Nine children, aged one day to 10 years, presented with
gangrene
at the Paediatrics Department, University College Hospital, Ibadan. The aetiological factors included septic
skin spots
(4 cases), snake bite (1 case), femoral vein thrombosis (1 case), and congenital arterial block by embolized placental tissue (1 case). One patient had polyarteritis of undetermined aetiology, and another probably had vasculitis complicating a staphylococcal septicaemia. Two of the nine patients died. Five of the seven survivors had a significant physical handicap subsequently.

Calcification of the media of the arteries of the lower limbs, giving a linear type of calcification roentgenologically is typical for diabetic arteriosclerosis.
Spotty
calcification of the arteries of the lower extremity, which histologically is found in the intima, is also seen a little more often in diabetics with
gangrene
. There are no major differences between the other histological vascular structures of diabetics and arteriosclerotics. Maximal stenosis of the lumen is found more peripherally in diabetics than in persons with normal carbohydrate metabolism. The results favour the idea of predominant femoropopliteal occlusion in arteriosclerosis and popliteal-tibial occlusion in diabetics.

Diabetes mellitus is defined as an hereditary chronic metabolic disease due to an absolute or relative shortage of insulin. There are various types to be differentiated, among others, (I) the insulin-dependent and mostly juvenile type, and (II) the insulin-independent type, which mainly appears in later life. We discuss three main groups of cutaneous diseases. Infections of the skin and underlying tissue predominantly occur along with an acutely disturbed metabolic situation. The late diabetic syndrome includes macroangiopathy (arteriosclerosis) and microangiopathy (rubeosis, necrobiotic granulomas, diabetic
skin spots
, epidermolysis bullosa diabetica,
gangrene
). Owing to failure of sensible, motor, or autonomous fibers, diabetic neuropathy may lead to various symptoms - in the extreme case, to neuropathic malum perforans. The diabetic foot represents a collection of various diabetic complications in both early and late stages.
