Cases of human
infestation
by Fasciola hepatica are not uncommon in Spain and other European countries. We report our experience with 20 patients diagnosed from 1982 to 1991 and present a critical review of published cases from western countries. Because F. hepatica has a special tropism for the liver, abdominal pain, hepatomegaly, and constitutional symptoms are among the most common manifestations of acute-stage fascioliasis. However, in the chronic stage,
biliary colic
and cholangitis are the predominant manifestations. The clinical spectrum of fascioliasis is variable, and patients may present with extrahepatic abnormalities, such as pulmonary infiltrates, pleuropericarditis, meningitis, or lymphadenopathy. Therefore, a high index of suspicion is required to establish a correct diagnosis. Eosinophilia is the most frequent laboratory abnormality. The CT scan has become a useful technique in the diagnostic work-up. A definitive diagnosis may be established by the observation of parasite ova in the feces, but most cases may be diagnosed by serologic methods. Triclabendazole and bithionol are the most effective drugs against F. hepatica. The efficacy of praziquantel is controversial.

Human
infestation
with Fasciola hepatica is observed occasionally. Cholestasis due to parasitic obstruction of the common duct is an infrequent complication. A case of fascioliasis is described in a 56 year-old male with symptoms of
biliary colic
and biochemical cholestasis. The diagnosis was based on the indirect hemagglutination test and the evidence of Fasciola hepatica in the extrahepatic biliary tract shown by ultrasound and ERCP. The patient presented an acute pancreatic reaction during an attempt at therapeutic endoscopic sphincterotomy. Medical treatment was initiated then with Bithionol. Cholestasis as well as the parasitosis were eliminated. Recovery was demonstrated by the normality of the titres of hemagglutination and of the biliary tract by ERCP.

The most common infection of the hepatobiliary system and of the pancreas is the
infestation
with Ascaris lumbricoides. Pancreatobiliary ascariasis may present as recurrent
biliary colic
, acalculous cholecystitis, cholangitis, pancreatitis, or hepatic abscess. Although ultrasonography is a highly sensitive and specific method for the detection of the disease, endoscopy may have also therapeutical potential. The majority of these infections is registered in developing countries, but the number of reports from Europe and North America is increasing. So far there has not been any publication from Hungary. Both of the two reported patients were admitted the hospital with colic pain in the right hypochondrium. The laboratory parameters revealed cholestasis. The transabdominal ultrasonography was normal in one case, but suspected alien body in the choledochus in the other patient. Ascaris lumbricoides was identified with endoscopic procedure in the ductus choledochus in both cases. Endoscopic extraction of the worm resulted in cessation of the complaints in both patients, their cholestatic laboratory parameters became normal. Although the parasitic tests in the stool were negative in both patients and in their relatives, mebendazole therapy was administered.

Biliary ascariasis is a less frequent, but important complication of ascaris
infestation
, because it may cause
biliary colic
, pyogenic cholangitis, and septicemia. Early diagnosis and treatment is important to prevent these complications. We present here a five-year-old girl with biliary ascariasis, whose main complaint was abdominal pain. After giving piperazin salt, multiple ascaris worms were seen in the stool within 10 days. She is at the follow-up without any complication.

The human
infestation
caused by Fasciola hepatica is a rare zoonosis, with an incidence of about 10 cases/year in Italy. We report a case of cholecystitis and obstructive jaundice in a patient affected by fascioliasis in which the diagnosis was secondary to the extraction of viable flukes from the bile duct during ERCP. The endoscopic examination permits, in addition to a rapid, correct diagnosis, direct clearance of the bile ducts. Oral drug therapy, when carried out following the endoscopic treatment, is aimed at killing any flukes potentially evading mechanical clearance. The healing achieved is confirmed by normalisation of antibody levels 6-12 months after therapy. Cholecystectomy is indicated and appropriate for the frequent occurrence of
biliary colic
related to acute and chronic cholecystitis and cholelithiasis, induced by the presence of the flukes.
Infestation
by Fasciola hepatica has to be considered among the differential diagnoses of obstructive jaundice. ERCP plays a major diagnostic and therapeutic role, and cholecystectomy, considering the pathogenetic effects of flukes on the organ, is mandatory.
