Transscleral Nd:YAG laser cyclophotocoagulation was performed on 100 consecutive patients. A contact lens designed specifically for this operation was used, and the results were compared to those of a previously reported series of 100 patients in which the same procedure was performed without the lens. The lens provided intraoperative advantages of eyelid separation, compression and blanching of the conjunctiva at the treatment site, and precise measurements for placement of the laser applications. Early postoperative advantages were reduced conjunctival burns and less hyperemia. However, the long-term results were comparable between the two series, with the exception of a higher incidence of phthisis when the lens was used. The thinning and blanching of the conjunctiva may increase the percentage of laser energy reaching the ciliary processes, which suggests a need for reduced energy levels when using the lens.

A multicenter clinical trial of therapeutic ultrasound for the treatment of glaucoma included 20 centers in the United States in which 1,117 treatments were performed on 880 eyes. The study was limited to patients with refractory glaucoma who had not benefited from conventional medical and surgical techniques. Approximately 782 of 1,117 treatments (70%) showed an initial decrease in intraocular pressure from a pretreatment mean of 38.1 mm Hg to 22 mm Hg or less. By Kaplan-Meier survival analysis, the single treatment success rate (intraocular pressure between 6 and 22 mm Hg) was 48.7% at six months posttreatment. When retreatment was used subsequent to failure, the one-year multitreatment success rate was 79.3%. The most common complications were an immediate posttreatment intraocular pressure increase lasting a few hours and mild iritis. Other complications included scleral thinning in 28 of 1,117 treatments (2.5%) and phthisis bulbi in 12 of 1,117 treatments (1.1%).

Seventeen eyes of thirteen patients with Mooren's ulcer were treated with a combination therapy of local and systemic steroids, conjunctival resection, superficial keratectomy and application of cyanoacrylate tissue adhesive. The pathology was classified as acute, subacute and chronic. Ulcers were graded based on the extent of corneal thinning, degree and extent of ulceration, and amount of inflammation. Fourteen eyes (82.4%) healed completely with formation of a vascularised scars, while three eyes (17.6%) failed to respond to treatment and either went into phthisis bulbi or healed with gross tissue distortion. Our study suggests an early intervention of this therapy with cyanoacrylate tissue adhesive application for effective control of Mooren's ulceration.

Ultraviolet radiation (UVR) is a risk factor for the development of ocular disease in humans, including acute photokeratitis, chronic corneal spheroidal degeneration, and cataract formation. This report describes the ocular lesions seen in 21 mice chronically exposed to UVR as part of a skin carcinogenicity study. All globes were affected to varying degrees. The primary lesion, not previously reported in UVR-exposed mice, was marked loss of keratocytes relative to age-matched controls. Secondary lesions included corneal stromal thinning, keratoconus, corneal vascularization and fibrosis, keratitis, globe rupture, and phthisis bulbi. In addition, more than 90% of UVR-exposed and unexposed lenses had evidence of cataract formation; this is the first report of the occurrence of spontaneous cataracts in 129 mice. In a subsequent study, apoptotic cells were identified histologically and by cleaved caspase 3 immunoreactivity in the corneal epithelium and, less commonly, in the corneal stroma after acute UVR exposure. Based on this finding, we propose that the loss of keratocytes observed in the chronic study was due to UVR-induced apoptosis.
