We have compared the capillary density and muscle fiber type of musculus vastus lateralis with in vivo insulin action determined by the euglycemic clamp (M value) in 23 Caucasians and 41 Pima Indian nondiabetic men. M value was significantly correlated with capillary density (r = 0.63; P less than or equal to 0.0001), percent type I fibers (r = 0.29; P less than 0.02), and percent type 2B fibers (r = -0.38; P less than 0.003). Fasting plasma glucose and insulin concentrations were significantly negatively correlated with capillary density (r = -0.46, P less than or equal to 0.0001; r = -0.47, P less than or equal to 0.0001, respectively). Waist circumference/thigh circumference ratio was correlated with percent type 1 fibers (r = -0.39; P less than 0.002). These results suggest that diffusion distance from capillary to muscle cells or some associated biochemical change, and fiber type, could play a role in determining in vivo insulin action. The association of muscle fiber type with body fat distribution may indicate that central obesity is only one aspect of a more generalized metabolic syndrome. The data may provide at least a partial explanation for the insulin resistance associated with obesity and for the altered kinetics of insulin action in the obese.

Coincident pathological parameters were selected from 24 laboratory-diagnostic parameters of a second stage of diagnostics after x-rays screening concerning heart and vessel diseases tested by a check representative constellation in the shape of relative frequency was determined. For the parameters cholesterol, uric acid and glucose, belonging to the metabolic syndrome, it was possible to demonstrate relations to the erythrocyte sedimentation rate. The transaminases ASAT and ALAT especially showed correlations of pathological values among one another. There were found out one-sided relations for instance concerning the proportion of transaminases, thymol turbidity test as well as creatinine to the erythrocyte sedimentation rate. The connections as has been proved appeared in female cardiac patients in a more distinct way. The results, which were interpreted in the context of a further mathematic-statistical analysis, allow the conclusions for an efficient indicational application of clinical chemical research methods in chronic heart and vessel diseases in practice.

Thirty-six cases of acute arterial thrombosis treated in our department were investigated. The lesion was limited to the legs. The basic disease were 33 cases of arteriosclerosis obliterans, 2 of thromboangiitis obliterans and one cases of popliteal sclerotic aneurysm. The limb salvaged rate was 65% and the mortality was 22%. According to the findings of the extremities, they were classified into 4 groups. As for the results, this classification was useful to determine the prognosis of the legs. According to the clinical experience of myonephropathic-metabolic syndrome in (MNMS) 7 cases, staged revascularization was considered as one of the preventive treatments to avoid MNMS. The thrombolytic continuous perfusion therapy was supportive measure for these cases. This procedure was useful and available for limb salvage.

Systemic corticosteroids play a predominant role in the treatment of urticarial vasculitis (U.V.). If steroids are contraindicated, there may be applied a variety of pharmacologically different compounds. A female patient suffering from U.V. and metabolic syndrome, who had not shown any positive response to indomethacin, H1 and H2 antagonists, or colchicine, was treated with dapsone. As a result, the clinical symptoms improved within a few days and her hypocomplementemia normalized. We discuss the possible mechanisms of action of sulphones in U.V.
