Suicide has become a major concern of many Indian tribes and pueblos, as the rates in these tribes have increased dramatically in the last decade. One of the critical research questions is how to explain the vastly different rates of adolescent
suicide
among tribes. Research has identified some common patterns in experience and behavior among Indian adolescent suicides; these patterns are similar in many ways to those found in Los Angeles
suicide
research of Teicher (1979). Chronic versus
acute stress
factors in
suicide
are examined. Recent research has also identified a number of factors characterizing tribes with high
suicide
rates; these include failure to adhere to traditional ways of living, to traditional religion, and to clans and societies, and the resulting chaotic family structure and adult alcoholism. The roles of adoption of Indian children, boarding schools, and high unemployment in many tribes are also discussed. Suicide prevention and intervention programs are briefly described.

Numerous epidemiological studies report increased prevalence rates for women as compared to men for stress-related disorders such as
acute stress
disorder, post-traumatic stress disorder, and major depressive disorder. Stress disorders disrupt work and home life and pose a high risk for
suicide
. Multiple factors contribute to the increased vulnerability in women. Physiological differences account for some of the differential. Other factors that make a significant contribution to the overall risk for health problems in response to stressors or trauma include the nature and meaning of the trauma, accessibility of resources, and restrictive diagnostic categories. Increasing our knowledge of the individual impact of each factor as well as the interactions among the factors is central to understanding the development of stress disorders. Comprehensive sex- and gender-sensitive middle-range theory, which explores the role of key factors identified in qualitative and quantitative research, is required. The authors discuss structural equation modelling as one method of theory testing.

When two human corpses are found in temporal and locational context the assumption of a non-natural cause of death is close at hand. Suicide, extended
suicide
, homicide or accident (particularly with carbon monoxide) come to mind. When, however, a natural cause of death emerges for both after autopsy and further examinations, this provokes the question whether this incidence is coincidental. In our two presented cases, a married couple each was found dead together. In both cases, the wife needed nursing and was being cared for by a husband who died of cardiac disease shortly before the wife died. In one case, the wife died of acute uncompensated chronic congestive heart failure. In the other case, the cause of death of the wife was hypovolemic shock due to acute gastrointestinal bleeding from recurrent gastric mucosal erosion. It is to be discussed for both cases whether, given the pre-existing illness, the
acute stress
of the situation in view to the husbands' death may at least have favored the occurrence of death and may, thus, have led to the criminologically conspicuous situation of finding.

Classification of suicides is essential for clinicians to better identify self-harm patients with future suicidal risks. This study examined potential subtypes of
suicide
in a psychological autopsy sample (N = 148) in Hong Kong. Hierarchical cluster analysis extracted two subgroups of subjects in terms of expressed deliberation assessed by the Beck Suicide Intent Scale (SIS). The first group was associated with charcoal burning
suicide
, no psychiatric illness, indebtedness, better problem-solving ability, chronic stress, and higher overall SIS scores. The second group was associated with jumping from a height, psychotic disorders, psychiatric treatment,
acute stress
, and lower overall SIS score. The existence of a substantial cluster of subjects with lower expressed intent and preparation has important implications for the performance of the SIS as a predictive tool. Suicide prevention strategy may have to target potential subgroups with specific approaches.

Looking through the history, people have always been associating
suicide
with weather conditions, trying to understand and identify the relationship between meteorological factors and
suicide
. The aim of this study was to determine and analyze the meteorological conditions in the time of attempted or committed suicides, and examine the possible link between the changes of meteorological factors and the frequency of suicidal behavior. Retrospective study of pairs covered the period from January 2003 to January 2006. Examinees included in the study were persons who committed or attempted
suicide
in the region of Mostar. Meteorological factors included the days of attempts or committing of
suicide
, with meteorological factors of the day immediately prior to the days of attempts or committing of
suicide
, as well as with average monthly values of meteorological factors. Meteorological factors of the days with suicidal behavior were similar to the day prior to the days with suicidal behavior, but there was significant difference between meteorological factors of the days with suicidal behavior and average monthly values of meteorological factors: maximum pressure was significantly lower in the days with suicidal risk, as well as pressure gradient. Regarding the seasonal periods, examinees most frequently attempted to commit
suicide
in April. Results indicate that meteorological factors do not act as an
acute stress
factor for
suicide
behavior, but its change over time may be the trigger for a suicide attempt.

It has been reported that allergies are associated with depression and possibly
suicide
in women. Aggression is an important behavioral component that predisposes depressed individuals to suicidal acts. In the present study we examined the relationship between allergies and aggression to determine a potential contribution of allergies in factors of risk for suicidal behavior. Because stress plays a critical role in the manifestation of clinical symptoms of allergies and also in suicidal behavior, we also studied the role of
acute stress
. Female inbred Brown Norway rats known for their susceptibility to respiratory allergies were sensitized and challenged with a mixture of tree pollen and evaluated in the resident-intruder test for detection of aggressive behaviors. They were also subjected to
acute stress
by sessions of inescapable forced swimming and re-evaluated in the resident intruder test. Animals made allergic to tree pollen and subjected to
acute stress
displayed increased aggressive-like behavior as compared with control-saline treated animals or to their own aggressive scores previous to the stress session. These results suggest that allergies and stress increases aggressive-like behavior, indicating that these conditions may be important factors promoting altered emotional reactivity with the potential to influence suicidal behavior.

