Psoriasis
is a general inflammatory status in which the skin is usually visibly involved. Recent papers show that several comorbidities, including psoriatic arthritis,
metabolic syndrome
, cancer, osteoporosis, cardiovascular disease, chronic inflammatory bowel disease etc., are significantly associated with
psoriasis
. Detecting association between
psoriasis
and comorbidities, properly evaluate confounders and make a correct distinction among absolute risk, relative risk and clinical relevance is mandatory, before embracing the fact the
psoriasis
is, in fact, a complex and common international disease.

Psoriasis
is an inflammatory, immune-mediated cutaneous disorder that has recently been recognized as systemic disease that is associated with multiple comorbidities such as depression, obesity, and the
metabolic syndrome
. The
metabolic syndrome
is the constellation of abdominal obesity, dyslipidemia, hypertension and insulin resistance, and presence of the
metabolic syndrome
significantly increases a patient's risk for cardiovascular disease, stroke and type 2 diabetes. Recent studies have found that
psoriasis
patients are at increased risk for
metabolic syndrome
as well as the individual components of
metabolic syndrome
, and the two diseases appear linked through a common mechanism of inflammation. Speculation exists as to whether this association is causative or whether it is the result of other habits seen in
psoriasis
patients, such as increased rates of smoking, alcohol consumption, and sedentary lifestyle, which add to the complexity of the association between
psoriasis
and the
metabolic syndrome
. However,
psoriasis
treatments have been shown to reduce the risk of developing
metabolic syndrome
components and comorbidities. Future studies are needed to better understand the nature of this relationship and the implications this could have for management and treatment of patients with
psoriasis
.

Recently, it has emerged a strong association between increased adiposity, obesity, and
psoriasis
. Body Mass Index (BMI) is a simple index of weight-for-height that is commonly used to classify underweight, overweight and obesity in adults.
Psoriasis
has also been associated with systemic obesity-related disorders including type 2 diabetes, hypertension, ischemic heart disease, and combined hyperlipidemia, as a part of
metabolic syndrome
. Not only the obesity may be associated with higher
psoriasis
incidence and activity, and prevalence of obesity-related syndromes, but it may also influence the therapeutic approach to disease and the clinical response to systemic treatment. Consequently, the approach of the experienced dermatologist will take into account all the aspects of the patient clinical conditions including the analysis of BMI for the choice of the best suitable therapy.

Psoriasis
is a chronic inflammatory disorder that affects approximately 2% of the general population. Numerous studies have evaluated the increased prevalence of comorbid diseases and risk factors in psoriatic patients, including obesity,
metabolic syndrome
, cardiovascular disease, psoriatic arthritis, autoimmune disease, psychiatric illness, liver disease, smoking, malignancy, chronic obstructive pulmonary disease, sleep apnea, and alcohol abuse. Insight into the overlapping pathogenesis of these comorbidities of
psoriasis
highlights the importance of immune-mediated mechanisms in these disease states.
Psoriasis
, with its comorbidities, must be approached in a multidisciplinary manner to effectively and comprehensively understand, manage, and treat those with this complex disorder.

Psoriasis
is now classified as an immune-mediated inflammatory disease (IMID) of the skin. It is being recognized that patients with various IMIDs, including
psoriasis
, are at higher risk of developing "systemic" co-morbidities, e.g., cardiovascular disease (CVD),
metabolic syndrome
, and overt diabetes. In non-psoriatic individuals, the pathophysiology of obesity, aberrant adipocyte metabolism, diabetes, and CVDs involves immune-mediated or inflammatory pathways. IMIDs may impact these co-morbid conditions through shared genetic risks, common environmental factors, or common inflammatory pathways that are co-expressed in IMIDs and target organs. Given that pathogenic immune pathways in
psoriasis
are now well worked out and a large number of inflammatory mediators have been identified in skin lesions, in this review we will consider possible mechanistic links between skin inflammation and increased risks of (1) obesity or metabolic alterations and (2) CVD. In particular, we will discuss how well-established risk factors for CVD can originate from inflammation in other tissues.

Psoriasis
and atherosclerosis share immunoinflammatory mechanisms and patients with
psoriasis
may carry an excess of cardiovascular risk factors (hypercholesterolemia, hypertension, obesity,
metabolic syndrome
, diabetes mellitus, smoking etc.) and increased risk of atherothrombotic disease. The current review summarises the available evidence in this area of research and calls for increased awareness of cardiovascular risk assessment and treatment in patients with
psoriasis
.

Several recent studies have found an increased prevalence of non-alcoholic fatty liver disease within
psoriasis
patients. The exact pathophysiological mechanisms behind these observations are unclear, but are likely related to the high prevalence of obesity and
metabolic syndrome
within this patient population. Chronic inflammation, mediated by either proinflammatory adipokines or skin-derived cytokines, may contribute to fatty liver disease development by increasing insulin resistance which in turn promotes hepatic lipid accumulation. These same adipokines in addition to hepatic cytokines may act on the skin to influence
psoriasis
disease severity.

Psoriasis
is considered to be a polygenetically influenced, immune-mediated, organ-specific disease of dysregulated inflammation that is triggered by environmental factors such as infections, medications, and physical and/or emotional stress. It is recognised as one of the most prevalent skin diseases, affecting 2% to 3% of Caucasian populations. Major advances in understanding of disease pathogenesis indicate that patients with
psoriasis
have an increased risk of comorbidities such as
metabolic syndrome
and cardiovascular disease. A wide range of systemic drugs have been developed in recent years for treatment of
psoriasis
and comorbidities. Low-dose methotrexate (MTX) is one of the classical agents and is still one of the most frequently used systemic treatments for
psoriasis
worldwide. Low-dose MTX is also effective in treatment of psoriatic arthritis. The mechanism of action is not fully understood, but MTX is suggested to act primarily as an anti-inflammatory and immunosuppressant drug. A favourable efficacy and safety profile has been established for MTX in a large number of clinical trials, as well as in common practice. This review summarises the nature of the disease and our present knowledge about MTX in the treatment of
psoriasis
, including combination therapies.

Psoriasis
is primarily a chronic inflammatory skin disease burdened by some comorbidities including psoriatic alopecia, arthropathies, Crohn's disease, the
metabolic syndrome
and some cardiovascular involvement. During the past years, several biologicals corresponding to monoclonal antibodies were offered to treat
psoriasis
refractory to other potent conventional treatments. We review the effects of biologicals, in particular adalimumab (Humira), on psoriatic comorbidities.

Evidence has been provided for acute and long-lasting effects of climatotherapeutic rehabilitation of atopic diseases (neurodermatitis, asthma bronchiale) in moderate altitudes. However, to sustain the climatotherapeutical evidence of
psoriasis
and non-allergic asthma treatment, controlled studies are required, especially for long-lasting benefits. For other indications of climatotherapy in moderate altitudes (heart and circulatory diseases, osteoporosis,
metabolic syndrome
) the results demonstrate an acute effect. At North Sea and Baltic Sea, the climatotherapeutic effects remain uncertain, especially for the well-known indications such as
psoriasis
, atopic dermatitis and bronchial asthma. There is evidence for the acute beneficial effects at Canary Islands and long-lasting remissions of Dead Sea climatotherapy and thalassotherapy.
