The ocular and systemic findings in 112 Mayo Clinic patients with relapsing polychondritis were reviewed. The incidence of males and females was equal, with median age at diagnosis of 51 years and the median follow-up of 6 years. Most patients had several organ systems involved at the time of the diagnosis. Twenty-one patients had ocular symptoms at the onset, and 57 developed ocular symptoms during their course. Major ocular complications included proptosis, lid edema, episcleritis/scleritis, corneal infiltrates/thinning, iridocyclitis, retinopathy, and optic neuritis. The major system involvement included otorhinolaryngeal, respiratory, arthritic, renal, cardiovascular, dermatologic, and neurologic diseases. Generally, laboratory studies were not helpful in making the diagnosis but were valuable in monitoring the disease. Based on the experience in these cases, the indications for the various therapeutic modalities are offered.

An 11-year-old girl developed peripheral corneal thinning and superficial pannus formation with no other ocular or systemic signs. During the subsequent two years she developed inflammation of both ears, progressive collapse of the nasal cartilage, and polyarthritis. A clinical diagnosis of relapsing polychondritis was made. A spontaneous perforation of her right eye secondary to progression of peripheral ulcerative keratitis was successfully repaired. Treatment with dapsone, a nonsteroidal anti-inflammatory agent resulted in remission of her ocular and systemic disease.

An adult, Japanese domestic cat presented with bilateral swelling, distortion, and erythema of ears and deformation of the limbs. Biopsy of the pinnae confirmed auricular chondritis. These lesions and the cat's general condition subsequently deteriorated, and the cat died. At necropsy, chondral changes were present in the pinnae, costae, larynx, trachea, and limbs. Histopathologically, these chondral tissues showed marked deformation with lymphocytic inflammation. The limb joint inflammation was associated with chondral erosion, deformation of subchondral bones with pannus, and thinning of cancellous bones. These lesions were consistent with the diagnostic criteria for human relapsing polychondritis. However, the cat had erosive arthritis, which appeared to be different from human relapsing polychondritis.

Peripheral ulcerative keratitis (PUK) is a sight-threatening condition characterized by an epithelial defect, crescent-shaped stromal inflammation, and progressive stromal thinning. Peripheral ulcerative keratitis as a purely inflammatory entity is most commonly associated with collagen vascular diseases, including rheumatoid arthritis, polyarteritis nodosa, Wegener granulomatosis, systemic lupus erythematosus, and relapsing polychondritis. PUK can also be associated with infectious and inflammatory conditions such as hepatitis, syphilis, herpes simplex keratitis, fungal keratitis, Mooren ulcer, and marginal keratitis. We describe a case report of PUK associated with the inflammatory condition of sarcoidosis.
