Characteristic findings of depressed early gastric cancer were summarized by neoplastic histological type according to the roentgenographic findings as follows: Undifferentiated cancer: The depressed surface shows many irregular changes and granular shadows of various sizes. The depressed edge is clear and shows a coarse or small wavelike pattern. The mucosal folds of the stomach are often interrupted by the depressed edge or show sharp thinning. Differentiated cancer: The depressed surface shows very few irregular changes, and is rather squamous compared with undifferentiated cancer. The depressed edge is fine and shows a thornlike pattern. The margin shows spindle or low IIa-like protuberance findings. The mucosal folds show slight thickening at the margin and gradual thinning at the depressed site. Thus, the folds are rarely interrupted at the margin.

When 58 minute gastric cancers less than 5 mm in diameter from 55 patients were classified into 22 of the single group (minute gastric cancer alone) and 36 of the multiple group (associated with other large gastric cancers), the preoperative correct diagnostic rate by x-ray was 22.7% and 11.1% in the single group and in the multiple group, respectively, with a total rate of 15.5%. The diagnostic rate by endoscopy, aided by endoscopic biopsy, was 95.5%, 13.9%, and 44.8%, respectively. Therefore, it appears that endoscopy and endoscopic biopsy are most efficient diagnostic tools for the detection of such minute gastric cancers. Since the detection of the depressed type (IIc) of minute gastric cancers is considered most significant because of their frequent submucosal invasion, their characteristic endoscopic findings are emphasized: (1) irregular and polygonal shape, (2) distinct depression, (3) clear demarcation, (4) nodular margins, and (5) moth-eaten appearance and abrupt thinning of the mucosal folds.

We report a 41 years old man with rapidly growing and tender lump on the anteromedial surface of tibia. The patient had the history of gastrectomy and gastrojejunostomy due to gastric carcinoma. On admission, the Simple X-ray of lower extremity disclosed a slight thinning of the anterior cortex of tibia without cortical destruction. The whole-body bone scan with (99m)TC MDP revealed activity of lesion in all 3 phases. The histopathological evaluation showed an infiltration of bone with tumor cells. Review No the literature revealed in previous cases of skeletal metastasis from gastric cancer in the tibia like this.
