The introduction of oral pharmacotherapy dramatically changed practice patterns in erectile dysfunction. The direct effect was to decrease the numbers of patients seeking penile implants; an indirect effect has been the changing ratio of new, to re-do operations. Patients seeking replacement inflatable penile prosthesis (IPP) surgery pose challenges both in diagnosis and management; in very select cases preoperative MRI can be useful in the evaluation of the patient's complaint and in planning operative management. Imaging can supplement the physical exam; in certain cases imaging will reveal: crural herniation, corporal distortion, corporal fibrosis, and hardware migration. Inflatable penile prosthesis has a high rate of satisfaction for patients (89%) and partners (70%). The principal reasons for dissatisfaction are penile shortening, pain and frustration with reoperation. If pain is not due to infection, it may be secondary to malpositioning, improper sizing, cylinder cross-overs, or herniation. After one or more reoperations, penile distortion may occur secondary to tunica albugenia thinning or fibrosis. A series of cases will be presented to highlight the utility of MRI for IPP problems, define normal appearance and abnormal appearance of penile hardware. Careful collaboration between the Urologist and Radiologist in the review and final dictation of these cases is warranted to properly document problems and to plan device replacement/penile reconstruction.

This investigation was aimed at the study of age changes of penis connective tissue structures. Tissue fragments of penis were obtained from 20 cadavers of men at the age of 20-38 years in group I, and from 20 cadavers of men at the age of 41-59 years in group II. The criteria for the exclusion of material from the research were arterial hypertension, diabetes mellitus, atherosclerosis of internal iliac arteries, Peyronie's disease, and anomalies of genital organ development. It was shown that in the cavernous body of penis, aging was associated with the increased amount and thickening of collagen and argyrophilic fibers, decreased content and thinning of elastic fibers, and the reduced amount of smooth muscle cells (SMC). The average area of fibroblast and SMC nucleolus was not different in both groups studied. The average area of endotheliocyte nucleolus was equal to 1.9+/-0.9 microm2 in group II, being lower than that one in group I, in which this index was equal to 2.1+/-0.9 microm2. No differences in the content of type III and IV collagen were found between the study groups. Age-associated decrease in the average area of endothelial cell nucleolus in the cavernous bodies may reflect the reduction of the activity of these cells and may indicate the development of endothelial dysfunction, which is one of the most important steps in the morphogenesis of age-related male erectile dysfunction.

The aim was to adapt and validate the Modified Transplant Symptom Occurrence and Symptom Distress Scale (MTSOSD-59R) for kidney transplant recipients undergoing immunosuppressive therapy in Korea. The MTSOSD-59R has been used with solid organ transplant recipients globally to assess the adverse effects of immunosuppressive medication. A descriptive cross-sectional design was used. MTSOSD-59R was first translated, and pilot tested. Next, content validity was established with nine organ transplant experts. Then, from October 2017 to October 2018, the Korean MTOSOSD-59R was administered to a convenience sample of 122 kidney transplant recipients recruited from a single center. Ridit analysis was used to measure symptom occurrence and distress. The known-group approach was used to test the construct validity using Mann-Whitney U tests for between-group comparisons. The content validity index for MTSOSD-59R was 0.98, and known-group validity was confirmed. The split-half Spearman-Brown corrected reliability coefficient was 0.902 for symptom occurrence and 0.893 for symptom distress. The four most frequent and distressing symptoms were fatigue, lack of energy, thinning hair, and erectile dysfunction (male). Results suggest this Korean MTSOSD-59R adaptation has adequate language, construct validity, and reliability to gather meaningful information from kidney transplant recipients in Korea.
