We examined three patients who had dermatologic and biochemical manifestations of porphyria cutanea tarda and localized thinning or excavation in the sun-exposed interpalpebral sclera, adjacent to the cornea. All three patients had signs of acute scleritis. The acute scleritis responded to oral indomethacin in one patient, but systemic corticosteroids were required to control the inflammation in the other two patients. Phlebotomy, protection from sunlight, and refraining from alcohol played an important part in the treatment of the patients.

Keratoconus is pathological condition of eye characterized by paraxial stromal thinning leads to corneal surface distortion. It is well documented that the corneal thickness is abnormally altered in keratoconus; but it seems that racial differences are exist in the amount of changes. This study compared the central and peripheral corneal thicknesses (CCT and PCT, respectively) in eyes with and without keratoconus in a large Iranian population. CCT and PCT were measured by ultrasonographic pachometry in ten thousands eyes undergoing refractive surgery during a six-year period (2003-2010) in Tabriz Nikookari Teaching Eye Hospital. These two variables were compared between the eyes with and without keratoconus. Two hundred and two eyes out of ten thousand eyes were diagnosed with keratoconus. The mean CCT and PCT were significantly lower in the eyes with keratoconus comparing with the others (CCT: 479.90 +/- 28.07 microm vs. 534.12 +/- 39.35 microm; p < 0.001; PCT: 515.04 +/- 148.44 microm vs. 560.78 +/- 32.93 microm; p < 0.001). This study confirmed that both CCT and PCT are lower in the eyes with keratoconus. Likewise, due to large sample size, the results can be used for epidemiological purposes.

Purpose: To report a case of two siblings affected by porphyria cutanea tarda (PCT) presenting with varying severity of ocular disease and evaluation of difference in the presence of risk factors. Methods: Case report Result: A 42-year-old Indian male diagnosed with PCT and history of daily intake of alcohol, presented with sudden decrease in vision of right eye. Examination showed perforated cornea surrounded by rim of 360-degree avascular necrotic sclera, with no evidence of infection. Similar systemic history was present in his elder brother. He did not consume alcohol and presented with mere localized scleral thinning. Since the patient's sibling acted as a natural control for the evaluation of alcohol as an independent risk factor, the difference in severity of disease can be explained on the basis of difference in alcohol intake. Conclusion: In the presence of progression of the disease, other risk factors like intake of alcohol should be evaluated.
