The internal contamination with depleted uranium (DU) isotopes was detected in British, Canadian, and United States Gulf War veterans as late as nine years after inhalational exposure to radioactive dust in the Persian Gulf War I. DU isotopes were also identified in a Canadian veteran's autopsy samples of lung, liver, kidney, and bone. In soil samples from Kosovo, hundreds of particles, mostly less than 5 microm in size, were found in milligram quantities. Gulf War I in 1991 resulted in 350 metric tons of DU deposited in the environment and 3-6 million grams of DU aerosol released into the atmosphere. Its legacy, Gulf War disease, is a complex, progressive, incapacitating multiorgan system disorder. The symptoms include incapacitating fatigue, musculoskeletel and joint pains, headaches, neuropsychiatric disorders, affect changes, confusion, visual problems, changes of gait,

, lymphadenopathies, respiratory impairment, impotence, and urinary tract morphological and functional alterations. Current understanding of its etiology seems far from being adequate. After the Afghanistan Operation Anaconda (2002), our team studied the population of Jalalabad, Spin Gar, Tora Bora, and Kabul areas, and identified civilians with the symptoms similar to those of Gulf War syndrome. Twenty-four-hour urine samples from 8 symptomatic subjects were collected by the following criteria: 1) the onset of symptoms relative to the bombing raids; 2) physical presence in the area of the bombing; and 3) clinical manifestations. Control subjects were selected among the sympotom-free residents in non-targeted areas. All samples were analyzed for the concentration and ratio of four uranium isotopes, (234)U, (235)U, (236)U and (238)U, by using a multicollector, inductively coupled plasma ionization mass spectrometry. The first results from the Jalalabad province revealed urinary excretion of total uranium in all subjects significantly exceeding the values in the nonexposed population. The analysis of the isotopic ratios identified non-depleted uranium. Studies of specimens collected in 2002 revealed uranium concentrations up to 200 times higher in the districts of Tora Bora, Yaka

, Lal Mal, Makam Khan Farm, Arda Farm, Bibi Mahro, Poli Cherki, and the Kabul airport than in the control population. Uranium levels in the soil samples from the bombsites show values two to three times higher than worldwide concentration levels of 2 to 3 mg/kg and significantly higher concentrations in water than the World Health Organization maximum permissible levels. This growing body of evidence undoubtedly puts the problem of prevention and solution of the DU contamination high on the priority list.

In case new diagnostic procedures for Alzheimer's dementia (AD) appear, Nuclear Medicine (NM) would like to be aware of them in order to evaluate its own contribution to diagnose AD by SPET and PET brain studies. Recently, sleep disturbances were studied in AD and tend to be diagnostic for early AD. In AD the actual time of night sleep was found to be 5.7 h, while awakeness time for the same night sleep increased to 2.7 h. Also in AD, the REM and the slow wave stage (SWS) during sleep are shorter and hypopnea and apnea phases are abundant. Internal body temperature during night sleep is only slightly increased in AD, while in temporofrontal dementia and in normal individuals this increase is significant. The circadian rhythm of melatonin is disturbed in AD. The normal duration of inspiration and expiration during daytime which is reversed during normal night sleep, has not been studied in patients with AD. However, this reverse condition favoring inspiration is expected to provide more oxygen to the brain. Chronic but not acute stress causes

and is currently being studied by us as a possible causative factor for
memory loss
in AD. Tomographic SPET and PET brain studies can locate the site of brain damage in AD. This is important since memory has recently been classified into four categories, namely episodic, semantic, procedural and working memory. In early AD only procedural memory remains intact. This means that these patients may drive a car, do computer word processing and play some games at home or/and in the field. This memory is located in specific nuclei in the cerebellum and the occipital frontal area which do not relate to sites of other kinds of memory. This difference could be well identified by tomographic SPET or PET studies. Thus NM may also diagnose the early stage of AD. Another issue refers to the indications that the unified Medicare and Medicaid system in the USA has issued on September 15, 2004 for performing a PET (18)F-FDG study for AD. This preliminary study determined if subjects with
memory loss
problems demonstrate changes in memory and cerebral blood flow (CBF) after a simple 8-week meditation program. Fourteen subjects with memory problems had an IV inserted and were injected with 250 MBq of Tc-99m ECD while listening to a neutral stimulus CD. They then underwent a pre-program baseline SPECT scan. Then subjects were guided through their first meditation session with a CD, during which they received an injection of 925 MBq ECD, and underwent a pre-program meditation scan. Subjects completed an 8-week meditation program and underwent the same scanning protocol resulting in a post-program baseline and meditation scan. A region of interest (ROI) template obtained counts in each ROI normalized to whole brain to provide a CBF ratio. Baseline and meditation scans and neuropsychological testing were compared before and after the program. The meditation program resulted in significant increases (p< 0.05) in baseline CBF ratios in the prefrontal, superior frontal, and superior parietal cortices.
