Seborrheic keratoses in five patients were rubbed and biopsied at varying intervals after injury. Microscopic examination revealed acute and chronic patterns of histologic change. Hemorrhage, hyalinization of dermal papillae, and necrosis of epithelial tips were conspicuous early changes. Specimens taken more than 48 h after rubbing showed a spectrum of changes which included: loss of epithelial mass, expansion and interconnection of keratin cysts,
thinning
of the epithelium, proliferation of epithelial strands from the residual epithelium, increase in size of epithelial cells, and evidence of hair follicle relationships including trichostasis spinulosa and, in one specimen, hair germ proliferation. Dermal lymphocytic infiltration was variable and only rarely involved the epithelium. These observations demonstrate a patterned response of the
seborrheic keratosis
to trauma, and also indicate a relationship between
seborrheic keratosis
and the hair follicle.

Psoriasiform keratosis is a recently reported unique clinicopathological entity characterized by the appearance of a solitary lesion mimicking
seborrheic keratosis
, actinic keratosis, or squamous cell carcinoma and histopathologic features closely resembling psoriasis. We report a 74-year-old woman presenting with a solitary, scaly lesion on the dorsum of the left arm of several months duration and histopathologic findings of psoriasiform acanthosis of the epidermis with
thinning
of the suprapapillary plates and intraepidermal spongiform neutrophilic pustules, consistent with a diagnosis of psoriasiform keratosis. On additional work-up investigation, human papilloma virus type 6 was detected in the tissue examined by polymerase chain reaction technique, an association not described before.

Dermatologically, FGFR3 mutations can lead to acanthosis nigricans (AN), epidermal nevi, and
seborrheic keratosis
. A recent case report found that topical rapamycin (sirolimus) can improve FGFR3-induced epidermal nevi with AN features in children, specifically with Fitzpatrick skin type (FST) I/II, and we would like to expand these findings to skin plaques with extensive AN-like features in the FST IV/V adolescent population. An 18-year-old female with FST IV/V and FGFR3-induced hypochondroplasia presented to our clinic with extensive AN-like plaques. Significant improvement with lightening and
thinning
of the plaques was observed after applying 1% topical rapamycin cream twice daily. Topical rapamycin should be considered as a treatment option for AN, particularly in FST IV/V adolescents with FGFR3-induced AN.