Over 1.9 million U.S. veterans have been deployed to the Middle East since 2001. Statistics showthatwhentheyreturn, a significant number of them will develop psychiatric/psychosomatic disorders. Many of these returning veterans will be seen in primary-care offices or at hospitals. It is important for physicians to be familiar with combat trauma, post-traumatic stress disorder (PTSD), traumatic brain injury (TBI), and
acute stress
disorder (ASD). Best standard of care, pharmacological treatment of war trauma and its comorbidities-depression,
suicide
, aggression, addiction and other disorders are reviewed. As part of the treatment protocol, the author has also designed a 12-Step Self-Help Proposal for combat veterans with PTSD, inspired by addiction and AA self-help programs. It is an experimental design which has helped many patients but needs further research to confirm the benefit. To the best of my knowledge this is the first time that a 12-step program has been developed forwartrauma soldiers.

Immune functions in the brain are associated with psychiatric illness and temporary alteration of mental state. Microglia, the principal brain immunologic cells, respond to changes in the internal brain milieu through a sequence of activated states, each with characteristic function and morphology. To assess a possible association of frontal white matter pathology with
suicide
, we stained autopsy brain tissue samples from 11
suicide
and 25 nonsuicide subjects for ionized calcium-binding adapter molecule 1, cluster of differentiation 68, and myelin. Groups were matched by age, sex, and psychiatric diagnosis. We classified ionized calcium-binding adapter molecule 1-immunoreactive cells based on shape, immunoreactivity to cluster of differentiation 68, and association with blood vessels to obtain stereologic estimates of densities of resting microglia, activated phagocytes, and perivascular cells. We found no effect of psychiatric diagnosis but 2 statistically significant effects of
suicide
: 1) The dorsal-ventral difference in activated microglial density was reversed such that, with
suicide
, the density was greater in ventral prefrontal white matter than in dorsal prefrontal white matter, whereas in the absence of
suicide
, the opposite was true; and 2) with
suicide
, there was a greater density of ionized calcium-binding adapter molecule 1-immunoreactive cells within or in contact with blood vessel walls in dorsal prefrontal white matter. These observations could reflect a mechanism for the stress/diathesis (state/trait) model of
suicide
, whereby an
acute stress
activates a reactive process in the brain, either directly or by compromising the blood-brain barrier, and creates a suicidal state in an individual at risk. They also indicate the theoretical potential of imaging studies in living vulnerable individuals for the assessment of
suicide
risk. Further studies are needed to investigate specific phenotypes of perivascular cells and blood-brain barrier changes associated with
suicide
.

In-custody deaths have several causes, and these include homicide,
suicide
, natural death from chronic diseases, and unexplained death possibly related to
acute stress
, asphyxia, excited delirium, and drug intoxication. In some instances, these deaths are attributed to undefined accidents and natural causes even though there is no obvious natural cause apparent after investigation. Understanding these deaths requires a comprehensive investigation, including documentation of circumstances surrounding the death, review of past medical history, drug and toxicology screens, and a forensic autopsy. These autopsies may not always clearly explain the death and reveal only nonspecific terminal events, such as pulmonary edema or cerebral edema. There are useful histologic and biochemical signatures which identify asphyxia, stress cardiomyopathy, and excited delirium. Identifying these causes of death requires semiquantitative morphologic and biochemical studies. We have reviewed recent Bureau of Justice Statistics on in-custody death, case series, and morphological and biochemical studies relevant to asphyxia, stress cardiomyopathy, and excited delirium and have summarized this information. We suggest that regional centers should manage the investigation of these deaths to provide more comprehensive studies and to enhance the expertise of forensic pathologists who would routinely manage potentially complex and difficult cases.

Suicide remains a significant public health problem for the United States military. Trauma-related diagnoses such as
acute stress
disorder (ASD) or posttraumatic stress disorder (PTSD) may exacerbate
suicide
risk, particularly among service members psychiatrically hospitalized following
suicide
-related events. To date, treatments to address
suicide
risk and trauma symptomatology among service members within inpatient milieus have been nonexistent. To address this gap, a randomized controlled pilot trial of Post-Admission Cognitive Therapy (PACT) was conducted to evaluate a targeted cognitive-behavioral program among traumatized military personnel (N = 36) hospitalized following a recent suicide attempt. All participants met criteria for ASD or PTSD and were randomly assigned to receive either PACT and enhanced usual care (PACT + EUC) or EUC alone. PACT consisted of six 60- to 90-min individual psychotherapy sessions, adapted from Brown et al.'s (2005) cognitive therapy protocol for
suicide
prevention. Blinded follow-up assessments were conducted at 1-, 2-, and 3-months postpsychiatric discharge. The primary outcome was days until repeat suicide attempt. Secondary outcomes included depression, hopelessness,
suicide
ideation, and PTSD symptoms. Participants did not significantly differ in reattempt status. However, based on reliable change index analyses, a greater proportion of PACT + EUC versus EUC participants met criteria for clinically significant change on measures of depression (100% vs. 78%), hopelessness (83% vs. 57%), and PTSD symptom severity (100% vs. 38%), but not for
suicide
ideation (60% vs. 67%). PACT is an innovative inpatient protocol, currently under evaluation in a well-powered multisite RCT for its efficacy in reducing subsequent suicidal behaviors. (PsycINFO Database Record
