A 21-year-old man developed transient, recurring right arm and leg weakness 2 weeks after a motorcycle accident. He ultimately proved to have an intimal tear and thrombosis of the left internal carotid artery with transient cerebral ischemia due to distal emboli. Neurologic deficits eventually resolved after anticoagulation and balloon occlusion of the vessel. The differential diagnosis of the patient's presentation is discussed, and the topic of nonpenetrating trauma of the carotid artery is reviewed, with special attention to mechanism of injury and clinical presentation.

Cerebral ischemia was produced in the Mongolian gerbil by bilateral occlusion of the carotid arteries. Although the cerebral ischemia so produced was not total, a mortality rate of 100% was obtained if the occlusion was maintained for 60 min in gerbils weighing 45--55 gm. Few deaths were observed after 50 min of bilateral carotid arterial occlusion. Test drugs were administered, after the removal of the arterial clips, to groups of gerbils to determine the mortality rate associated with each drug. Isoproterenol 50 mg/kg, amphetamine 5.0 mg/kg, and methylprednisolone 35 mg/kg improved survival after cerebral ischemia. Atropine 1 mg/kg, thiosemicarbazide 4 mg/kg, aminooxyacetic acid 100 mg/kg, theophylline 100 mg/kg, and phenytoin 50 mg/kg were associated with a reduced survival after cerebral ischemia. The known tendency of the gerbil to exhibit spontaneous seizures and the frequency and severity of the observed post-ischemic seizures suggest that the lethality of prolonged cerebral ischemia may be, in part, related to seizures triggered by the cerebral ischemia.

A 21-year-old man presented with sudden weakness and dysesthesia of his right limbs. Computerized tomography (CT) scan showed a low density area in the posterolateral part of the left thalamus. Right vertebral angiography revealed a small aneurysm at the P2 segment of the posterior cerebral artery. Magnetic resonance imaging (MRI) demonstrated intra-aneurysmal clot and signal void in the residual lumen. There were no other lesion and no predisposing risk factors that produced cerebral ischemia. It was thought that the aneurysm was the source of emboli resulting in thalamic infarction. The patient underwent a left subtemporal craniotomy, and the aneurysm was clipped. Following surgery, there has been no recurrence of ischemic attacks. The diagnosis and the therapy were discussed, with reference to the literature.

In the present study, we investigated the neuroprotective effects of Kangen-karyu (KGK) in a repeated cerebral ischemia model (2 x 10 min, 1-h interval). A 21-day pre- and post-ischemic treatment with KGK (10 - 300 mg/kg) and aspirin (5 mg/kg) improved the spatial memory impairment and neuronal death in the hippocampal CA1 region induced by repeated cerebral ischemia. However, a 7-day post-ischemic treatment with KGK did not attenuate the spatial memory impairment and neuronal death in this model. To determine the mechanism of action of KGK, we investigated the effects of a 14-day pre-ischemic treatment with KGK on cerebral blood flow in the hippocampal area of the repeated cerebral ischemia model using laser Doppler flowmetry. The 14-day pre-ischemic treatment with KGK increased the cerebral blood flow during reperfusion. These results suggest that a 21-day pre- and post-ischemic treatment with KGK can protect against brain damage caused by cerebral ischemia by increasing the cerebral blood flow in the hippocampal area.
